A lady had lower eyelid surgery and fat grafting almost 4 months ago, but is concerned her lower eyelids are still noticeably swollen, especially on the left side where more work was done. She still notices puffiness, and a dark spot under her left eye where she had the most bruising. She is hoping she is a slow healer, but is wondering if the long healing time time is normal, and wants other professional opinions.
New York oculofacial plastic surgeon Amiya Prasad, MD reviews her images in this video, and discusses the risks of transferring fat into thin eyelid skin:

Sorry to hear about your problem. Unfortunately, removing grafted fat from the lower eyelid is not as straightforward a procedure as those you mentioned, which is one of the reasons why I strongly recommend people don't have it in the first place. Removing fat grafts in the lower eyelid can be a very involved and specialized procedure done by an oculofacial/oculoplastic surgeon who specialized in eyelid revision surgeries. You will need to have a physical exam to determine the procedure, and it may take more than one surgery to remove. I hope this clarifies things for you.
I had fat injection under my eyes a year and a half ago. This morning I woke up with swollen under arrea at the left eye. It is where it was injected. I slept on my belly last night. Could this be because of how I slept? I did not have problems until now, could this be temporarily?

I hope you understand that I can't recommend a drug like Accutane without first examining your skin . I suggest you follow up with your surgeon who performed the fat grafting, or see a dermatologist for a proper examination before considering Accutane. I hope this helps you.
What about upper eyelids? I have one eyelid that has less fat than the other and it drives me insane. I’m too young to commit to fillers, and I would really like a permanent fix

While it is possible to have fat grafted behind the upper eyelid, and below the brow since the skin is thicker, grafted fat is still unpredictable as 30-70% of it is absorbed soon after surgery. Also, if you're too young for filler treatment, you would also be too young for fat grafting in the upper eyelid/lower brow as your face is still growing and maturing. I hope this clarifies things for you.
How do you feel about putting nanofat in the eyelid in multiple thin strings? I had that done to treat the underlying redness because i have a thin skin and little fat there. But i also have some baggyness too, so what can be done about that?

Nanofat is still fat grafting, and grafted fat needs to take to a new blood supply, and has an inconsistent shape, so it remains an unpredictable procedure prone to complications. I still prefer treatments like hyaluronic acid fillers to treat hollowness, PRP to thicken lower eyelid skin, and likely lower eyelid surgery to treat eye bags caused by prolapsed orbital fat. @@AmiyaPrasadMD Thanks for the answer. I have some kind of redness/swelling on my right side even 7 months after the fat transfer, but apparently it shouldn't be a real problem. My surgeon said that it just takes a long time to heal.

Once fat is removed from its original blood supply, it becomes highly unpredictable in taking to a new blood supply, and retaining shape, so it may not be just healing taking a long time that is behind your current appearance. You can wait to see if things improve, but just letting you know about the behavior of grafted fat that your doctor likely didn't tell you about. I hope this helps you.
@@AmiyaPrasadMD I would like to know more about this, is there more info somewhere? My surgeon used very little fat so no volume changes happened, and the fat had mostly just the growth factors that rejuvenate the skin.

A physical exam is needed before recommending an exact procedure for your situation, but I usually start skin and tissue rehabilitation with platelet-rich plasma (PRP), which can help thicken thin skin.
