    As a new LPN and aspiring aesthetic RN I have been following your techniques and videos for a while, and you are incredible!     Dr Pearce I love your webinars ! This one is very important . Many practitioners , injectors and doctors are losing so many units of this very expensive toxin . I started taking off the hub about two years ago and drawing up from the bottle without piercing the hub. Sometimes I do transfer syringes and it’s interesting to see that the needle can get dull from tapping the glass. Thank you so much as always you are awesome! I am sharing this video with many colleagues here in the US. I feel it’s a huge problem.. Thank you so much .. lullabella

    Thanks you very much for the tips. Another way of withdrawing last drops is to gradually withdraw needle out while still spiriting when holding vial upside down.

    I always thought about this. Doctors don’t care because we are paying for it, not them. I have also wondered about them rushing through or trying to hide fear if they are not experienced with Botox.

    Thank you for this. I’ve been paying close to $680 for mine. I really enjoyed this. The worst is the counter pressure and the drip.

      after you add the saline when you mix it leave the 25 ga. needle in the vial and unscrew the syringe from it. this will release the vacuum that remains in the vial. you will hear it when it when the air rushes into the vial. I am assuming this is the counterpressure you are referring to.

    Ver, very useful Vieo. Thank you. Instead of rip the rubber out, I take a 20G 0,9x40mm needle to fill the vial with NaCl and get it out completely with the syringe. Might be more comfortable. I think, if you penetrate the rubber with the same fine needle you will use for injection, the needle will be blunt too.

    Could add a category. Accidentally hitting plunger before needle insertion. As a dentist we always have a finger on that plunger and I have to be very aware of this.

    Well done! I always draw from the vial with bevel down against the glass and then transfer to a new syringe. Transferring slowly to reduce trapped air and lessen the need to flick the syringe. Beware too much flicking! which can loosen the oil lubricant and result in what I call 'phantom divots' on a patient's forehead.

      Can you elaborate more on the phantom divots? Is the oil lubrication coming from the plunger in the needle?

      @@ceileann yes, the silicone lubricant oil in every syringe, that allows the plunger to glide smoothly.

    I just read an interesting pilot study about IM vs ID botox injections for the forehead. The photos were really fascinating bc they actually injected half intramuscularly and the other half intradermally. The differences in brow ptosis were obvious.

    Dr. Pearce, thanks a lot for this very informative video. When you talk about 4 or 2 units, which Botox do you mean ? Are these Allergan Units ?

      yes, it applies approximately to allergan and the newer toxins which try to use the same numbers - eg bocouture/ zeomin and others

    You should consider adding a strong light at 11 or 1 o’ clock. light with diffuser so you don’t get eye glass shadows

    We were taught to avoid aggravating the Botox as it can impact the efficacy of it. Could flicking it do this or would it not do anything?

    Love your tips thank you- now Im sure I haven't made this up but I remember one of your vids discussing be cautious using steret wipe with alcohol on as if this gets into the botox it will kill it. But torbac or equiv. has alcohol in. Please put my monkey brain at ease thanks tim x

    My injector do botox per area, not per unit but they do adjust cost per "frozeness" so if you want more natural it's a specific cost per area. And if you want fully frozen it's slightly more per area. I thought that was clever as you should be paying for expertise as well. Not just per unit cost.

    I used to double dose everything because of using BD syringe, just to remind that one point of it is 0.02 mL, I didn’t notice this until after the procedure, then later the patient end up with frozen forehead 🤦🏻‍♀️🤦🏻‍♀️🤦🏻‍♀️🤦🏻‍♀️

    I am looking for professional advice. I have bilateral Dystonia and have had a bilateral TMJ replacement due to this. My problem now is as I have aged my masseter muscle has become much thicker and tighter. Even with regular Botox but additionally due to lockdown I have not had my usual treatment. Which is done by the NHS max fax. I am now left with a face that looks like a prune because the knock on effect from the muscle constriction. I am asking advice on what can I do now my whole face is affected. I have been unable to find the help and advice I need for this.

    Very nice video! I wanted some advise , i recently got botox for the jaw and asked the practictioner do my crows feets around my eyes, instead he did botox in my cheeks!! Im hating the look and now because of it i cant even smile fully and when i do smile my skin under my eyes gathers which in turn makes me look like i have wrinkles under my eyes! Will the cheek botox disappear and if so how long? I cannot believe this happened and im so worried. Please advise!

      Sorry you don’t have an ideal aesthetic result. Sometimes a ‘shelf’ like bunching can occurs when muscle relaxants are used around the eyes. It’s important though if you don’t have an ideal result that you return to your practitioner so they can support you and at least learn what technique they did to cause this problem.
      After a couple weeks the result will start to relax a little. But it could be a couple of months too.

    The US is crazy the pharmaceutical companies are crooks and everything seems to be driven by money it’s incredibly sad we also help other countries more than we help our own! I was a nurse Absolutly loved it all I ever wanted to do was help people but now as a mom of 4 I try to stay homeopathic and have lost all trust in doctors and pharmaceuticals!

    Hi Dr Pearce, you say you typically do 4 units per dose in 99% of your patients. So I take it that includes men? So you wouldn't typically inject more for a man?

      During consultation it’s a great opportunity to discuss your fears and worries and what would they do if you were to come into complications.
      We are very happy to hear patients ideas and thoughts. After all it’s your body and it’s important you know what’s happening.

      Some unlucky people can be resistant to toxins unfortunately. Using a different toxin can be an option but it still may not work.

    If you pull back and forward won't that give you a whole in the face??? When I have been injected with botox in a spot where they pulled syring back like that I got a dent =( sounds silly but it happened three times! 😅

    How do you tell your injector or share this video when you are a patient who is genuinely concerned if she knows all these precious information? 😂😅 Should we as patient discuss or ask such things with them, for fear that we are undermining their knowledge and skills? 😩

      I admire patients who have a bit of knowledge about their treatment. I believe after all we should know what is happening and I teach my patients what I know when they’re sat in my clinic. Talk with your practitioner. They may learn something from you too! Healthy discussions are gold!

    Why not just aspirate everything into the syringe and change the needle instead of a new syringe?

      These needles do not come separate. But yes it is possible to draw it up into different syringes and swap the needles. The gradients for dosing are a little different though.

    I do not know where this doctor buys is Botox but I can tell you that it actually costs a third of the price he mentioned.

      It's very expensive in the US, much less in the UK. Not sure about the rest of the world
