I do have something i want to ask. I'm an Aussie citizen doing medical school in Pakistan. I am inclined towards coming to Australia after I graduate. I was confused on whether i should take the standard pathway or competent pathway (Uk) as i have heard that the latter are prefered doing job selection and training (usually). Would love your opinion!

Hi Anthony, thank you so much for the videos! I have a question, when the Board says 12 months of supervised practice, do they mean that you had to be training? or do they mean a valid working position? (like when you have a manager /superior etc) in NHS for example?
A working position where you can legitimately demonstrate you were working under supervision.
Thanks a lot for making these videos, I've been looking for forvr to find something useful on this topic. So, the question being, I'm studying medicine in EU (outside of UK), but have a huge wish of moving down under. I came up with two options and was wondering which one you thought would help me more: 1. do foundation programme in UK and move to Australia afterwards for specialty training and enter through the competent authority pathway OR go straight to Australia after medschool, complete all the tests and try to get a medical internship directly there (a different pathway)?
Hi TJ if the Foundation program is a good possibility for you. Then do this. It is not possible for an IMG to apply for the internship in Australia. Which is why so many find the Standard Pathway difficult.

So long as you can get the experience signed off by an authority it should be fine. I would imagine longer term Locums would be ok. But if you are doing short stints. Might be an issue.

Thanks for th explanation doctor. I don't see much of a difference between competent authority pathway and a non competent authority (eg UK vs India), as long as someone has got their assessment from their respective speciality medical college in Australia both the competent and non-competent authority are pretty much the same - just that the non competent authority will have to sit for an exam after the end of a year or two of their supervised position AND spend AUD4000 for their initial assessment from their college. My wife got a specialist registrar position in Australia from India. The BIG CAVEAT is that there are a very few positions for international candidates, and the open positions also depends on the type of speciality needed at that particular point in time (BASICALLY it is extremely tough to get a role as an IMG, you've to be extremely lucky to find a position for which a hospital will be willing to sponsor a visa - basically right place right time).

Thanks for the comments. Yes. It is tough for IMGs in certain specialties in Australia.
You are conflating between 2 separate issues which is the specialist pathway versus candidates who are eligible for competent authority or standard pathway. If you are eligible for the specialist pathway and competent authority then the registration is slightly different but you still need to pay for your college assessment like other SIMGs and go through the same process.
CA is definitely better than SP if you are coming here as a trainee doctor.
Hi Anthony, I am a US citizen who graduated from a Mexican medical school listed in both the AMC and the WDoMS. I have completed a USMLE steps 1 and 2 and will be taking step 3 (last part) soon. However, I haven't gone through a medical residency in the US; instead since graduating in 2016, I have been working under the supervision of licensed physicians in the US for 2+ years. Would I qualify for the provisional registration process under the competent authority pathway?
Hi Islam the Medical Board of Australia is quite clear that you need to have "Successful completion of a minimum of two years of graduate medical education within a residency program accredited by the Accreditation Council of Graduate Medical Education." As I understand it you did an internship residency in the US. If this was accredited by ACGME that would probably count towards one year. For your other experiences these will likely be not counted by the Board. I'd advise you to try and get into a residency program.

Some people recommend that instead of AMC 1 and AMC 2 exams it’s more rational to take Plab 1 & 2 and then get your GMC registration after one year of supervised work and then go to Australia through competent pathway
As the Uk 🇬🇧 exams are much cheaper and pass rates are also higher
I wanted to know what’s your opinion about this ??

Hi I personally don’t recommend this for a couple of reasons. It’s indirect and at the end of the day it’s really about Whether you are good enough to get a job here.
That being said. The PLAB is as you say cheaper and easier.
It probably depends on whether your primary aim is to work in Australia or you are looking at both Australia and the UK.
Thank you for your video Anthony.It is really useful. I've been reviewing the video, comments and information online but still unsure of some details regarding employment. To be fully registered under the competent pathway you need to have 12 months (47weeks) of supervised practice as I understand.
Does this need to be with the same practice/hospital for the entire duration? For example if the contract you're given isn't for the entire year and you need to find other jobs to do a years worth of full time.

You get registered for 12 months initially but it’s fairly easy to get a 12 month extension if you need it.

Yes. It means you outrank anyone on a visa for a job that you are eligible for.

I had a question about eligible work locations under the competent authority pathway. Through this pathway, are you restricted to work in only areas of need? Or are you able to work anywhere in the country? ie. in cities such as Sydney or Melbourne?
You can pick up a job anywhere. You will have more chances in the regions. You are not restricted so long as you are not working as a specialist.
@@CareerDoctor Thanks for the reply! It seems that Australia considers family medicine/general practice a specialty. If my training in Canada as a family doctor is approved under the specialist route, am I then restricted?
Yes GP is a specialty. Canadian GPs are highly sought after in Australia especially if you have rural experience.

Yes. But candidates from CA countries are preferred over candidates from SP countries by employers here. So you still have an advantage over a number of other IMGs.

Hey there. When I visit the medical jobs WA website, it says that positions for RMO applicants ( I am currently an intern in Ireland) doesn't open until may. How can this be? I hear some people that have applied have job offers already. Time is pushing on and I feel I am really missing the boat. I am looking for job offers on the website but I can not see anything!
Hi Thomas thanks for the question. There are usually two different sections on each of the state and territory drop boards. One section will have information about the annual medical recruitment campaign, which normally opens around May of each year.
The other section will have all the current vacant jobs. This is probably where other people are replying at the moment. You just have to keep an eye on the casual vacancies and see what comes up over time.

Hi Rabbiya you do the supervised training with whom ever offers you the position. The Board just reviews the plan and signs off on it if they are happy.

Hi Breno. For Australia you would need an English exam. The PLAB itself includes an assessment of English. So as far as I'm aware its not required.

If you are just applying for Aus then you need to tell EPIC to send it to the AMC.
I'm going to apply for a GMC full registration but I will not do the PLAB exam because I'm EU citizen. May I go through the Competent Pathway without the PLAB?
Sorry that’s not possible you have to do standard pathway in these circumstances
Hello. I have completed my internship outside the UK. I'm hoping to pass PLAB and then work at a FY2 non training post in the UK for 1 year.
(1) Is experience at a FY 2 "non training" post enough to be eligible for the competent pathway?
(2) If a FY 2 non training post is not enough, where do I need to work in order to be eligible for the competent pathway? ( FY2 training post or a FY1 post)

Generally most doctors in non training UK posts can still produce a supervisor report which will satisfy the Board here.
I have a question doctor!
I’m a European medical school graduate who moved to the UK (but I was fully recognized without having to do the PLAB due to the EU laws) and done GP training (3 years plus AKT , CSA and got the CCT and then the membership of the MRCGP UK) , do I need to do the AMC exams or can I apply and work as a GP in Australia because I obtained my GP training in a “competent authority” country ?

Hi Juba with the CCT for general practice it sounds like you would be eligible for specialist pathway for a GP post here.
You would not be able to go through competent authority.

I have been watching your videos for quite some time and and they are quite useful. I did watch this video but there are still some doubts I have which I would like to ask, they are as follows-
1) for the competent authority pathway(via PLAB) ill be needing to qualify PLAB-1+PLAB-2+ how many years of supervised training (foundation years)? one or two?
2) Once I get my provisional registration in Australia through competent authority pathway, will I be needing to do one more year of supervised training in Australia?
3) Is it possible to do my further studies in Australia? like say Cardiothoracic surgery etc? Or will I will only woking as a GP for the rest of my life?

1) you need a year of supervised training on top of your PLAB
2) you work for a year as a provisionally registered doctor in Australia 🇦🇺
3) once you complete your provisional year you get granted general registration and can apply for any training program
However see below
4) you don’t need citizenship or PR for the pathway but go for certain training programs
After general registration,will there be exam to get into the training or what do we say residency?
Some colleges have psychometric tests as part of their entry process. Most do not.
Do you need to clear just the steps (1 and 2) of USMLE or also some clinical experience in the united states as an IMG for the competant authority pathway. Because I've been thinking of keeping australia as a second option in case I dont match in US after taking the steps. Is that a viable option?
As explained in my video you also need to obtain the required amount of experience.
Hi Anthony, I'm from Ireland and I'm about to graduate from medical school in Bulgaria (EU)- so no need for PLAB. My plan is to work in the UK as an F1 & F2 standalone to get my crest form signed off. I really want to move to Australia for a couple years after this. Good day doctor, just a few questions and clarifications about the Competent Authority Pathway. I am a Filipino IMG and is a licensed physician in our country but I haven’t done any residency training. I’m planning to take PLAB 1 and 2 after which will undergo FY2 because I’ve done my internship already in the Philippines. My questions/clarifications are:
1. After taking FY2 (which I believe is a non training job in the UK), there would be a higher chance to secure a job in Australia compared to Standard Pathway. Am I right?
2. Doing FY2 in the UK doesn’t necessarily mean that I have to do it under NHS. Like I can do it in a private hospital as long as I have a certificate that I was supervised? If yes, do you have an idea if there is a specific document that I need to present proving my 1 year internship in the UK is supervised?
3. May I know the next steps after securing a job in Australia towards the residency pathway?
4. Can an IMG undergo a residency training in Aus or only Australian residents have the right to be trained in Australia?
5. If so, would getting a PR will help me land a training program in Australia?
6. Do you have any idea how long before an IMG will be able to acquire a PR?
7. I am planning to train under emergency medicine or any training of the same scope. Would an IMG have a high chance to be accepted in these kinds of specialization?

Hi Claire I’ll answer the first question. The process you have outlined including doing a FY2 job (which is normally supervised) would put you on track to competent authority for Aus.

Not sure I understand your question. If your medical degree is recognised by the AMC then you can sit the test.

Enter Australia after becoming a surgery consultant in the NHS
I’d say both are similarly difficult as you need to gain entry into training in either the UK or Australia.
Can you cover the process of becoming a doctor in the UK after completing medical school in Australia?
I believe there are specialties in Australia that are recognised in the UK, Canada and Ireland i.e. if you're a GP registered under the RACGP, you can also practice in the UK, Canada and Ireland.

Hi Ivan USMLE only counts if you have also done an accredited residency program for two years in US

How can GMC registered via MTI pathway along with fellowship training apply in Australia? Is it competent pathway? And can they apply for general registration directly?
The Competent Authority Pathway is not open to you. You would have options via the other 3 pathways depending on the rest of your circumstances.
Hello Anthony, I am an Australian who is in an Irish med school at the moment and I wish to complete my internship/FY1+2 in either Ireland or the UK. I think that I am eligible for the competent authority pathway once I finish training, but I am unsure if there will be any changes in the pathway in the near future. Also, I heard that IMGs are subject to a 10 year moratorium but it is rather arbitrary from what I was told. I heard that getting into training can be a nightmare for many as well, and as an IMG, I am possibly placed at the bottom of the barrel. EM is known to be relatively easier to get into training, but I am wondering about the difficulty for other specialties such as anaesthesiology, psych, paeds and IM. Derm, plastics, and surg specialties are definitely the most challenging ones though. Having permanent residency/citizenship is a must for applying to the surg college, and that prerequisite alone already scares me, even though I am a citizen.
I wonder what are my chances of working in a larger city for F3, as I am not really a fan of regional/rural Australia. I think most IMGs from UK and Ireland work in rural or regional areas initially to combat the shortage, but they are frankly not my favourite places in Australia. But it is best for me to not be picky and maybe stomach those few years. Perhaps it will be easier for me to get into my desired specialty. For some reason, ads of immigration to canada and NZ are plenty on instagram though 😂. The NHS strikes are crazy and it puts me off from working in the UK. Those countries are using this opportunity to fish UK and Ireland grads, must be intentional innit
OK. Lots of questions there. So I will do what I always do and answer your first question. Yes. You are eligible for competent authority once you gain your Certificate of Internship. I would not worry too much about the moratorium.
Various colleges including the Surgical colleges count working a year in a rural area as contributing 3 points to your application for training equivalent to getting a Phd. The moratorium also applies to people from New Zealand. You may get a position in a public hospital in the city but competition is fierce. Perhaps you can work a year or two in rural and then work your way in. The moratorium should not worry you if you plan to go into specialist training as getting on and doing the training will take the better part of 10 years.
@@beekiat I didnt know the moratorium also applies to ppl from NZ. The whole procedure seems to be extremely daunting and I don't know why so many UK/Irish grads are still romanticising the arduous system
If a IMG does MRCP, gets GMC registration and 1 year work experience in Uk, is he exempted from AMC?
No. Because you have not done the PLAB or completed a CCT.

I would say it’s unlikely. Is Hamad accredited as a residency program by the ACGME?

If you are able to apply for Competent Authority you should always do this. The main benefits are no AMC exams. And CA eligible candidates are preferred over SP candidates. So the timeframe for a job offer will be shorter.
Are you paid the same as Australian graduates at the same level if you are employed with provisional registration?
Yes. Sometimes more if they accept you overseas experience. This varies.

After doing PLAB if one works in non-clinical role (as functional assessor/ Disability analyst) for more than a year in which she is in constant interaction with patients and is supervised all the time, would it be considered as ‘one year supervised training’? No. That would not count.

Hi Udita you will also need to have done some training in the UK or another competent authority country. Here is the current status via the Medical Board of Australia: "Successful completion of the Professional and Linguistic Assessments Board (PLAB) test since 1975, AND :
successful completion of the Foundation Year 1, or
12 months supervised training (internship equivalent) in the United Kingdom, or
if the 12 months supervised training (internship equivalent) is completed in another Board approved competent authority country, approved by that competent authority."
If you work in the uk private sector (not in nhs) for a year, will it be acepted as well

As explained in the video you still need to do a year of supervised practice in Australia first.

Hi. The whole point of the Competent Authority Pathway is that you don't need to do the AMC exams.
Yes. Generally CA doctors are preferred over SP doctors.
Hey Anthony, I am a Greek medical graduate. Getting a full GMC registration in the UK does not require for us to pass the PLAB test since we are EU citizens. Do I still need to sit the PLAB test in order to apply for the Competent Pathway or the GMC registration and a FY1 are enough?
Yes you still need the PLAB.

So long as you don’t work too senior in your first role. It’s possible to be granted general registration but only in a certain specialty if that makes sense.

Hi Neha the term is "partially comparable". Yes if you gained this and were offered a post by a hospital they would be required to support you to attempt the rest of the process. Obtaining specialist recognition in the UK would generally then mean that you would be deemed substantially comparable.
How about if you’re Australian and you studied in Ireland for example? And wish to do your internship back in Australia?

They will tell you this. And it can happen for some doctors. But it’s a risky strategy. The Medical Board strongly caution against this course of action. Very few are able to come in via this path.

Hi Anshuman as I point out in the video, the only way you can apply for competent authority from a Rep Ireland perspective is if you completed your medical degree in the Rep Ireland.

Hi Joyce as per the MBA rules you need your Certificate of Internship from Ireland. So I am thinking you don't have that.
There is however a weird work around.
If you pass the AMC Part 1. You can apply for RMO posts in Australia. And this experience can also double count for the CA Pathway requirement. So you don't need to do the Part 2.
Do we need to do a NHS job (gov job) for this competent pathway, or NIS job (private sector) can also be suffice? @career doctor

Not sure I really understand your question Edu. You have to obtain a job offer in order to get registered. The Medical Board checks that the job has the right supervision in place.

But as of December 2019, there seems to be another part to it. It says that with completion of intern year and getting the certificate of experience, you need "Prescribed post examination or post training experience ". What is that and how do I get it? Sorry your question is a bit confusing. You are asking if with the PLAB and 12 months in a SHO role (supervised) will you be eligible for Competent Authority. The answer is yes.

WBA only applies to those on the standard pathway at a select number (11) facilities.

You can normally apply after a few years. You need to discuss with a registered migration agent.

Hi Pushpo thanks for the questions. If you complete the PLAB you also need 1 years supervised experience. This can be completion of the FY1 or there are a range of other options. This might be possible. The MBA will recognise an internship that is recognised by the competent authority. So if the UK recognised this it would probably work.

Yes. You can general get into training after completing your probationary year. It depends what specialty.

Generally the PESCI is only used for general practice (family medicine) positions

Rabbiya Aamir its a very complex question to answer sorry, it depends on who is employing you and in what capacity. If you are coming via competent authority and doing a provisional year in general practice you possibly won't need to do a PESCI. PESCIs are more often applied if you are from a standard pathway country or taking up a more senior appointment as a GP.

So long as you were working under supervision and can prove this you should meet requirements of the Medical Board here.

Hi experience in Ireland is not recognised by the Medical Board of Australia so you need experience in the UK or USA or Canada first.
