    A lady got filler injections in her tear troughs in August 2015. Over the course of a year, she noticed the area becoming more pillowy and saggy. She had them dissolved in two sessions, but 2 years later, her tear troughs still appear puffy, deep, and quite pronounced. She wants to know what can be done to fix them.

    Wow, after reading through the comments below I must admit all of the plastic surgeons I've met have that huge ego and superiority complex!     Thank you for this. I’ve seen several local dermatologists and plastic surgeons to address my own similar issue, and they all suggested lowers eyelid surgery instead of fillers. I felt pressured into a more expensive procedure; I think because I didn’t understand the way they were framing the underlying issue. I can understand now why they suggested what they did and will be reconsidering my options, and returning for perhaps an additional consultation.

      I'm glad the video helped clarify the issue for you, Rose Sinister. I also don't recommend fillers for more moderate to severe eye bags. I do suggest you find a doctor who does the transconjunctvial approach to eye bags, and not the transcutaneous approach that is more common. The transcutaneous approach involves an external incision, and possibly skin removal, which can lead to rounding of the eyes, and compromising lower eyelid support. It may be cheaper in the short-term, but also may involve expensive revision surgery. The transconjunctival approach requires no external incison or skin removal, so it looks natural, and does not compromise the lower eyelid anatomy. I hope this helps you.

      Was there a reason given? Filler is certainly a treatment option for the tear troughs, and involves no surgery, but it does take skill to inject the under eye area, and many doctors don't inject or operate here due to the precision and the delicateness of the eye area.

      Amiya Prasad, M.D. I actually had two cosmetic surgeons advise against it and one said I will be ruining the fascia and look worse later on and the second said surgery would give me a better result and longer lasting

      Sorry, when you said tear troughs, I thought you were referring to hollowing, not puffy eye bags. If you have eye bags, surgery is more the definitive procedure. Since I haven't done a physical examination of your eye area like your doctors have, I will have to defer to their judgment. I hope this helps you.

      I only use softer hyaluronic acid fillers under the eyes like Restylane and Juvederm to treat hollowness. I do combine them with PRP if the skin quality, pigmentation, and any discoloration of the lower eyelids also need improvement. I hope this clarifies things for you.

      Yes, that's the same approach I use - hyaluronic acid fillers for safety, predictability, and reversability; and blunt-tipped cannulas for less trauma, and improved safety. I hope this clarifies things for you.

I got versa injected 9 months ago under my eyes and now I have under eye bags... do you think I should wait until it goes away(and how long does it take for them to go away) or there is anything it can be done to correct that.       Although I don't use Versa in my practice, it is a hyaluronic acid filler, so it usually lasts about a year before being safely metabolized by the body. Hyaluronic acid fillers can also be dissolved with the enzyme hyaluronidase. The route for you to go with is entirely up to you. I hope this helps you.

    Sir ihave severe dark circles issue and realy depresed cheek bones as well.my doc did my face mapping and sugested me alot of fillers bit my main concern is that will my face look the same after the effect of filler is gone.secondly for how long restalyne fillers last .she told me it will lst for almost 1.5yrs.ihave heard that when filler efect is gone it leaves the skin saggy n you would need more amount of fillers over time..is this true.needs help.thanxalot

    Dr. What can you say about Bellafil? I know you mentioned you recommend Hyaluronic fillers but what kind of filler is Bellafil? Is it true it lasts up to 5 years? I am 26 yr old with eye bags and dark circles first time doing something, what would you recommend for me Dr? Tia

      I don't use permanent or semi-permanent fillers like Bellafil in my practice as I believe the potential complications of non-resorbable fillers pose a risk such as difficulty in removal, and a foreign substance being in the body that long. Hyaluronic acid fillers are much safer in terms of tolerance and metabolization as hyaluronic acid naturally exists in the body. I can't make a recommendation for filler or surgery for you without a physical exam, but surgery is a longer lasting and definitive treatment for eye bags.     İ have eye bags and that makes my tear troughs sooo visible and it complexes me, my local plastic surgeon tells me to remove the eye bags fat y blepharoplasty and then transfer it to my tear troughs, is it safe to do this? I know there is a risk of blindness if something is mistakenly injected to an arteria and im sooo afraid

      It is common to have puffy eye bags and hollowing at the same time, but as an eyelid specialist oculoplastic surgeon, I don't recommend the fat transpositioning surgery your general plastic surgeon described.       Eczema and dark circles can be related, and skin dryness from eczema can lead to hyperpigmentation and allergic shiners, which are factors in dark circles.       Teosyal is a hyaluronic acid filler. While I don't use this particular brand, I do use hyaluronic acid fillers in the tear trough. It's important to remember that the filler brand doesn''t determine results because it's just the material, but it's the doctor using it who determines the results. I hope this helps you.

    Hi Doctor, can you please answer this question: I am considering lower blepharoplasty, but am concerned that in some time the undereye fat might return, since my body maybe naturally "stores" fat there, also it's in my family to have very hollow and tired-looking eyes. I have had them since I was little, but now notice some fat pockets also. I am 30 y.o.

      With lower blepharoplasty, once the fat is sculpted, it does not return. I recently had a patient who I did lower blepharoplasty on 30 years ago write in to say that fat still hasn't returned, and her eyes still look great. However, lower blepharoplasty doesn't stop new orbital fat from prolapsing under the eyes. While for most people the fat doesn't prolapse again, it is still possible for new fat to prolapse, but if it does happen it's many years down the line. I hope this clarifies things for you.

    Dr. What are the chances of getting the surgery and having bags in another year? Is that a very rare case? Would you recommend surgery or filler?

      Hi Timothy Anderson,
      Puffiness coming back within a year after surgery is extremely unlikely because once the fat is removed, it's gone. While surgery won't prevent orbital fat from prolapsing forward again, for most people this is at least 10 years or away from the possibility, and in many it's permanently gone. I generally recommend the transconjunctival blepharoplasty procedure from behind the lower eyelid without any external incision, and without any skin removal as it looks more natural, like you've never had eye bags, and avoids complications that can happen with an external incision and skin removal which is often practiced with the transcutaneous blepharoplasty procedure. Unlike fillers that only camouflage slight cases of eye bags, surgery removes the fat that causes eye bags. It's important for the surgeon to leave a little fat behind for a nice contour, and to avoid a hollow look under the eyes. I hope this clarifies things for you.

      Hyaluronic acid fillers like Restylane and Juvederm are hypoallergenic, and hyaluronic acid naturally occurs in the body, so chance of an allergic reaction are virtually none. There are still risks, but risks are minimized be seeing an eye or eyelid specialist when having filler treatment in this area. Hyaluronic acid fillers can be dissolved, such as in the rare cases where lumps may form. I hope this clarifies things for you.

    My dark circles are genetic and are so deep and hollow.I am having PRP for my dark circles I have taken 2 sessions but I can’t see any changes in the hollowness.even now it is involving my cheek area?can I recover only using PRP or I should go for PRP with hyaloronic acid??

      PRP can help with pigment and discoloration that cause dark circles, but it won't have a significant effect on hollowness, so a hyaluronic acid filler is needed to treat hollowness. You can refer to these videos about treating hollowness under the eyes, and in the upper cheek:

      I don't administer Ultherapy because I find it heats abruptly rather gradually. I perform radiofrequency treatment with Pelleve, along with various lasers. However, none of these are the correct treatments for eye bags. I hope this clarifies things for you.

      There are no side effects associated with soft hyaluronic acid fillers below the eyes like Restylane or Juvederm as long as it's done by an experienced doctor, and it is the appropriate treatment. Sometimes non-surgeons try to correct prominent eye bags with filler instead of surgery, which can lead to overfilling this area. I suggest you see a doctor who is experienced in both under eye surgery and under fillers to get the best recommendation and results. I hope this helps you.

    Dr. I just went to get the fillers I really wanted Juvederm or the other one that you recommend but my local Dr. used Voluma on me she said that was the best for me, but she said I will be needing more (since I have alot of eye bag) so I will be back in a month to get the other procedure, I asked her about surgery if that would be a better option, for a long lasting but she said she wouldn't recommend because since I have alot of eye bag there would be alot of skin left, (she doesn't do surgery though) she said fillers would be best, but I've seen bad comments from people about Voluma, my Dr. also told me that this procedure will last me about 2 yrs, I want to also add that she used this mainly on the cheeck bones because she said she had to start from there to fix my problem and so Im thinking she will use something different for the next time, she will focus more on the eye bags, I am hoping she will use Hyaluronic acid next time, I wanted to ask you your opinion Dr. hope to hear from you soon.       Hi Mexi Veg,
      I do suggest you see a doctor who can perform both under eye filler and under eye surgery so there is no bias toward any treatment, and the best treatment is recommended for your case. Voluma is a hyaluronic acid filler made by Juvederm, but it is a much more rigid filler, that won't move under the eyes with normal facial expressions, so it may look lumpy and unnatural when you smile, squint, laugh, etc. Voluma is primarily a cheek filler, and I I do administer it often in my practice, but for the cheeks, jawline and chin, which are solid bone areas that don't move like the under eye area. Also, even if you remove a significant amount of under eye fat, the skin will not become loose, even in patients who are significantly older. If you have significantly puffy eye bags, and you try to camouflage them with filler, you may end up with even larger bags. It's mostly appropriate to camouflage slight eye bags with filler, but beyond a certain elevation of the bags, surgery is the best option. I recommend finding a surgeon who performs the tranconjunctival method of lower eyelid surgery as it involves no incision, is less prone to complications like lower eyelid retraction, and looks completely natural, as well as a doctor who leaves little fat behind to prevent a hollow look. If lower eyelid skin quality is an issue, talk to your doctor about complementary procedures such as PRP and laser treatment, but you can be assured that removing orbital fat will not leave extra skin behind. Removing skin because a doctor mistakes it for causing wrinkles is what most often leads to lower eyelid retraction and ectropion, which involves much more complex and expensive revision or reconstructive surgery from an oculoplastic surgeon like myself. I hope this clarifies things for you.

My husband says my eyes look a bit swollen, the Dr. Said it was totally normal and would go away in a few weeks, In my opinion I see my eyes the same.

      It's not my place to say your doctor was wrong since I have not seen you in consultation, but I can say that I don't use Voluma in the tear troughs, and do prefer softer fillers like Restylane or Juvederm XC, and I would use Voluma for harder areas without a lot of movement like the cheekbones, jawline, and chin.

    Hello Dr, can tear trough fillers move upwards into the bottom eyelid? Does it mean it can move close to the eye, potentially under or behind the eye?

      It is unlikely for a significant amount of tear trough filler to migrate upward. Amount of filler material used in the tear trough usually isn't much. If you are worried about it, and examination is needed. If a hyaluronic acid filler was used, it's normally easily metabolized, and it is not a dangerous material as the body does have its own hyaluronic acid, which is constantly being metabolized and replenished.     Can you PLEASE help? I got restylane filler for tear trough eye hollows. Now I have a (greenish appearance) tyndall effect under 1 eye in particular. How do i get rid of it? They suggested bbl laser. They already used hydrolize but the greenish appearance is still there. Help?

      There are risks to under eye filler treatment since it is a sensitive and delicate area, but the risks of under eye fillers are reduced by the experience of the doctor who is skilled at working in this area.       Teosyal is a hyaluronic acid filler. While I don't use this particular brand, I do use hyaluronic acid fillers in the tear trough. It's important to remember that the filler brand doesn''t determine results because it's just the material, but it's the doctor using it who determines the results. I hope this helps you.

      A dissolving agent like hyaluornidase only dissolves hyaluornic acid which is what many cosmetic fillers are made of. It's not possible to dissolve the cheekbone, although volume loss due to aging from bone, muscle, and fat does noticeably affect the cheeks. Options to restore volume in the cheeks do include implants and filler. I hope this clarifies things for you.

      Hi Rey Armendariz,
      If you want a long lasting treatment for eye bags, surgery to remove and sculpt the fat that causes them is your best bet. I do personally recommend and perform the transconjunctival method where no incision is made, and no skin is removed to avoid complications associated with lower eyelid surgery, and also so there is no visible incision so it looks completely natural, like you never had eye bags.       Eye bag surgery is not painful because you are anesthetized. I administer local anesthesia so you don't have to go under general anesthesia, which also makes recovery faster.     I've had dark circles since i was very young from 4 or 5 years old.       Sorry to hear about your problem. Often issues like this can be outgrown as your face is maturing, especially the hollowness.       The volume loss may be genetic, but it's best to confirm with your doctor if there are any other issues causing it. Volume loss can only be addressed by adding volume, and the safest and most cost-effective way to do this is with injectable cosmetic fillers. I hope this clarifies things for you.

    I wish you had an office in Los Angeles. I’m 27 and I’ve had dark circles since an early age. The dark circles are genetic from my fathers side. I don’t have bags under my eyes, but I have dark circles and hollow shadows. I really want to get Restylane fillers. I love how I look, I just hate my dark circles. Even when I wear concealer I still look tired.

    Is there a way the fat that cause puffiness under the eye be sucked out via needle. By inserting the needle inside the enter bottom lid and sucking the fat out. Has anyone tested or tried it.

      The fat that causes eye bags is too thick and solid to pass through a needle. Also, a needle would not give accurate results as the fat is sculpted rather than having it all removed to prevent hollowing under the eyes. There is an art to eye bag surgery to achieve a nice contour without the puffiness. I do the surgery from behind the eyelid so external excision is made, and often use laser assisted surgery as well.       Weight loss isn't likely to affect the area directly under your eyes, but it may be more noticeable after you have lost weight. Eyelid skin is naturally the thinnest skin in the body. If you are seeing the underlying anatomy through your lower eyelids, the you can consider improving the skin quality and thickness with PRP. You can refer to these videos about it:

    It seems that it may be less challenging (for lack of a better word) to correct fat prolapse than to add fat or volume around the eye area when it is extensive hollowing such as seen in aging. The process of removing is much simpler than adding when it comes to the eye area, Of course that’s only my unprofessional opinion.😁 Really helpful videos!!! Thank you.

      It is indeed much easier to sculpt fat under the eyes, than it is to put it back. One removed from its original blood supply, transferred or grafted fat needs to take to a new blood supply to remain viable. Also, fat doesn't have a consistent shape once removed from its original blood supply, which are factors in why fat is not recommended to be grafted under thin eyelid skin, which I highlight in these videos:

      An anesthesiologist is not needed for local anesthesia with sedation, or for topical or injectable anesthesia. An anesthesiologist is needed to monitor dosage and vital signs of patients under general anesthesia, and avoid general anesthesia for almost all my surgeries, including extensive facelifts. Avoiding general anesthesia is not about the money, but for patient comfort, quicker recovery, and avoiding the risks to the patient associated with general anesthesia.
