The baby was so small when the parasitic twin was removed, more extensive surgery than what she was already getting could've been deadly. Also, the doctor said that her muscles there never formed properly because of the head being attached in that spot. Waiting until she was older, bigger, and stronger to try to fix it makes complete sense.

It's where the egg or ovum separates to form identical twins.. however,,, something happens and the Egg doesn't fully separate into two identical twins.. sometimes you get conjoined twins, who survive.. and in this case you get remnants attached to the other twin.. they are called parasitic twins,, because the partially developed twin feeds off the healthy twin..

It is because they are allowed to marry first cousins. Which isn't far enough for the genepool to mix. It is often because the parents of the bride have to pay, so often they will keep it in the family. It is a cultural thing that is noticable even here in the UK in our health services. It is very sad but well known.

I'm unsure if that could have been fixed at the same time as the removal of the head or if it would have all been too much for a baby as young as she was. I assume it would have been too much to do it all at once since he doctor wants to wait until she's bigger to do the second operation.

It was technically alive at first, then died and fused with the other fetus. The second head was basically a tumor at that point being supplied with blood from the live baby.
