I've always loved Silent Hill 4 but seriously... Another part that always stood out as a "wtf guys" moment is a little later, at the bottom of the water prison, when all the twin victims are lined up perfectly, like the devs just Gmod copy/pasted their models... I'm interested to see what Vinny says when he reaches that part, he will no doubt comment of how out of place it is.
One sure thing is they didn't know how to animate and program falling monsters, they just made a "slide the model a bit with a burp until floor is flat". What a mess of a game lol
I’m almost positive this was intentional and they wanted to simulate characters rolling down the steps when they died, but it did not translate well or came out unpolished.

I'm not sure how true it is, however I have seen some claims online about Japanese people having a serious taboo concerning flatulence. It's possible that from the perspective of the person doing the sound design it may not read the same way. It would probably still be funny to an extent, but there's potentially another layer to it, kind of like how in Elden Ring the Loathsome Dung Eater is meant to be threatening precisely because he kills people by eating ass and sucking the life out of people through their anus, whereas to you or me that may just be laughable. That said, I think that it's definitely intended to be absurd when they fall down the stairs like that.

Thing is, i kinda get the idea they had. Bloated corpses full of rotten air inside of them due to decomposing is metal as hell. But even SH2 had the decency of having more than one sound effect for its enemies.
i agree it just needed an actual impact sound, and better timing. it also needed to be way less generic. a burp makes sense, but not one from a random sound effects cd. bespoke burpage was required. it needed to sound properly dark, and dead, and gutted. air exploding directly from wounds you’re making in the stomach, stuff like that. mix it up definitely

SH3 has such insane visuals and sound design, im sure this is why some SH4 fans are salty about 3 being clearly the favorite of the two lmao. So, when you first get there, you see Wally removing some organs from one of those nurses, presumably their lungs. Meaning there's no lungs to make sounds, so instead they can only expel gas from their stomachs. I get the theory behind why they burp, but it will still always be one of the funniest things.

Looking at the patient's model there's a clearly visible scar around her belly, and also I think her tits were cut out too? I think Walter just completely mutilated her aspects of feminity and motherhood. As for why they burp I feel it was because sound design in a lot of places was made in a rush. Remember dog's cheetah stock sounds, gumhead's monkey and bear stock sounds. Akira Yamaoka did use stock effects in previous games too but they were all combined and distorted beyond recognition. And considering that he was a producer, composer and sound designer for both Silent Hill 3 and 4 which were made in parallel, I think he probably just didn't have time to finalize the sounds.
There's so many stock sound effects in this one that I can't help but feel like they were just placeholders that ended up getting left in because they ran out of time.

There is visual design, sound design, methods for movement, lighting, building up tension, etc. If even *one* of these isn’t implemented well, the whole mood/atmosphere is ruined. It’s why we see more bad horror projects than good. It’s tricky! This video is a comical example of the fragility of the genre.

This may even tie them into Eileen; when the whiteboard with Eileen's x-rays is more clearly visible in a cutscene, the term "vomicae" is present, which is a condition where a pus-filled cavity caused by tissue decay, typically in the lungs, is found and causes the expulsion of the vile fluid. Perhaps the Patients are a vision of what Walter planned of Eileen's body to become.

The worst part is even in the isolation of this clip, the monster before you know what was genuinely intimadating. The way it just quietly walked over, Vinny running away, that strafe side to side while squaring up with it. It's a shame really. At least you can appreciate an equally genuine f*ck up, it's how you can know that something is real.

There's a scene in Zombie Vs Mardi Gras where a guy gets zombified while in the middle of shitting, and he walks around making fart.mp3 sounds. But that's not quite what you were asking, I just have PTSD from that movie
A plausible explanation would be that the zombies reanimate faster than the gas buildup could start, with the movement of their bodies forcing the gas out. That said, it would make sense that they would be burping (and possibly farting) throughout the entirety of their shambling, saddening existence.
If I remember correctly, this particular Silent Hill installment also has the Ed Edd N Eddy jaguar sound effect for when you stomp on the stunned demon dogs
