Eye Surgery While Awake Allows Patients to Help Surgeon

  • A minimally invasive surgical technique to correct the double vision and crossed eyes in people with strabismus - performed under topical anesthesia with the patient awake - enables a patient to gauge the results while still in the operating room so that the surgeon can make any adjustments as necessary to ensure the best outcome.
    The approach, pioneered for the last several years by Joseph Demer, M.D., Ph.D., ophthalmologist at UCLA's Jules Stein Eye Institute, has significantly expanded the number of people with strabismus who can benefit from surgery, not only by offering a procedure without the risks of general anesthesia, but also by facilitating surgical treatment for patients with smaller angles of strabismus than in the past.
    Strabismus, in which the eyes fail to line up in the same direction when focusing, is the major cause of double vision in older adults. Until now, patients with strabismus at a large enough angle have been treated with surgery under general anesthesia to manipulate the eye muscles in an effort to correct the problem. Since the result couldn't be determined until the patient was awake, a second procedure was often required in an effort to achieve the desired outcome.
    Patients with small-angle strabismus, or whose general health makes them poor candidates for major surgery, have used thick and cumbersome prismatic glasses to manage their symptoms. "These glasses are sometimes effective, and sometimes not," Dr. Demer notes. "But even when they do help, the ability to free patients from being dependent on prisms - especially since many have had cataract and laser surgery so that they don't need spectacles other than for their double vision - is a major benefit."
    The traditional surgical methods tended to "overshoot" the correction for patients with small angles of displacement, essentially giving them double vision in the other direction, Dr. Demer explains. With the minimally invasive procedure, instead of detaching the eye muscle completely, the surgeon can partially trim the tendons of the muscle, allowing the remaining tendons to stretch so that smaller angles of misalignment can be treated more reliably. All of this is done in 15-20 minutes, without the grogginess and bandages that come with general anesthesia. "It's like getting a filling at the dentist - you can go right back to your normal activities later that day."
    "Many adult patients with double vision have been told that surgery is not an option," Dr. Demer adds, "but with this minimally invasive approach they are now candidates."

    I had surgery on my right eye for diplobia four days ago, at the Beyoglu Eye Clinic in Istanbul. I was awake, with local anesthesia. The doctor told me everything she did as she did it, and why, in English. I could actually feel the thread pulling through my eye. I didn't participate, just held still. No fun, but manageable. The eye is still red and weepy, but the diplobia is GONE.

      I once considered surgery for my lazy eye. But unfortunately, I saw people having surgery with their eye hanging out of its socket and decided against it.

      @@aryanaek6516 I didn't actually SEE the thread go through my right eye. My left eye was covered, and I remember light. It wasn't painful, just creepy. But afterwards, through a slit at the bottom of the bandage as they wheeled me out, I could see the double vision was GONE. It's been three years now and my eye is perfect.

    Im 30 years old so its not really shameful any more compared to the grade school days. Anyways, I have full control to move 1 eye sideways at a time. Most likely I'm esotrophia and exotrophia. I have to manually move both of my eyes to have proper alignment every molly second of the day and if I don't focus on it with my brain they go crazy. Need I say more.

    2 years ago, I had a Head Injury due to a fall that caused a massive Brain Bleed and had ER surgery for a massive right side blood clot and had a Massive Subdural Hematoma.If I did not get the surgery, I probably would died said my Los Angeles Neurosurgeon. The buildup of pressure and compression of my right side of my brain caused some damage to part of my brain that controls the muscles horizontally in my right eye.
    After 2 years, still suffer from Double vision and my Optometrist can
    not fix with Prisms lenses. Almost got into a Car accident today due this double vision. Have to drive with one eye almost closed. Life sucks, hope no one needs this problem. When i get tired, it only gets worse. Almost thinking I need to stop driving take a Bus.
    Life is not fun with Double Vision

      i had a minor concussion back in 2011 on the left side of my head.. i think it caused my blurred vision and made my strabismus worst. now im having a second surgery in the next 2 months

    How much is it for this operation? I have had 5 operations in the uk and they just cant get my eyes right. They have made them worse. I have to wait a year every time for a new operation.

      DO NOT GET LASIK. IT CAUSED CORNEA KERATACONIS .IT WILL NOT HELP STRABISMUS..IT RUIND AND WEAKENED MY EYE MUSCLES SURGERY I HAD AS A CHILD.. 6MNTHS LATER I DEVELOPED CATARACTS IT CAUSES NERVE DAMAGE THAT NEVER HEALS PROPERLY..PLEASE DONT DO THIS..

    I had surgery when I was 12 now I'm 27 and when I take a selfies and relax my eyes mostly the right it goes far to the ride it scares me I dont want ppl to see my eye like that.i control it but my vision is getting worst and when I relax ita crooked.i also been having ache and pressure in both eyes.

      My problem started when as a child I began using doubled vision to escape the tensions of my situation. In short, I induced my own lazy eye condition.Gradually, a temporary doubled vision became permanent and I cannot get my eye to work right again ever since..

      Sometimes the muscle loosens and you simply need to have another surgery. Talk to your optometrist about seeing a specialist.

    Im cross eyed and i go to eye therapy, But it will take a while, so i want to see if its worth it to get surgery

      New surgeons are allowed to practice on your eyes. They have you sign a whole bunch of papers in order to exonerate them in court if something goes wrong by showing that you knew and agreed to that possibility.

    What if you have horizontal congenital nystagmus in both eyes with a notorious esotropia just in the lazy eye. Can you still use local anesthesia so patient awake?????? Do you touch both eyes or just the lazy one? I´m very puzzle about.
    THE CASE.-
    The esotropia appeared at 1 year-old. As soon as possible vision therapy until 17 year-old. Lazy eye never got the center but its deviation from it was, say, socially "tolerated". After university and 10 years of work esotropia became very notorious, On the other hand the dominant eye began to lose night vision and color perception (cause fatigue? too many hours in front a screen?). However exams say retina OK. Evoked Potential exams show damage in the auditive channel. OCT shows it flat, lazy eye flatter than dominant's. Decrease of night and color vision in the dominant eye stimulated considerably the use of the lazy eye which now sees better in darkness or perceives very faint colors better than the dominant eye. Currently dominant eye is much-much-much better in reading, lazy eye much better recognizing faces and colors. And what with this????? Simply, it gave different behaviors during ocular exams for a strabusmus surgery:
    -Exam 1 with Oculist 1: lazy eye looks more reactive or responsive than it naturally is, patient knows the target to see is about to reach the lazy eye visual field so he volontariy switched the eyes in a very responisve way deviating the dominant one to use the lazy one. Ocullist 1 concludes to operate both eye with local anesthesia since he need to prevent double vision and still good alignment.
    -Exam 2 with Oculist 2: similar to Exam 1. Oculist 2 concludes the same but don't remember if he said local or general anesthesia
    Exam 3 with Oculist 3: There´s favorable ilumination for the dominant eyes so it remains dominant, lazy eyes doesn't receive switching help and remains lazy. Oculist 3 concludes to choose general anesthesia because of the strong nystagmus and operate just the lazy eye.
    Exam 4 with Oculist 4: Oculist 4 knew the patient for so long so he is very surprise to see the lazy eye having increased deviation and dominant eye having lost sight. Ilumination in the examination room of his office is poor so lazy eye becomes more active so for moments Oculist 4 notices the lazy staring at him instead of the dominant eye which become esotropic. So Oculist 4 concludes general anesthesia and operate both eyes to make less surgery in each eye. However there's a lot of trust between patient and Oculist 4 so conversation goes on after examination in other rooms of the office and Oculist 4 verifies that in an iluminated room the lazy eye remains very lazy., so he doubts about his first conclusion. No time to repeat exam.
    As descrived, the lazy eye is needed by patient to see in the dark, however treated as too lazy will it condemn it to too much surgery and then limited mobility? Operate also the dominant is a good risk? Awake or asleep surgery? Double vision risk?
    Please UCLA Health (or someone esle), can you give me advice for strabismus surgery in this case?????

    i have Strabismus in my right eye operated once at the age of 12 but not much success. now i am 33. i have recently visited the doctor and she said that you will have the double vision problem if got the surgery and double vision cannot be treated. i wish i could have the surgery. could any one recommend the doctor for this.

    I hear the screams you also try to feel emotional by tearing using my emotion to trick or do acceptance. You try to emotionally use it to do acceptance and people can feel pitty.

    My experience
    Surgery can not solve everything sadly. It is very sopisticated what eyes consulted of..
    Before surgery many people dont know , they can add one vision with two eyes whatever distance is far or near, it just cause when people tired, their eyes cant move together,
    But they just think that it is most serious problem also it seems weird to other person,so they cant endure that condition for any reason finally
    But after surgery when people who got it move their eyes togther for near distance or far, their one vision can easily be disrupted.
    This cant be solved . If not serious
    I dont recommend surgery , dont believe dr's word . In world there are so many disease what cant finally be resolved . It is one of them sadly

    I have strabismus in my left eye since childhood. no iam 21yrs old, so the vision from my left eye is so poore in such critical case it is possible to do strabismus surgery?

      I had what your asking,got lasik then became binocular diplopia..get a prizm in the one eye in glasses, stop being vain,about wearing glasses..Just warning ya..

      @@soupricemf1260 Surely U know your eyes have many muscles that control & hold it. Sometimes 1 muscle gets weaker & longer & can pull your eye more to 1 cider pull it up or down causing double vision or blurred vision.
      It can B due to nerve disorder, diseases, accidents or just aging eye muscles, etc..
      I have heard doing prisms makes U dependent on them & u R stuck with permanent double vision. So probably getting the surgery instead of the glasses would B a better decision.
      Yes it is scary, but double vision doesn't just go away. The top doctor told me the eye muscle doesn't get shorter once it is longer. In fact it can get even weaker.
      I have been told mine is doing that even though I don't look crossed eyed my eye R doing that so I see side by side images intermittently.
      I'm afraid but I guess I am going to do the Dr. Demer at UCLA & cross my fingers & do everything I'm supposed to do to make the results a success!
