Well she's supposed to be dead. Are you suggesting that when you kill a boss, they are stored off-screen? Also, look at my map in the video, i am in the same room where she spawns originally; its just a glitched out version of that same room.
What usually happens when fighting a boss is: when entering the room the first time, the game loads a special room with the boss in it. Then after you kill the boss and enter the room again, the game does not load the boss room. It just lets you enter the empty room where the boss was. Some games have glitches that allow you to access a boss you already killed (SotN - Succubus fight) and most final bosses of games are like this. In this video, I forced the game to load a strange version of the first boss room, and it was just broken.
The secret is that its not in a place that you can normally reach. In fact, even following my video exactly, you only have a 30% chance of actually reaching this secret room.

Not a secret boss but just used assets on the boss + bosstage
