How to use a TENS Unit for Lower Back Pain Relief - Ask Doctor Jo

    Some common back injuries that may benefit from a TENS/EMS unit include things like muscle strains, herniated discs, stenosis, spondylosis, spondylolisthesis, and general arthritis pain.
    TENS (Transcutaneous Electrical Nerve Stimulation) is used to help reduce pain and increase circulation. Basically the vibration of the TENS follows the same pathway as the pain pathway to the brain and helps cancel it out. It is a great alternative to pain medication. A TENS uses two or four electrodes with cross currents to surround the area. The deep vibration/massage helps relax the muscles and allow for healing. You should not get a muscle contraction when using TENS.

      @@user-hh8ss4gn1n It depends on what stage of recovery you are in and how your body reacts to treatment, but in general, therapeutic exercises can be done 1-3 times a day, every day.

    I’ve had low back pain a long time and severe intermittent SI joint pain for more than a year. I had an TENS treatment at a physical therapist office. He then did a little massage followed by polychromatic light therapy and laser. The treatment was very effective in that I felt much better. The TENS felt something like a massage. I’m sold

    I just found your channel. all I can say is thank you. I am a disabled(functional quad) veteran and last summer my VA therapist got me an H-wave tens unit for my dislocated shoulder. for the past week I have been dealing lower lumbar back pain. everything my pain management dr could do made it better. but still had me down all week. I just happened along and seen your video on using the unit for lower back pain. and finally some relief

    This is very helpful, I just got a TENS unit because I've been having lower back pain from hauling heavy pallets across my work 6+ hours a night, I needed a video to understand the correct placement for the pads and this was very clear and right to the point

    Thanks for the informative session. Can you use that large pad for muscles areas like thigh or upper back? For treatment of general soreness from working out, no specific injury or pain.

    Hi there Dr, I have minor cervical and lumbar spondylosis. But most of my discomfort comes from my right deltoid area. I've been playing lots of sports which uses most of my dominant right hand, and the muscles on the right side are much tighter than the left. Would tens be suitable to loosen it up and get the muscles on both sides to be balanced? If so where should I place the electrodes?

    Hello, I have 4 spots of subluxation in my spine. I went treatment at my chiro 3 times a week for 14 weeks. My back felt better and now (6 months later), my back hurts again. I have a 1 and 2 year old so I don’t have time to go to the chiro and it’s very expensive. Will TENS help with my lower back pain ?

    If I had lumbar ddd and it was giving me sciatic pain then could TENS be an effective means of helping alleviate flare ups and the symptoms? Would i look to use the pads at the source of the ddd or target the areas the pain is radiating to i.e. my buttocks and down the back of my legs? Thanks.

    Hi Dr. Jo, just wanted to ask if this device can help my mother who has a cauda equina syndrome, she had the operation for the nerve compression last Feb 2017. Unfortunately, she was not able to recover her nerve damage specifically on her bladder and bowel control. So she is still using cathether as of now because she cannot control her urine. Just wodering if this device can help her nerve and muscles?

      The TENS is just for pain control. It does not strengthen anything. EMS units can help retrain muscles, so it might be worth looking into, but you should have her go see a physical therapist for that.

    Dr. Jo what kind of manipulation would a doctor do for t6, t7 area? I’m getting such back pain/pinched nerve on middle thoracic area that I seriously think it’s giving me gastritis Stomach problems. Have you heard of this area causing this? Thank you for all you videos ❤️

      The thoracic area is usually the easiest to manipulate. If you can, you should really go see a physical therapist. Has your doctor checked your gallbladder? Pain in the thoracic back with stomach issues can be a sign of your gallbladder not working properly.

      AskDoctorJo ....thank you for replying. ❤️. Yes my doctor has checked gallbladder, pancreas all came back fine. I don’t know what’s going on with my stomach and this pinched nerve in my back. I get full fast and heavy chest pain with nausea. It sucks! I’ll try the physical therapy and see what exercises they recommend. Thank you so much 😊 ❤️😊

      "Pig tail" pads with the push in pin are usually pretty universal. The snap ones often are, but sometimes they don't exactly fit.

      My precautions booklet for my tens unit says “we don’t know the long term effects, we recommend 30 minutes, if used again, use with a 3 hour gap”

    finally a video that answers most of my questions about tens units but not all. I have a 2 channel tens but i also have different cords. One channel cord has 2 pads on it and i have another cord with 4 pads on one channel cord. So what i have been confused with was if im using one channel with 4 pads, I place them mid back using the 4 pads on one channel cord and place two pads on each side of my mid back along spine. So what i mean is 2 running up and down about an inch apart on each side of spine. Is this OK? I have seen videos etc saying if i use 2 cords(2 channels) with 2 pads on each side that crossing them one on each side of the spine making an x pattern was Dangerous. Your showing its safe? I am so not trusting whats right or wrong. also im reading to NEVER place any pad directly down the middle of my spine, is that true or not either???? lol..so confused.

      With TENS, since there should not be a contraction of the muscles, it's generally okay to cross the spine since it's just stimulating the surface nerves. If EMS is being used where there is a muscle contraction, it should not cross the spine.

    Question, I'm in my clinicals for pta and they told me not to cross the tens unit on the spine.
    Doesn't this risk electrical stimulation of the spinal cord? Which in return may send an unwanted stimulation.
    However, you're the doc so any advice on this?

      The electrodes shouldn't cross the spine if it's NMES, but TENS is transcutaneous nerve stimulation, so it's stimulating the surface nerves.

      As long as you are cleared by your doctor first. In some cases, it shouldn't be used, so make sure to check.

      I have lumbar stenosis and tens has helped me at times. As you know stenosis can be extremely painful and the tens helps knock the pain down some.

      You're welcome. The same company has a really small one that fits in the palm of your hand. They also (and other brands as well) have wireless ones. They are a little more expensive, but then you don't have to worry about the bulk of the wires.

      All TENS/EMS units are slightly different, but they should have a little sheet of instructions that tells you which setting is which.

      I'm not sure why a doctor would've said that. Your settings will feel different. turn up the tens too high and it feels like a shock, ems contracts your muscles like a hard flex.

    Hi Dr Jo,
    I have ankylosing spondylitis and chronic low back pain (though the AS is apparently under control according to my rheumatologist, I'm not entirely convinced). Anyway basically the last visit to my physio I asked him about whether it was worth getting a TENS unit, he tells me there's no evidence for them and not to waste my money. My last visit to my GP he actually said to me to look into a TENS unit, so yeah I'm just completely frustrated and confused.
    I like my physio, he takes a scientific evidence based approach to treatment (not that a GP wouldn't in saying that). I guess I don't really have a reason not to trust what he has to say, I'm just as I said frustrated and confused. I don't actually know what I'm trying to ask of you in this regard, perhaps if you think it is something that is really worth trying?

      There is evidence that shows they work, so not sure what your physio is reading :-) I know from personal experience that they have helped me through all my orthopedic surgeries (5 of them), and I was able to use TENS and ice without using pain medication. I can't say for sure it will work for you, but they are so affordable, I think it's worth trying.

    I have this exact TENS unit... I had a super bad, huge infection the end of 2018 in my psoas muscle (spent 3 weeks in the hospital after being med-flighted to Honolulu)... it's left me major back and groin pain. I've seen images of 6 pads (4 on the back and 2 on the upper thigh of the side with the groin pain), but they aren't clear if the 4 pads in back are crossed, then the 2 showing in front are the same channel, or if they are crossed somehow. I can't seem to find anywhere that shows how the 6 pads are broken up into the 3 channels... help?

      If the 4 on the back are suppose to be covering the whole area, then they should cross. This is what makes it cover the area in between. So it could be two channels in an X pattern as seen in the video. As far as the one for the groin in the front, it would most likely be a completely separate channel, and can be controlled independently.

    My tens unit says not to cross the spine like you’re doing here. Is it okay to do?

      A TENS unit can cross over the spine, but not EMS. If there is any muscle contraction, you should not cross the spine.

    Hey doctor jo so I have a lot stiffness and tightness in my neck normally I'm a very happy confident person.But I find with this stiffness and pain I'm very quiet and have a lot unusual negative thought when in pain because it lasts for a long time when there's no pain the thoughts go away is this a common symptom of being in pain with stiffness for a long time just feeling bad and a bit negative in general :)

      It's possible if it's really painful. Have you been to your doctor yet to find out what is going on with your neck? The best thing to do is try to figure out what is going on so you can get it better.

      You can look through my reviews, but they all have different functions and are different prices, so you will have to pick the best one for you. TENS does not "cure" low back pain. It helps relieve the pain so you an do the stretches and exercises you need to do to get better. Good luck.

      You're welcome. No, they don't have to be exact. As I mentioned in my video, you can move them around to what works best for you. So your placement could be very different than mine.

      They don't have to be exactly in the same spot on each side. As long as the channels are crossing, the placement doesn't have to be exact. The TENS should help with pain and increasing circulation to the area, so it should help you recover, but you will need to do stretches and exercises as well. Good luck.

      Sometimes you have to either move the electrodes around a bit (especially if you are on an area that has less bulk), or try a different mode. Some of the modes do cause the muscles to do a quick contraction, but it should not be a sustained contraction. Good luck.
