    Using sunscreen daily is what made the biggest difference in my Melasma. It's faded to a point of almost not being visible unless you look really close in bright light. Think about applying sunscreen like brushing your teeth. It's a daily routine. Some people tell me "I don't even go outside"- yeah, but you have windows in your house, don't you? Unless you live in a basement you need sunscreen. It's the visible light not just the UVA & UVB - so make sure to use a good bright spectrum one that feels good so you will use it.

    Please do the video on oral tranexamic acid!!! Have been successfully managing melasma after kids since 2020 and do well with SPF, tretinoin/azelaic acid, and short term hydroquinone every fall. But I’m so curious about this medication!

    A few tips for warmer weather: invest in a portable fan that goes around your shoulders during peak hours of warm weather outside. Also avoid metal framed sunglasses as they can draw heat to the face and cause pigmentation as well.

    My husband has had that, what looks like melasma on the side of his neck and top of his head. He hasn’t been diagnosed but he says he’s had it all his life, comes and goes. It’s very hard for me to convince him to wear sunscreen all the time, oh well. but he’s gotten better.

    Melasma is an awful curse- got it a few years ago only on my upper lip- NOT where you want it. Was never pregnant- no idea what caused it. In my mid-late 40s. Can’t use oral tranexamic acid due to a past pulmonary embolism. Have been using Tretinoin, hydroquinone, and desonide but it’s such a pain.

    Would love to hear about Tranexamic Acid in pill form. I've heard about it, but am concerned about any of the serious side effects.

    Thank you for this video! I am currently on a treatment plan with my dermatologist that includes topical Tretinoin and Hydroquinone and oral Tranexamic acid. I was just reading about the link between HRT and melasma. Thank you for confirming this, as I may need to talk to my dermatologist about getting off of this to improve my chances of getting rid of this. This was such a helpful video!

    I have also incorporated looking at the UV index on my weather app. I try to avoid being outside (within reason, of course) during peak UV hours.

    I do all that you recommend. No topicals work. My hormones are in the normal range. I stay out of the sun. I stay away from windows. I wear UV bluestone visors. I use UV umbrellas. I don't let water or snow reflect on my face. I don't wear hats as they make me hot. I wear neck fans to cool down. I put blue light shields on all of my devices. I don't get steam on my face from cooking or unloading the dishwasher. I don't have hot showers. I'm not on any BC. I don't exercise. I literally do nothing in order for my melasma to not spread, but it still does. And the places I got melasma do not budge or fade. I apply 4 layers of sunscreen in the morning both chemical and mineral. Both with a SPF 40 or 50 P++++. All I apply is sunscreen no other make-up. If I have to scratch my face I apply more sunscreen to do so. My doctor won't let me go on oral tranexamic acid pills due to blood clot risks. Melasma has made me more depressed than all of my other autoimmune diseases (which do not contribute to my melasma). I cannot do activities with my child because of melasma. I'm a bit emotional here, but to clarify I do activities with my son. I just have to be careful when it comes to outdoor activities. If I cannot partake than my husband subs in for me. But I feel like I am missing out. I just want to be free from all of the health crap. I don't hang out with friends because of my melasma. No one that I know has melasma. This disease is devastating and controlling. NOTHING WORKS!

      I’ve never felt more understood in my life! I’ve tried it all, nothing works. It controls every aspect of my life and has caused major depression. Nobody understands how devastating it is to wake up with this condition everyday.

      Sunscreen all the time and till have on my upper lips. Was on vacation and layers tons of sunscreen and came back with darker melasma on my upper lips. Just saying.

      She mentioned that being pregnant can cause melasma. I have 4 kids, and for each pregnancy, I have the most beautiful skin. It's a struggle. I don't know what I really believe in.

      I completely understand how it feels. I’ve tried everything you’ve mentioned, spent a lot of money on promising treatments and cosmetics, but nothing has changed. I can’t go out without makeup, and it’s hard to cover. So many times, people have asked why my skin looks burned just because they see the patches. I don’t know… the next step, I think, is to accept that I’ll have to live with melasma and try to embrace it. But honestly, I’m struggling to do that.

    Useful information. Dermatologists FAILED to provide me adequate supervision in use of prescribed medicinals. Clearly they lacked knowledge or were just lax. Now I know!👍🏼👍🏽👍🏻

    I used musely for over a year and all it did was create blotches. Plus an allergic reaction to retinoid that they never helped me through… seems like it worked better on ppl with paler skin and not olive skin like me. Since then I’ve only been using sunscreen and finding shade but I need more. I’m scared to try retinoids again but it seems like I should. Thanks so much for this video.

    You mention hormones that drive melasma. Are there hormones that can improve melasma? I had a dermatologist tell me that the root of melasma is actually hormonal. So I’m wondering, if that is the case, could people with melasma be deficient in certain hormones that prevent melasma? Would balancing one’s hormones potentially improve the appearance of melasma?

    Thank you. So informative! Just wondering if oral spironolactone can worsen melasma? I watched your video about spiro and it seems like a promising acne treatment but there was no mention of melasma so I was just wondering. Birth control did wonders for my acne but made the melasma worse so I stopped it. As predicted my acne came back with a vengeance but my melasma improved. It's so tricky having both acne and melasma :( It's like picking the lesser of the two evils.

    I never knew I had melasma till I got an IPL treatment. Lots of the pigment flaked but I ended up with a 1cm very dark patch on the central face which I still deal with years later (and I had never really noticed before IPL). Likely it was the heat.
    What dose of oral tranexamic acid do you use for your patients?

    Are you familiar with Thiamidol? An ingredient from Eucerin to treat hiperpigmentation?

      It's very effective - I use products with thiamidol since a year with great results. You can use it 4 times a day and add to routine with retinoids, vitamin C and all other effective melasma treatments.

    2. Not having a long-term tretament strategy
    3. Not wearing sunscreen
    4. Not wearig enough sunscreen1/4 tsp for the face
    5. Relying exclusively in sunscreen (wide brim hats)
    6. Treating melasma while being on birth control
    7. Not using prescription topicals
    8. Not considering oral trenaxemic acid
    9. Not avoiding excess heat and blue light (not phones, sun emmited)

    I have had good results from the Melanage peel in addition to tretinoin and 8% hydroquinone and cycles of Trenxiemic acid orally

    I developed melasma very recently for the first time in my life. On both cheeks. It literally appeared overnight. I woke up one day and it was there in full force. It didn’t develop gradually, and it hasn’t gotten any worse so far. I am Indian and 66. I have never had children or been on birth control or HRT. It looks awful.

    I've had an under eye patch for ~5 years. Nothing helps. And I think it started to spread. I've used 20% azaleic acid for 3 years nightly. It helped me with other things but not with this... I've used many acids since then because of acne, to keep my skin relatively discoloration-free (and of course spf50 always).
    I thought it happened because I wear glasses and the spf might have been wiped off when I sweat. But I also had thyroid issues flare at the time it showed up. Or once I used a self-tanner on my face which was a stupid idea. I thought there's a way to get rid of it.. well... at least now I'm not delusional about it lol.
    I guess I could get better with my uv protection... But when I'm biking, and I'm sweaty, and I need to wear my glasses (or sunglasses even if I use lenses), with my makeup on (acne scars..).. I can't wear a hat because I'm already wearing a helmet (with a peak but I don't think it helps). It's the biggest struggle for me. I use a thin layer of a tissue to suck out the sweat and then use spray spf 50 during the day but I don't feel it's helping in that particular area where the glasses touch my skin. I'm starting to wonder if I should just put a bandaid on that patch for situations like that.

      I have sun spots and melasma and use an LED mask. Mine has not gotten worse. I’ve been using it for a year. I have the current body LED mask. I know you asked her, but I thought I’d comment about my experience. 😊

      ​@@itrinhJust keep an eye on the pigmentation daily. I used red light therapy and had it worsen very quickly. I'm in a skincare group where about 50% of the people had to stop using red light. Every person is different so I would just recommend being mindful of your pigmentation.

    I have Melasma on my forearms. I wear sun sleeves and sunscreen. I do not do anything else. It does not bother me since I do not have it on my face yet.

    I understand the clinical findings dont support blue light to be a trigger ror fair individuals but that has NOT been my personal experience. I get rosacea and pigment flares not just from devices but also just sitting next to a lamp. Fitzpatrick type 2 and its a constant struggle FR 😔😕

      It’s great! Similar to benzoyl peroxide, it can kill odor-producing bacteria.

    I'm wearing sunscreen on a daily basis even though I'm just at home, but it does nothing to protect me from darkening melasma. I just went swimming at the beach one day and my face was covered with 50+++ UVA/UVA sunscreen
    For a one day swimming my melasma is as black as ass now. And it's so difficult to lighten it again.

      A lot of times it’s triggered by the heat. Mine is always worse in the summer (and there’s some laser treatments my derm doesn’t recommend for those areas for me bc it the heat)

    I can’t believe people who can afford to go to the dermatologist wouldn’t wear sunscreen… like you’re probably spending more at the doc than you would on sunscreen. It’s so silly to me. I can’t afford it and I still wear it everyday. Why spend so much on treatments just to throw it away by not putting it on in the morning? Come on people! 😹

    I have my prescription in the drawer tretinoinand HQ mixed. My question is how often do I use it in a week, every day? I usually use tretinoin by itself so if I start this how do I incorporate it in my routine? What can be use the days that are not this actives?

    Chemical sunscreen Nivea uv face gave me melasma within seconds of being close to sun only the heat absorbed by the sunscreen marked my cheek within seconds.

      Did the sunscreen have the old generation of chemical filters? Older generation chemical filters may cause irritation, combined with this sunlight, may have caused hyperpigmentation.
