  • Went to Malta. Got schooled. Here are my five biggest lessons from Rebreather Forum 4....
    5 Key Learnings From RF4.... it was difficult because I learned a ton. Once the full report from RF4 is published, I will share it on Divers Ready social media. Thanks again to Michael Menduno and his team for putting on such a great CCR educational forum.

    That’s interesting about the gas bubbles. I’d been looking at the O’dive unit.

    Thanx, James, for the very informative video. I fully agree the necessity of checklist usage. It would be useful for OC divers, too. I have seen so many odd situations with OC (rec) divers. Most of the problems could be prevented if a simple check list would be used. It is not any coincidence that pilots always use checklists even when flying several times a day with the plane. It is worth to remember that breathing device is a life safer.
    Check lists are useful for many other situations, too. E.g. I have a checklist for my underwater camera system.

    Seems like you could track the sales of both sorb and the extendair carts to judge how big the market is. Between those consumables and o2 cells.

    I'm just open water diver, however I'm an ICU doctor as well. 2 things...
    1. Do CCR units include continuous capnography? It seems pre-breath capnography would work to detect if the scrubber is malfunctioning and post breath capnography would detect hypoventilation/low minute ventilation.
    2. Anyone who questions checklists should read Checklist Manifesto....

    • @DavidRabbit
      @DavidRabbit Год назад

      Regarding #1, they do not. Everything I've seen involves a 3-5 minute pre-breathe procedure in order to determine functionality (holding proper PO2) and to verify that there isn't a CO2 breakthrough on the scrubber or another CO2 retention issue. Even then a paper in 2015 highlights that this isn't sufficient in all cases to detect a CO2 breakthrough issue since a 3-5 minute period wasn't enough time in some subjects for the minute volume to change.
      I do know real-time minute volume monitoring was looked at in a paper a while back (I think early 2000s?) but the result was inconclusive if it would actually be beneficial given confounding factors (depth changes, activity changes, orientation/work-of-breathing changes, etc).
      I think the easy next step that's being looked at right now is pulse and pulse-oximetry monitoring. A 2022 paper specifically looked at the later and concluded a pulse-oximetry monitor on the forehead or in the nose should give a diver 30 seconds of warning. Given the Oceanic/Apple Watch collaboration I wouldn't be surprised if they or another dive computer company 'jumps the gun' and offers it as an added feature in the next 1-2 years.

      What’s interesting to me is that continuous waveform capnography is already a standard on the medical side for monitoring patients undergoing sedation or are on a ventilator. Unless there’s a pressure/durability issue or other practicality issue that I’m missing, continuous monitoring seems like an easy solution to detect either breakdown of the scrubber or hypoventilation.

    Losing 5 CCR friends is really shocking. Since they were friends, I’d assume they really knew what they were doing. And yet they died. Is it possible to see a video that describes the circumstances?
    It would be hard, but it seems like really important information, because if these things happened to these CCR divers, they could happen to any of us. At any time. I can’t imagine more impt information. The personal relationships would make this uniquely impactful.

    • @DiversReady
      @DiversReady  Год назад +3

      Not only were they friends but they were also mentors and people I consider to be better divers than myself. Sadly, I don't really know what happens in most cases. I still don't know what happened to Andy, Chris, Mikhail...

      There is a few large gotchas with CCRs some of which could be solved pretty easily. #1. The Shearwater controllers up until recently didn't vibrate or beep when in serious error condition, huge (negligent?) issue. #2. No active monitoring if o2/dil tank valve is open. #3 no full reliable co2 monitoring yet but getting there on one or two models. Patents will likely cause other units to be slow to have the same feature though. Also, not sure why they don't double up the mushroom valves on the mouthpiece for redundancy.

      @@telmnstr interesting- wonder if the issues were among those 5 CCR deaths. Losing 5 friends in independent events- who really knew what they were doing- would be.shocking in any sport.

      It wasn't broken down by brands. The whole appeal to the manufacturers in offering up their sales figures was that it would be anonymous. For example, lets say 5000 units were sold last year, there was no breakdown of how many were from each brand, and this was intentional.

    👍😎🇵🇭🤿! Never used a ccr but I did look down that road. Pretty high cost but I can see it’s benefits for certain divers. I still enjoy the education you are providing! Thanks!
