I was happy with the look for one month, but the density decreased significantly after that.

Placing filler in the lower brow/upper eyelid is also reliant on technique so the filler is not lost behind the eye. While fat grafting is possible in this area, it's not as predictable, or as convenient for the patient, as a properly executed filler placement. I hope this helps you.

Yes, there are filler treatments for this as shown in the video, but the droopiness in your eyelid also needs to be properly diagnosed if it is caused by hollowing, or the weakened upper eyelid muscle called the levator muscle. I hope this clarifies things for you.

Fat transfer is possible in the space below the brow and above the upper eyelid, even though I don't recommend it done for thin lower eyelid skin. You are right about filler lasting about a year, but it is the safest, most predictable, and most convenient for the patient. Fat transfers involve surgery to graft the fat for placement, and you may need two procedures as 30-70% of the fat may be absorbed by the body shortly after the procedure. I do combine PRP with hyaluronic acid filler, especially for the lower eyelids to treat hollowing as well as well skin quality issues such as dark circles. I do find PRP does extend the effective life of hyaluronic acid fillers. I hope this clarifies things for you.

As an oculoplastic surgeon, I am also trained as an ophthalmologist, so I know this area well enough to place filler at the level that is most beneficial to the individual patient. Depending on the area being treated, if a cannula can be used, it is safer because it is not as likely to pierce or snag a blood vessel or other anatomic structures. Cannulas also reduce temporary bruising and bumps after treatment. I hope this clarifies things for you.

Sorry to hear about your problem. A single session, depending on temperature, and length of exposure, can possibly cause volume loss in this area. Other factors may have led to hollowing as well. Hello doctor, I have a different question. How about the area between eye and nose, the corner where eye meets the nose. Can filler or fat grafting possible in this location? It is like the top of nose and I don’t know it has specific name. My nose is so thin and hollow in this area and I haven’t seen any filler application in this specific spot, it is disregarded, why?

This is the lower eyelid or the tear trough, which is covered in a different video. I generally don't recommend fat grafting under thin lower eyelid skin because of the high risk of complications due to the unpredictable shape, and behavior of grafted fat. You can refer to these videos about it:

There are risks to injectable fillers placed near the eyes, however these risks are minimized by the experience, and expertise of the surgeon performing it. Hi there doc, I would like to ask, for one of my eyes, a bit of skin above my eyelid (area between brow and eyelid) droops slightly and hides the upper area of my right eyelid slightly (can't see the area where it folds). My left eye is different where I can see my upper eyelid fully. My right eye appears like its swollen but it isnt and seems like I had this for quite some time, maybe perhaps since young. I'm 24 and do you think this is an issue? Could the issue be my right eye has a different size than my left eye or genetics? doesn't affect my vision though.

Some men do have a fuller lower brow which is most often due to genetics. It likely isn't an issue, but if you are worried about it you can have it examined for a proper diagnosis. I hope this helps you.

I got upper eyelid filler yesterday and it looks really lumpy and you can clearly see the shadow of the border between the filled/ non filled skin. Have you every experienced this with a patient and what should I do? (my doc is normally also really careful and experienced..) :(

As we all have different practice focuses, I can't recommend one specifically for filler treatment in the eye area. Many doctors don't perform injection treatments near the eye as it's a delicate area, so it's best to see or ask local doctors (perhaps through their websites) if they perform cosmetic procedures in the eye area. If you do do multiple procedures for fat grafts, how often do you get a stable result with minimal complications? As opposed to body rejecting it like with the lumps in the pictures you showed?

I don't recommend fat grafting in the lower eyelids at all because of the complications, but it is possible for the upper eyelid without complications. Like you said, it usually does take more than procedure due to fat metabolization, or not taking to a a blood supply. I hope this clarifies things for you.

It is safe and effective in my approach, but the safety of placing filler in this area depends on your doctor's skill and experience, not on the filler material itself. Sorry to hear about your problem, but it would have to be properly diagnosed in an examination to determine the cause. Permanent fillers are highly risky as they stay in the body too long, which can cause a foreign body reaction or infection, and they tend to migrate to other areas. If there is an infection or a complication, surgical removal of these fillers won't remove all the material. An oculoplastic surgeon specializes in the eyelids, while a plastic surgeon is a generalist from head to toe procedures. However, not all oculoplastic surgeons focus on cosmetic eyelid procedures. I think you should look for an oculoplastic surgeon who focuses on cosmetic eyelid procedures, or a cosmetic surgeon who focuses on eyelid procedures. Risks of filler treatment are reduced by doctors experienced with filler treatment, and adept at injecting in the eye area. However, hooded upper eyelids are what most people are trying to relieve, not trying to actually attain, so you will have a hard time finding a doctor who is willing to do that for you. Filler can improve the appearance of your crease if you have hollowing in the upper eyelid/lower brow, as shown in the video. As for the lower eyelid, if you want it to appear smaller, filler won't lift it, as tear trough and cheek augmentation is meant to treat hollowing, not to lift the lower eyelid. People with low body fat can look have a more hollow appearance in their face surrounding the eyes, such as the brow, and the cheeks, which can in turn make the eyes look more hollow. This appearance can fluctuate if your body fat percentage increases, and there is extra fat volume in the face as well. I hope this clarifies things for you.

Most plastic surgeons and dermatologists I’ve met avoid injecting filler into the upper eyelids, but I’m seeing great results online. I want a touch of volume under the whole length of my brows, but I have been turned down by 3 doctors, they said I could go blind if they injected there. It’s so confusing. One doctor agreed to do PRP injections instead, and after a few days of swelling, it looked fantastic for about 3 weeks before going back to normal. I don’t know why he couldn’t just place a light filler in that area instead (like filler used for the lips), it would last a lot longer and cost the same, 1cc is enough to do both sides and do my mouth corners at the same time as well.

Injecting filler in the upper eyelids is a fairly complex procedure to avoid complications, and also to to place the filler in the right space so it's not lost behind the eyeball. Some doctors are not comfortable injecting into the delicate eye area, and some are not experienced in the right area for placement. I hope this clarifies things for you.

I have upper hollow in my right eye and that makes my eyes assymetrical since my birth. And now as i grow older and okder, the assymetry is getting far more worse. Worst is that my eyebrows are also assymetrical. One eyebrow is lifted high and is in perfect v shape while the other is not lifted and is straight rather than curvy. This makes me look creepy and loony. I don't even like to look at my own face😢😢. What to do now. Need help. Also i want to know what causes such assyemtry.

Sorry to hear about your problem. The face is naturally asymmetrical because the two halves of the skull are formed separately as a fetus. However, noticeable issues like hollow upper eyelids/ lower brows can be addressed as shown in the video.
As for the brows, I would have to perform a physical examination before recommending treatment, but it may be possible to address this non-surgically with treatments like Botox. If you have lack of fat in your upper eyelids causing them to look hollow, then you can have cosmetic filler done without requiring surgery. I heard that this is the typical result after ptosis surgery since the body's subconscious response to having ptosis is to keep the eyebrows set higher than normal.

A physical exam is needed before making any specific recommendations. As you can see in the video and thumbnail, hollowness above the eyelids can be mistaken for ptosis. Ptosis is due to a weak, detached, or stretched levator muscle, which is the muscle that lifts the eyelids. I hope this clarifies things for you.
+Amiya Prasad, M.D. Yes, ptosis is what I have (this has been examined and confirmed since I was very young) and I was born with this condition. It affects the left eyelid and it is unable to move as far as the right eyelid, a problem occurring because of the weak eyelid muscle. I have this and hollowness above the eyelids.

I would still have to assess your ptosis in person, as well as observe your habit on how you raise your upper eyelid, and keep it open. One of the signs that you are using your brow to open your eyes wider. After an exam, I'm sure we can work out a treatment plan for you incorporating all your conditions.

I have performed fat grafting successfully in this area, but keep in mind that likely more than one procedure is needed as 30-70% of grafted fat doesn't take to a new blood supply, and/or is metabolized soon after surgery.
