  • Braces Explained: TADs (Temporary Anchorage Devices), Mini Implants, Mini Screws, etc
    In this video I will be reviewing the question of “what are TADs? and what are they used for?”
    TADs (temporary anchorage devices) come in a number of different names, including mini implants, mini screws, micro implants, micro screws, etc. Although they have a number of different names they all serve the same purpose and that is to serve as an anchorage for tooth movement.
    When we put a force between two teeth, we have an equal and opposite reaction. If you can imagine two people that weigh the same are on rolly chairs that pull a rope, they will both towards one another. The same is true with teeth. When you place a power chain between two teeth, they will move towards one another. This limits tooth movement only in the direction of where teeth are. TADs serve as anchors that are temporarily placed in your gums into your jaw bones. They are relatively painless and you can resume normal activity once the anesthetic fades! At the end of your treatment, you orthodontic can easily remove these TADs.
    With the advent of TADs, orthodontists have been able to achieve a number of things that used to not be possible with braces alone. We can decrease the need for headgear, jaw surgery and extractions (although sometimes these things may be necessary). They are really great but like everything else, require your help in keeping them clean! Make sure to brush gently around the TADs with a soft bristle toothbrush when brushing your teeth to make sure no food and plaque accumulate and your TAD remains healthy!

    I got 1 TAD yesterday and tbh it doesn't hurt at all. I was so scared ( I literally cried in my orthodontist's office) bc everyone told me that it would hurt af. I know everyone has a different pain tolerance level but I only had a little headache. So please, if you're going to get TAD, be relax, it'll be fine. Sorry if my English is not perfect, I'm from Belgium.

      i have some questions if you can answer ( in french if you want i understand)
      i have a gummy smile too and i don't know how can i ask my orthodentist to give me a TAD
      i have one side lower than the other and it does bother me and i avoid smiling... i want to ask my orthodentist i he could help with that but i dont think he will and it's stressing me out
      the only reason i got braves is to fix that but it has been almost a year now and my gum didnt change at all :/

      @@mfg9838 of course they did and I have to have it for a year. But tbh I don’t even feel that I have a TAD in my mouth 😃

    I had four TADs placed on my upper gums to help reduce my gummy smile. It has been such a life changing experience in many ways. I was told that I had a "horsey smile" and I have always been a little self conscious about my appearance when smiling big. I think this is going to change oral surgery in so many ways and much more than was originally anticipated. I would love to see a video about gummy smiles and TADs!

      Me too... i mean i have the “horsey smile” when i have big smile... so how was it going for u? Does it pain? And whats is the result? Im afraid to put the stad... but i wanted to fix my smile😭

    to anyone who is afraid of getting tads or about to get them, be assured that they’re literally painless! having screws drilled into your jaw sounds scary and it’s normal to be anxious but the most painful part of the whole process is the injection to numb your gums, after that’s done you won’t even feel a thing. my orthodontist warned that i might feel some soreness or discomfort for the first two days but i did not experience any of that. it’s been three months since i got my tad implants and i don’t even notice that they’re in there anymore.
    after care is relatively easy too, i had to use a special mouthwash and q tips to clean the area for about a week after the surgery but now i just ensure that there’s no food trapped on the screws after every meal and i use a mouthwash every night. if you take proper care of your braces i’m sure you will have no problems with the screws too.
    if you’re still hesitant, just know that getting the implants is one giant step towards a better smile. all the best! 😬

    Will be getting mine in 3 weeks to prevent a gummy smile. My deep bite has got worse over the years and deep at almost 10mm😲 decided to get braces a year ago for the sake of my health! Love your videos they are extremely useful!!!

    I’m going to have to use tads. My dentist told me that today & I kinda freaked out! But after watching your video I understood why I would need them. You’re such a good educator.

      How is it going for you? I had two tads inserted in the roof of my mouth a month ago, but had to postpone installing the metal thing. Fixing my overbite. The way my orthodontist explained it was that she wants to push my back teeth upwards which will shift my front teeth down/inwards.

    Thank you for the explanation. I had three TADs placed a few days ago. My orthodontist will start pulling my molars into the missing spots in two months. I'm feeling discomfort on my right side particularly upper TAD, but I'm rinsing with a prescribed oral rinse and brushing to keep mouth clean. However, my face on the cheek area is tingling. Hoping that goes away soon. The procedure was not painful...more scary than anything else.

    Got my TADs 2 days ago. It is not painful at all. 😊 Before , I got consultations to other dentist and they always suggest jaw surgery which I think is costly , painful and will take longer amount to heal but last yr I met my super awesome dentist who suggested me TADs that will help my case.
    Before I got my TADs my dentist make sure that my teeth is healthy, aligned and ready for TADs, that why it took a while but I am really looking forward to the progress. You are really awesome too Doc and Yes , TADs is really revolutionary for patients like me cause it give us hope for a beautiful bite and smile. 😊❤

    I had two lower TADS placed. I experienced excruciating pain on the right side only. My orthodontist ran x-rays to ensure they did not hit a nerve. I cannot think of an explanation otherwise as to why I was in pain only on the right side. They managed to place the TAD, and ran another x-ray to view placement. My orthodontist ended up having to remove this TAD due to bad placement. During the removal process the pain was so unbearable, out of instinct I pushed my doctor’s hand away ( of course getting turned into a large scene). The TAD on my right side was the worst pain I have every experienced, all in which they wanted to blame my behavior ( crying in pain) on anxiety. Mind you, the left side I felt nothing more than pressure.

      I had to get TADs a month ago because my front teeth needed to be moved to the side slightly to be aligned properly and I didn’t want to get extractions/headgear. They didn’t hurt getting in at all, but it was definitely sore the days after like getting braces put on the first time

      I have TAD's because I've got no second upper teeth and my third teeth are next to my ones so i'm moving them to their right spot leaving room for artifitial secounds(i don't know how to name the teeth in eng so sorry for thatXD)

    I got SOOOOOO close to getting a TAD, my student ortho was ABOUT to administer the anaesthetic when her supervisor was looking in my mouth and decided that we would wait… the teeth needing to be moved was achieved through elastics And
    I ended up not needing the TAD at all 😮‍💨 #closecall

      Update: Had to remove them because it came out and treatment was getting delayed so got teeth extracted in february 2021

    According to my initial treatment plan I was supposed to have at least 1, possibly 2 TADs used to help close the gaps left by extracted teeth in my upper jaw. When the time came to close these gaps, though, my orthodontist decided we could probably get the desired result without using TADs after all, so we've been doing that and it seems to be working well, TADs, however are not completely off the table just yet. I wouldn't mind, though, if I ended up not needing them, just because of the extra cost I could most definitely do wothout :)

    My next appointment is in 2 weeks... My ortho is supposed to be changing my wires to a thicker one. I am correcting an overbite. I know TADs are effective but I hope I don't have to have them.
    Already had two extractions (#5 & #12) and I'm ready for my first wire change (or so I think lol)

      *TheChangingFox* I had to have four of my permanent teeth extracted and now I have my eye teeth coming in on the top and they have to pull them down it has been very painful so far

      You should not be pulling your teeth backwards into your face if you can help it. There are some (not so) new orthodontic methods, which actually aim to expand / develop the upper jaw so that the lower jaw can be positioned properly. These methods never require extractions, etc. I know you are already on your path, but do some research if you are interested because retractive orthodontics can have some side effects down the line that are not desirable.

    I had a deep bite but my tads were screwed on my upper jaw not lower. They worked very well.

    Just got my mini screws yesterday on both right and left upper jaw. My dentist said my upper jaw teeth are extremely hard to be moved even use Power chain and spiral weren't enough to make those move to the wanted position, so I need to apply the mini screws to bring the teeth inside.

    My ortho used TADS to expand my upper jaw. I never got fully numb though, which was one of the most painful things i have experienced lol. Regardless they did prevent me from having jaw surgery at 24. I can confirm keeping them clean is absolutely nessisary! Food would get suck under the screw all the time and if it was left there it was very painful. I used my water pick to really clean the area after every meal and that helped a ton. Its weird that i never hear orthos except for mine use TADS for jaw expansion. It really helped me as an adult because we were too poor when i was a kid to afford braces. They stopped my countless migraines from my bite and i did not have to get surgery. Although 3 turns a day on my expander was... rough LOL. I wouldnt have changed a thing even though it was quiet the journey with my TADS.

    I got my Tads placed last night. I wasn't scared at all cause according to everyone they don't hurt at all. The process itself wasn't painful I just felt a lot of pressure. But once the anesthesia wore off it was literal hell. I've experienced a lot of pain in my life but nothing compares to this. It was like someone was tearing my gums with a blunt knife and then piercing the wounds with needles. It was so bad that I wanted to tear my gums out. Even breathing made the pain unbearable. Literally cired all through the night. Took 4 different painkillers but nothing made it better. Its less painful today but god I wanted to kms last night.

      @@ayat5929 I can't imagine what you're going through. Take some painkillers and try to sleep cause that's the only solution. My dentist initially recommended panadol but when the pain got worse he asked me to take Synflex. You can also consult your dentist and dw you'll get through this. I got mine removed last month.

      Glad I am not the only who thinks that this was the worse pain ever!! I have 3 and I was crying in the inside as I was taking the bus home. I couldn’t eat and I couldn’t do anything!!

    I have a TAD for 2 months and it fixed my bite. Loved your video. The implant was fine, just a little pressure like you mentioned. There is one thing to add though. After the implant, I was driving and decided to drink water. Well, I couldn't help but laugh as the water ran out of my mouth and down my neck. Apparently, the numbing medication impacted my ability to drink. Funny when it happened. Otherwise, good experience with TAD.

    Living in the UK, I love many aspects about the country. However when it comes to dentistry/orthodontics, the US definitely demonstrates more innovative and revolutionary techniques. I wish I had this procedure to supplement my braces as this would have seriously improved my overbite. Does anyone happen to know any London clinics that offer this? Really informative video though!

    I have 2 top molars missing and that created spaces between my both incisors and their respective canines. My ortho is just using powerchains to move them back. I think tads will do a better job what do you think ?

    Hi Doc Greg, My orthodontist recommended for me to have TADS because my left upper first molar is already going down as my left lower first molar was extracted when I was a kid. After my upper molar is back upward, (after the TAD treatment) the ortho said that I would need to put dentures in the gap. Is that the best treatment or you have other better option you can recommend? I hope to hear from you doc! And thanks!

    I have a skeletal anterior open bite. Do you think I’ll still be able to get Tads even if I already have one dental implant on my lower first molar?

    I had to get 4 miniscrews to secure my palate expander onto the roof of my mouth since I'm 18. I got injected with numbing stuff, so I couldn't feel the actual screws going in. The only pain I felt was from the bands on the expander tightening on my molars.

    I am currently in braces to correct an overjet. The first orthodontist I went to said tooth extractions would be necessary. I consulted someone else and they suggested the TAD's. Placing them wasn't painful. I did feel a bit of pain when the anesthesia wore off, but that pain was easily manageable with paracetamol. So yeah, they are nothing to be scared off, and they may be worth it as it means tooth extractions can be avoided.

    I have tads w the screw top inside my mouth and it literally scratches my tongue every time i eat. Will it calm down?

    I had a bone plate in place for a TAD because my orthodontist said he needed a very sturdy anchor, so now I have a plate that goes all the way up by my nose out into my mouth. Is this a normal thing or only for rare circumstances?

    Hey doc, I have a question. Why do some orthodontists not place brackets in the second molar, but rather place them in the 1st molar usually. Isn't it better to use the 2nd molar as an absolute anchor? I would appreciate your reply, or anyone that has an answer for that because I am curious. Thanks!

    Hi Dr.
    I’ve braces for almost 6 years now. Initially in 2014 while putting braces Dr. didn’t extract any teeth. But in 2019 they extracted 4 teeth’s. But still we didn’t achieve the purpose of braces. Now Dr. suggesting to remove upper 3rd Molars and put TAD to pull upper teeth’s. Now my questions are?
    1. Is it ok to extract 3rd molars (so, then totally I’ll be lost 6 teeth’s)
    2. Will TAD gives any problem in future?
    3. Is extraction of teeth’s effect nerves in brain or any problems do we get due to extraction?
    Sorry for big question. Answer would be appreciated. Thanks

    Hey I got a question. So I had 4 teeth removed a few weeks ago. Two on the left side and two on the right. Each side had one removed on the top and one on the bottom now I have gaps between my teeth where they were extracted. Would I need TADS to move to teeth back to cover the gaps?

    Can I put TADs elsewhere into my skull and use them as anchorage to change shape? I have a slightly low palpebral fissure length and I want to pull my temporal bone a mm or two outwards.

    So I have gotten a nance appliance, but the band on the right side is having a really hard time sticking to the 2nd molar. This is because this particular tooth is only partially erupted. The orthodontist said if it falls out again, they're gonna have to unsert a screw for anchorage (I've been trying to avoid it, kinda freaked out by the idea of screws in my mouth). But if I do get them, how are they removed? Do theg put dental filling into the jawbone? Will there be a hole in my bone forever or will the bone grow back by itself?

    I'm a cleft patient and have a crossbite I just got TAD in my lower segment for wisdom tooth removal and distilisation can you tell how much time it'll take in the complete procedure??? Also its kinda painful since the miniscrew are poking in my cheeks so bad 😔

    I had TADS a couple of days ago and it doesn't hurt as they use Anastasia for that part and after that also it will not hurt as TADS don't touch ur nerves but tbh I had a pain when they did on the left lower but after a while it an hour it was gone and I didn't even realize that.. Don't be worried... 🥰

    I just got 3 TAD's to correct a severe scissor bite, 2 screws on the upper right side and one at the lower.
    The lower one is hurting me tremendously, even though I have high pain tolerence. And my cheek is a bit swollen! Is that normal??

    I'm getting TAD's soon. I was supposed to get it at the orthodontists but I freaked out, and they referred me back to my oral surgeon as it would be easier if I'm asleep. Anyways what my orthodontists hope to achieve is to move my mandible back because it came too far forward.

    You say they don't prevent jaw surgery? I'm getting Invisalign with TADs to fix my open bite over the braces + jaw surgery route. Will I eventually have to get jaw surgery at some point?

    Wow thankew you so much doc.. Its been 3 months i have had braces. Last time my orthodontist told me we are going to pull your teeth using mini screws . I have an overjet problem my next appointment is on November 12 .
    But my question is why they took 3 months to put miniscrews or pull teeth? i don't feel any change till now 😤😥😥😥

    Dr. Greg, love your work!! I have a question. Currently I've been having TAG for a three months on my upper gum to treat "gummy smile" . But I'm not quite sure it's really working on me.. would you ever use it for that purpose?

      Yup, you can definitely use it for that purpose but that movement of "intruding" is one of the slowest in orthodontics!

    I just got 2 TADS and they hurt a lot, how normal is this? It was fine putting them in but as soon as i got in my car they started to hurt like crazy. Is this normal at all?

      Thank you so much! Typically a screw is just a TAD but a plate is something that goes under our gums and its fastened with TADs which are a bit more stable but have a little longer recovery time

    Is it common to develop canker sores around TADs placement? I had 4 screws and was fine the first 2 days. On the 3rd day morning, I felt pain in my mouth and found 4 canker sores on my upper lip where it touches those TADs. The the 4th day around TADs placement site also developed sores... I now have about 8 canker sores, pain as hell.... Is it a sign of infection? Or it’s just my body not used to those TADs?

    Ok so I have a problem with my teeth, I have a missing set of teeth next to my 2 front teeth and now my canine teeth are there and there is a baby tooth where one of my canine teeth were supposed to grow, my question is can you get crown implant right after you get your braces off or do you have to wait? I had a friend with the same problem and they stuck a fake tooth on her metal retainer before she got her implant.

    I got Tads put in a few days ago. I still have mild pain and swelling. I can feel and see the Tad on the roof of my mouth. But the one on the outer jaw bone I can't see. Is it possible it's lodged in my cheek tissue?

    Hi Dr. Greg! Thank you so much for the videos, I’ve learned a lot from you and I couldn’t be more thankful! I was wondering if you could do a video on the consequences of teeth extractions. I am supposed to get 4 of my premolars extracted in a few months and I’ve been researching. I’ve read people commenting about sunken face and lack of tongue space causing airway problems. I’m very worried and wanted to know your take on this. I have big teeth and a small mouth and I feel like I will have to get the extractions. I hope you reply back. Thanks again!

    Dr. Question please: for small closer, on just one side, on upper arch...Can spacers be used to move teeth fully bodily? close a space approx. 3mm. I have space between canine and 1st premolar. Wondering if I can slowly move the back teeth by placing a spacer distal of 1st premolar. Space created...Then place distal 2nd premolar...continuing all the way back, then repeat again starting at distal of 1st premolar until 3mm close it one direction. Do you understand what I am thinking? Thoughts?

    My orthodontist told me to do BSSO for my overjet. I said no to surgery, then Doctor inserted 2 TADs in my upper jaw to correct the overjet, but left side TAD came out twice what should be the reason? What shall I do now to correct the overjet?

    can you do a video on misaligned teeth? my top teeth are completely misaligned to my bottom where my two middle teeth that should be in the middle are shifted to the right side. would braces fix that?

    My ortho placed one tad in the middle of my upper jaw to pull my front teeth upward but he's not using elastics, just a wire. Does it work like that?

    That sounds cool and looks a bit intimidating. But I think I would’ve chosen TADs over getting 4 premolars extracted. My orthodontist didn’t even mention this option.
    Anywho, I’ve had my braces for 5 months and I can tell a bit of progress, but not much. Tuesday I had a powerchain put on my top arch to start actively closing the extraction sites. I also have elastics on each side to help with the closing. The pain wasn’t that bad! Excited to see some more progress ☺️

      Same. I knew about TADs but I don't think they use them everywhere (living in Sweden). When I asked about them I got some weird answer that its a surgery procedure bla bla.. I dno. I was pretty sure my overjet can be corrected with them but they gave me the pull out 4 premolars option.. meh.. but yea whats done is done.. hope the result will be good in the end.

      One isn't any "better" than the other. Using TADs is a lot slower than extractions most of the time

    Do you think you could do a video on how you deal with adults with hypodontia? I have 4 baby teeth, and only one has an impacted permanent above it that my orthodontist and dentist are hoping can be brought down with braces. I'm pretty scared that through the process of teeth moving that the others will get loose and fall out. I have more issues going on with my mouth and braces, but this is my main concern right now.

    Hello. So I got my herbst off a month ago :)) but today I went in and they put a deep bite thingy in my top back molars. Now when I shut my teeth it creates a gap.. do the things grind down so I can smile again or am I stuck like this :/

    How about Root Canal? My dentist mentioned it to my visit and my friends says its very painful.
