After learning this fact from your video...... I put a blindfold on for around 15-20mins and walked around the house. And all I could think was, my current face is FINE the way it IS, if there is even a 0.00001% chance of my precious sight being taken away.

Mine neither, just found out about this two weeks ago when an RN told me that a very famous celebrity doctor here in in LA won’t fill frown lines because the area is too vascular and too risky. I never knew this. I’ve put tape on my frown line every night since.
Did your Dr maybe give you a sheet to read and sign? I'm guessing a lot would cover it this way as worried it would put everyone off having the treatment saying "oh by the way you could go blind...so shall we get started then!" 😬

Excellent explanation! I had a blood vessel injected with Restalyne in the nasiolabial fold the very first time I got fillers. My whole nose developed a purple lacy pattern and got extremely cold. It was a Friday afternoon. Thank God I called the office to see if that was normal. The doctor, a dermatologist who actually studied with my husband, was mortified. He was able to dissolve it and I used nitroglycerin cream for a week, which brought excruciating headaches. He called me every night to check on me. I’ve since had Botox and under eye filler, but never in that area again.
Again Dr Gavin Chan, you are a very skilled doctor related to dermal filler injections. You give the right information and you are not avoiding the risks to tell online. Most doctors are not informing their patients about these risks. The patient needs to be informed completely and correctly. Your information is professional and respectful to your work and to the patient. Your are well aware of the risk, options and the end results of your preforming work. My personal respect for you as a doctor how is enjoying his life work.
Important info. Good to know certain areas to avoid. You are a wonderful doctor especially for refusing to treat the patient that was blind in one eye. Many drs would just do it without thinking of repercussions. Wish you were in the States!!
Yes, I agree. I in fact have only one eye, but have had dermal fillers before, fortunately that worked out well. Somewhere I did hear about blindness but it sounded just so extremely rare. I was planning on doing it again in a few months, but now I don't believe I will. We need a Dr. Chan in the US!
Things can go wrong even in the hands of the most seasoned injectors. Going blind is a very real risk. This is why I don’t understand why ultra sound guided filler injections aren’t mandatory. And why medspas-s run by nurses and sometimes even just aestheticians are allowed to inject things into people’s faces. The regulations should be so much stricter on fillers.
I agree the first time I got my lips filled was by a plastic surgeon board-certified ..this last time I went to a Medi-Spa that was very well known and very well-loved but I wasn't aware that they weren't at least one person board certified in there besides just RN's which Ill be at the end of the summer myself...it been 2 weeks but I'm scared.. last time I dont remember healing anything like this

The best dermatologists that do the fillers are knowledgeable about where the veins are! I've had this done 4 times for laugh lines and never so much as a bruise!

Now I understand why my injectors won't do certain areas. I wanted fillers under my eyes for the tear troughs and they won't do them. They'll only do the cheeks. Now I understand why. I haven't yet gone ahead but am in a few weeks. This video is honest and so informative.
I asked a cosmetic filler type of doctors to inject my year troughs too...after losing weight from being ill...and he suggested wait and see if I can gain some weight and gain a little bit of fat in the face...I am glad he suggested waiting...I was stressed and thinking fillers would make me happy...we talked about Botox on the forehead and glabella too but I wasn't ready to make a decision...this was just a consult. I later then thought about it and have heard of complications like blindness...so a year later I am feeling better and instead am using topical creams and stuff...not expecting miracles but just letting them and a little mineral makeup give me a bit of glow and self care. And so I have decided to not risk fillers..,we can't look young forever and even though risk is low I don't want to be the one where it could go wrong

As consumers (and imo even some injectors) of these types of procedures, there is a belief that these procedures are almost no risk. I agree that they have minimal risks, however, it is important to highlight the risks involved. You've also highlighted the need for CPD, for injectors. Absolutely. I had dermal filler injected slightly to the outside and underneath my orbital bone. The only vein checking done was a slight pulling on the syringe. I reported to the doctor my horrific bruise and he blamed me. Later my skin fell off atop the injection. He burst one of my blood vessels near my eye from having injected the entire syringe into it. Still "not his doing" and i seriously doubt he reported it. Now i have a depression almost the size of a dime beneath my orbital bone on my cheak. I told him i was afraid it was going to effect my eye since the bruising came so close to it. That entire side of my face turned black and nearly ten years later is still discolored with blotchy redness and a big white scar. Thank God that was what occured and not the permanent loss of vision. It also hurt like stinging death at the time of injection but i was not told i should not feel pain.
I came here because I was thinking I’d like to have filler in my smile lines... The information you provide has made my decision easier. I have changed my mind. I will try something less invasive with very little risk if losing my sight. Thank you.

Don´t be so radical please. Fillers beautify most people with minor to any complication. This is general info, go to a good practitioner and your risks will be close to none ;;D

I had filler injected in my nasolabial folds over 10 years ago by a nurse and she injected so deep, she hit my artery. Fortunately, the filler didn’t cause me to go blind thank God but it irritated that he artery and created a ‘tick’ in my face like a noticeable pulse especially when the weather is warm. That caused me enough stress but it’s nothing compared to going blind. It didn’t make me feel lucky but I guess I am. You never know how close you get to a traumatic situation and then are saved from it...
I know your comment is old now, but I’m experiencing the tick you describe and haven’t found anyone else describe this! Did you have it dissolved? Or did it go away on its own eventually?

I went to the temporal aria fillers Voluma today. And the first doctor, who told me that there is a possible risk of blindness. No one has ever told me that before. :( As a result, I made my cheekbones, (this of course will not help lifting as the temporal aria, but still there will be a some lifting effect) The doctor said the risk is less in the cheekbones. The doctor is very professional. And two years ago I did filler in the bridge of the nose. Now the nose is taboo. A few years ago, a surgeon told me to be very careful with fillers. And he said “ I shouldn't say that though. But just be very-very careful! “ Now I understand what he meant. It seems that beauty doctors have a mutual guarantee not to disclose such serious side effects! And I am glad, that there are still doctors who, to the detriment of their interests, provide such important information about this terrible side effects
I get filler in my nasal folds and I have never regretted anything I dont my whole life as much as this decision it looked great the first 3 months and i believe it moved and made my cheeks look weird and uneven, I am so scared that it may migrate into an artery one day as i don't think it disolved like they say it will such a scary thing I hope no one gets this side effect , such a scary thing that they never tell you could happen
This is terrifying , I've had Sculptra 2 times , once 3 years ago and once several weeks ago , one vial only - didn't want to be too puffy . I read about the blindness after the first time , and asked my provider about it before this second time .. It's really important to know your provider , that they have been in business a long time , etc.I would never go to some house party down the block with the neighbors if you know what I mean . Very scary, shocking and frankly off putting. The EU requires a medical license to apply fillers but here in the UK it is not regulated. Your average "beauty therapist" will have little knowledge and zero experience in the event of something going wrong but these procedures must be happening in their thousands every year. This from the British Association of Aesthetic Plastic Surgeons site: Two out of three surgeons seeing botched filler ops -Survey reveals 84% of problems with permanent fillers require surgery or deemed untreatable. You can only do this by injection of filler under ultrasound guidance. As far as I know, virtually no-one does this. It is very cumbersome to inject whilst scanning
There’s a plastic surgery clinic in S Korea called “Me Clinic” that claims to use it. I’m planning on getting fillers and nose job done there in about a year or less

MyEpiphany77 Botox doesn’t cause occlusions virtually impossible. You don’t need to aspirate when doing Botox. It’s superficial. Even if you hit an artery it would just swell/bruise at worst.

Hi Dr. Thanks for your video. I just had cheek and lip filler done. The nurse refused to do my nasal labio folds which I appreciated as she was concerned about doing this. If anything were to happen, I.e. blindness, would that happen at the time of injection or can it be deferred?...is the tingling feelings and sensations normal?
Doctor, I realize this is an older video, but I want to know should unfortunate accidents happen to patients WHERE is this supposed to be reported?? Doctors are board certified but nurses doing these procedures are not so how does the public check their "safety" record??
I don't think people know about this particular risk... also, does this apply to botox injections into arteries and does risk also apply to lip injections? ( that last one might be a dumb question )
You are right - many patient (and doctors!) don't know about this risk. It does not apply to Botox injections. If Botox is injected intravenous, it doesn't block the arteries. It is not viscous like filler. It is more like water in it's consistency.
There have been cases of blindness from lip injections, but only a few cases around the world, and probably millions of filler injections done worldwide.
Quick question regarding the risk of getting blind because of fillers: Wouldn't a Cannula, in comparison to a needle, completely get rid of the risk of going through an arterie and therefor also eliminate the risk of going blind?
Thank you, Dr. Chan, excellent video. I would update with a few preventative/treatment points for practitioners and informed patients to perhaps ask about/be aware of, see reference to article below. 1. Placing the tips of both index fingers on both sides of the top of the nose during injection of filler can help prevent any filler which inadvertently enters a facial artery from traveling retrograde into the Ophthalmic Artery to the back of the eye. Very simple preventative technique. (The Ophthalmic Arteries are located on either side of the top of the nose, before they travel to the back of the eyes, and flow is easily prevented by manual pressure with a finger tip). 2. Inject slowly, ask patient to report symptoms real time, and, importantly, inject no more than 0.1ml at a time (which is about the volume which can fill the vessel to travel retrograde). 3. If signs or symptoms suspicious of filler moving to back of eye occur, inject hyluronidase filler dissolver into the Supraorbital and Supratrochlear vessels, 1cc with pressure, and repeat. These vessels are relatively easily accessible to plastic surgeons and dermatologists as they are located directly above the eyes, and injecting them deposits the hyaluronidase directly into the vessels - they don’t depend upon diffusion the way the retrobulbar injection mentioned in the video does (further, retrobulbar injections are not done by plastic surgeons and even when performed don’t seem to be very effective). More discussion can be found in the article below and other similar articles. The best way to avoid complications is to be educated and prepared. This was very informative. Thank you. Question: why isn't the dissolvant you mentioned injected as a precautionary method prior to any dermal filling in the high risk areas of the face? From the things you said, it seems that it should be mandatory. Prevent it instead of trying to treat after the fact.
That’s an interesting question Joanna, it would counteract the filler all together. Hyaluronidase is an enzyme which breaks down hyaluronic acid. To inject it prior would mean that in the presence of the enzyme, the filler would simply dissolve as it was being injected, which means the procedure would be nullified and voided instantly upon the injection of the material. Hyaluronidase (as Dr Chan very informatively explained) is only used after a complication is present, not before a complication may happen :D lol.
It seems to me that if the pressure of the filler never exceeds the blood pressure of the patient then it would be impossible for the filler to fight its way up an artery. I imagine a syringe with a relief valve that is set to open if the pressure exceeds say one half of the blood pressure of the patient. Check it out as I think this would work.

I was having temples filled and after the first side I started seeing double and felt nauseated. The numbing injection went into muscle and caused lazy eye. It was back to normal by the next day. I haven't done a thing since. 😱
I have sever empty nose syndrome as a result of a turbinate reduction and septoplasty .I am going to see laurent castillo in nice pastuer university hospitol for hyaluronic acid injection in my inferior turbinates.he has been doing this for over 10 years .because in this video he says blindness Is rare risk and because of my ens I will have the h.a done.
Always assumed hyaluronic acid was a naturally occuring filler and that your body dissolves it after a period of time . Never thought it would be potentially dangerous !! 😳
wow, i am going to be fully honest. i have been doing fillers in my lips for 15-20 years.the first 5 done by about 7 different plastic surgeons. i tried every single filler there is,and none of them last for crap. maybe i love my lips for 6-8 weeks. BUT i was making great money so i kept doing it consistantly. after 5 years i started to order my own filler from Canada, and do them myself. for one thing i was able to numb myself so much better,and also i could put as much filler in as i wanted. actually i did it again about 5 days ago. THEN i heard for the first time this blindness thing. i am not doing them again. i AM blind in one eye (trauma from childhood attack). this info scared the heck out of me. i soent all day trying to see if there was a way to do it to make it almost impossible for an injury to happen. using cannulas, and really studying the facial vascular map .wow! what a tangled mess! i ordered 6 more mls, already on the way, but i wont be using it anymore. technique makes it a lesser problem, but now i know with the map of arteries that can be here and there even that isnt worth it. i do LOVE my lips big, pouty and plump so i am going to hope that eventually they will find a good, BIG permanent implant. 2 questions, are the new pen injectors very dangerous. i have seen them used and they sure dont go in very deep, and they also deposit such a tiny amount? also do you know of any good permanent fillers? something called softform came out a while back but i then never heard anything about it,so i figured they had issues with it. i wasnt going to get it cuz they only offered 2 sizes and for me they were both too small.???anything new?

I later then thought about it and have heard of complications like blindness...so a year later I am feeling better and instead am using topical creams and stuff...not expecting miracles but just letting them and a little mineral makeup give me a bit of glow and self care. And so I have decided to not risk fillers..,we can't look young forever and even though risk is low I don't want to be the one where it could go wrong

As consumers (and imo even some injectors) of these types of procedures, there is a belief that these procedures are almost no risk. Absolutely. I had dermal filler injected slightly to the outside and underneath my orbital bone. The only vein checking done was a slight pulling on the syringe. I reported to the doctor my horrific bruise and he blamed me. Later my skin fell off atop the injection. He burst one of my blood vessels near my eye from having injected the entire syringe into it. Still "not his doing" and i seriously doubt he reported it. Now i have a depression almost the size of a dime beneath my orbital bone on my cheak. I told him i was afraid it was going to effect my eye since the bruising came so close to it. That entire side of my face turned black and nearly ten years later is still discolored with blotchy redness and a big white scar. Thank God that was what occured and not the permanent loss of vision. It also hurt like stinging death at the time of injection but i was not told i should not feel pain.

Botox is a nice way to treat smile lines with very little risk. Generally fillers are not used for smile lines or crows feet

Hi Dr Gavin Chan! How much time passes until you start notice symptoms of blindness from nose fillers? What signs should a patient be attentive of? Will it show already at the clinic or after the patient has left?
If the blindness is going to happen, it will happen straightaway, whilst the person receiving fillers is still in the treatment chair.
I had filler injected in my nasolabial folds over 10 years ago by a nurse and she injected so deep, she hit my artery. Fortunately, the filler didn’t cause me to go blind thank God but it irritated that he artery and created a ‘tick’ in my face like a noticeable pulse especially when the weather is warm. That caused me enough stress but it’s nothing compared to going blind. It didn’t make me feel lucky but I guess I am. You never know how close you get to a traumatic situation and then are saved from it...
I know your comment is old now, but I’m experiencing the tick you describe and haven’t found anyone else describe this! Did you have it dissolved? Or did it go away on its own eventually?

I went to the temporal aria fillers Voluma today. And the first doctor, who told me that there is a possible risk of blindness. No one has ever told me that before. :( As a result, I made my cheekbones, (this of course will not help lifting as the temporal aria, but still there will be a some lifting effect) The doctor said the risk is less in the cheekbones. The doctor is very professional. And two years ago I did filler in the bridge of the nose. Now the nose is taboo. A few years ago, a surgeon told me to be very careful with fillers. It seems that beauty doctors have a mutual guarantee not to disclose such serious side effects! I get filler in my nasal folds and I have never regretted anything I dont my whole life as much as this decision it looked great the first 3 months and i believe it moved and made my cheeks look weird and uneven, I am so scared that it may migrate into an artery one day as i don't think it disolved like they say it will such a scary thing I hope no one gets this side effect , such a scary thing that they never tell you could happen

This is terrifying , I've had Sculptra 2 times , once 3 years ago and once several weeks ago , one vial only - didn't want to be too puffy . I read about the blindness after the first time , and asked my provider about it before this second time .. It's really important to know your provider , that they have been in business a long time , etc.I would never go to some house party down the block with the neighbors if you know what I mean . thanks for such an informative video. I watched a video which highlighted the disastrous effect of misplaced filler and the resultant necrosis of the artery and skin on several patients. Very scary, shocking and frankly off putting. The EU requires a medical license to apply fillers but here in the UK it is not regulated. Your average "beauty therapist" will have little knowledge and zero experience in the event of something going wrong but these procedures must be happening in their thousands every year. This from the British Association of Aesthetic Plastic Surgeons site: Two out of three surgeons seeing botched filler ops -Survey reveals 84% of problems with permanent fillers require surgery or deemed untreatable. The top reasons surgeons feel complications occur with injectables are:
Unqualified practitioners are administering fillers incorrectly
Lack of regulation has allowed unproven substances to be used in the UK
Patients think of them as an easy treatment that can be administered by anyone
Patients are unaware of the risks and side effects involved with the procedure

You can only do this by injection of filler under ultrasound guidance. As far as I know, virtually no-one does this. It is very cumbersome to inject whilst scanning

Imagine injecting botulism toxin (Botox) into your artery, where blood flows to your organs! The damage is irreversible.
Botox much safer than filler but filler work better to be look attractive.
MyEpiphany77 Botox doesn’t cause occlusions virtually impossible. You don’t need to aspirate when doing Botox. It’s superficial. Even if you hit an artery it would just swell/bruise at worst.

Hi Dr. Thanks for your video. I just had cheek and lip filler done. The nurse refused to do my nasal labio folds which I appreciated as she was concerned about doing this. If anything were to happen, I.e. blindness, would that happen at the time of injection or can it be deferred?...is the tingling feelings and sensations normal?
Extreme stinging pain is what you need to watch out for.

I don't think people know about this particular risk... also, does this apply to botox injections into arteries and does risk also apply to lip injections? ( that last one might be a dumb question )
You are right - many patient (and doctors!) don't know about this risk. It does not apply to Botox injections. If Botox is injected intravenous, it doesn't block the arteries. It is not viscous like filler. It is more like water in it's consistency.
There have been cases of blindness from lip injections, but only a few cases around the world, and probably millions of filler injections done worldwide.

Quick question regarding the risk of getting blind because of fillers: Wouldn't a Cannula, in comparison to a needle, completely get rid of the risk of going through an arterie and therefor also eliminate the risk of going blind?
Thank you, Dr. Chan, excellent video. I would update with a few preventative/treatment points for practitioners and informed patients to perhaps ask about/be aware of, see reference to article below.
1. Placing the tips of both index fingers on both sides of the top of the nose during injection of filler can help prevent any filler which inadvertently enters a facial artery from traveling retrograde into the Ophthalmic Artery to the back of the eye. Very simple preventative technique. (The Ophthalmic Arteries are located on either side of the top of the nose, before they travel to the back of the eyes, and flow is easily prevented by manual pressure with a finger tip).
2. Inject slowly, ask patient to report symptoms real time, and, importantly, inject no more than 0.1ml at a time (which is about the volume which can fill the vessel to travel retrograde).
3. If signs or symptoms suspicious of filler moving to back of eye occur, inject hyluronidase filler dissolver into the Supraorbital and Supratrochlear vessels, 1cc with pressure, and repeat. These vessels are relatively easily accessible to plastic surgeons and dermatologists as they are located directly above the eyes, and injecting them deposits the hyaluronidase directly into the vessels - they don’t depend upon diffusion the way the retrobulbar injection mentioned in the video does (further, retrobulbar injections are not done by plastic surgeons and even when performed don’t seem to be very effective).

From the things you said, it seems that it should be mandatory. Prevent it instead of trying to treat after the fact.

The dissolvant would dissolve all dermal filler injected so there is no point putting dermal filler in.
That’s an interesting question Joanna, it would counteract the filler all together. Hyaluronidase is an enzyme which breaks down hyaluronic acid. To inject it prior would mean that in the presence of the enzyme, the filler would simply dissolve as it was being injected, which means the procedure would be nullified and voided instantly upon the injection of the material. Hyaluronidase (as Dr Chan very informatively explained) is only used after a complication is present, not before a complication may happen :D lol.
It seems to me that if the pressure of the filler never exceeds the blood pressure of the patient then it would be impossible for the filler to fight its way up an artery. I imagine a syringe with a relief valve that is set to open if the pressure exceeds say one half of the blood pressure of the patient. Check it out as I think this would work.

So do the arteries tend to run deeper in the dermis, and not injecting too deep can help to prevent this? How many millimeters below the surface would one find a facial artery...on average?
I was having temples filled and after the first side I started seeing double and felt nauseated. The numbing injection went into muscle and caused lazy eye. It was back to normal by the next day. I haven't done a thing since. 😱

wow, i am going to be fully honest. i have been doing fillers in my lips for 15-20 years.the first 5 done by about 7 different plastic surgeons. i tried every single filler there is,and none of them last for crap. maybe i love my lips for 6-8 weeks. BUT i was making great money so i kept doing it consistantly. after 5 years i started to order my own filler from Canada, and do them myself. for one thing i was able to numb myself so much better,and also i could put as much filler in as i wanted. actually i did it again about 5 days ago. THEN i heard for the first time this blindness thing. i am not doing them again. i AM blind in one eye (trauma from childhood attack). this info scared the heck out of me. i soent all day trying to see if there was a way to do it to make it almost impossible for an injury to happen. using cannulas, and really studying the facial vascular map .wow! what a tangled mess! i ordered 6 more mls, already on the way, but i wont be using it anymore. technique makes it a lesser problem, but now i know with the map of arteries that can be here and there even that isnt worth it. i do LOVE my lips big, pouty and plump so i am going to hope that eventually they will find a good, BIG permanent implant. 2 questions, are the new pen injectors very dangerous. i have seen them used and they sure dont go in very deep, and they also deposit such a tiny amount? also do you know of any good permanent fillers? something called softform came out a while back but i then never heard anything about it,so i figured they had issues with it. i wasnt going to get it cuz they only offered 2 sizes and for me they were both too small.???anything new?
