Hyaluronic acid dermal fillers can be reversed with hyaluronidase, an enzyme that catalyses its breakdown.
The reasons for reversing dermal fillers range from incorrect placement to emergency situations to restore blood flow when dermal fillers block arteries.
Why do people want to have their cosmetic fillers reversed?
Reason 1 for getting cosmetic filler reversed: Incorrectly placed filler
If dermal filler has been incorrectly placed, it can be corrected.
Reason 2 for getting cosmetic filler reversed: Too much filler
Having too much dermal filler treatment can lead to a result that looks unnatural and ‘not like you’. We can help you correct this.
Reason 3 for getting cosmetic filler reversed: Asymmetrical filler
If cosmetic filler has been placed asymmetrically in your face, it can be reversed. You don’t need to put up with asymmetrical filler.
Reason 4 for getting cosmetic filler reversed: Lumps
Lumps and bumps after having cosmetic filler can occur. If these do not go away after a few weeks, we can help you correct them.
Reason 5 for getting cosmetic filler reversed: Tyndall’s effect
Cosmetic filler is a clear gel so when light reflects through the skin it will appear to be a blue colour. The Tyndall effect is a light to dark blue discoloration that appears mainly around the eye area if the fillers are injected too superficially into the skin.
Reason 6 for getting cosmetic filler reversed: Infected filler or granuloma
For patients with this issue, it is important that they are on a course of antibiotics prior to dissolving the filler. If the filler is dissolved while it’s infected, the infection will spread as the hyaluronidase breaks it down.
Reason 7 for getting cosmetic filler reversed: Emergency arterial occlusion
While this is very rare it can happen when the filler is injected too close to an artery. An experienced and highly trained Cosmetic Doctor and Cosmetic Nurse will notice this because when an artery has been occluded or partially occluded the skin will become blanched, white or mottled indicating a lack of blood supply

hi for all the people worried about the hyluronidase effects - i just wanted to say that i had to have many, many rounds of hyluonidase over 1 year+ period to dissolve multiple, persistent filler in many areas of my face including under eyes. Thank fully my skin is totally fine (2 + years without filler now) and all the hyluroindase has not left any change to my own skin at all. To all of your poor people suffering with filler you dont like/want - get it dissolved! it was an incredible relief of stress and saving grace for me when i found hyluronidase x be well and thank you doctor for all your wonderful videos.

Such good information. I am not comfortable going to a practitioner that doesn’t keep this on hand. And shocked at how few keep this in case of emergency.
Like in my case, this Baylor board certified plastic surgeon didn’t help me after he injected in two places. I still have those bumps on my face. I believe my body caused scar tissue around the bumps and have kept them as a bump for 13 years. Any suggestions would be appreciated!

Hello Dr. Gavin. Please answer to my question cause it's actually very important for me (and for everyone I believe)! In the past I had some Restylane being injected under and around the eyes. More recently, in July 2017, I had an injection of Restylane on the nose and a tiny bit between the eyebrows. I wish I had watched your videos sooner. It probably would have made a huge difference! I had an OCT in August 2017 proving that I had some spots on the retina of the left eye. Doctor said I had nothing to worry about and it wasn't degenerative, would probably not affect my vision and stay like that for the rest of my life. I went to do Fundus Oculi recently and another doctor this time said that some almost invisible spots are present in the right eye too and on Monday I will do OCT for this. How do I know that the filler is still not clogging the arteries leading to the eye after 2 years? Can I be sure that the arteries are now working properly again and no filler is in there?? And is the filler-induced damage going to stop or is it progressive? Thank you!

Hello. I had tear trough filler 2 years ago and then recently i noticed that my cheeks are getting fat and i suspect that the filler might have migrated to my cheeks. Do not hesitate to insist that you want Hylarondaise injected in the areas of puffiness. They will try to talk you out of it because it cost them money but I have in fact, I have gone back several times for even more dissolver because they always under do it it seems and don't add enough of the dissolver. Trust your instinct, and you know your face better than anybody else
I love your videos, I’ve been on sort of a binge watch.. but I’m wondering if you can do on the dangers of hyaluronidase? I’ve been reading a lot of reviews on RealSelf where people talk about hyaluronidase ruining their natural tissue, having advanced aging and developing an autoimmuned disease because of it, but I haven’t seen any doctors really discuss this
I don't believe this is true. I have literally done thousands of hyaluronidase injections with no problems. Our own hyaluronic acid turns over daily so we can dissolve it with injecting hyaluronidase.

I wonder why the very good doctors are ignoring this very important question ? All they say is just go to a board certified doctor n it should all be fine but they probably do not have a definite answer to these real time issues . My understanding is that in terms of cosmetic surgery procedures we r at an experimental stage right now , we r not THERE yet
to be honest, plastic surgery and the cosmetic injections world is not exact science. Some people may get no damage to their own tissue (own HA, fat tissue, connective tissue, muscle tissue) after hyaluronidase. But other do get unwanted effects like the fat tissue is affected, it looks unpleasing, and there are contour problems caused by hyaluronidase. Maybe there are better ways to get rid of unwanted filler (facial exercises, drainage of fluids out of your face, like the water that attaches to HA molecules, or massage, or vibromassage (there are plenty of such devices for face on the market))
I for instance now look puffy after a plastic surgeon injected too much HA filler in my tear trough are (0.6 ml on each side of the face). It makes my face look unnaturally puffy, and i hope to break it down faster with massage, facial exercises, lymph drainage...

I’ve had it multiple times to try and dissolve years old fillers and I’ve had no issues. Your body reproduce says hyaluronidase every day. It breaks the blinds on the filler which is the difference.

Why are there so many negative reviews for this dissolving agent in regards to it destroying the native tissue. I am getting my undereye filler dissolved and am worried. I know you are saying any native hyaluronic acid regenerates itself but the reviews on this are discouraging.
Because it really is awful stuff.

Filler can be detected on MRI scans. MRIs show the layers of the skin (ex. dermis, epidermis, smas, etc) & dermal fillers pretty clearly. There are new studies using MRI scans specifically to detect (& prove) filler migration.
Quick question regarding the risk of getting blind because of fillers: Wouldn't a Cannula, in comparison to a needle, completely get rid of the risk of going through an arterie and therefor also eliminate the risk of going blind?

But isn't it a fact that natural HA holds collagen together and if we temporary loose HA in our skin, wouldn't that mean that we also loose collagen? And collagen has not such a rapid turnover like HA...

Yes it makes sense but it is replaced within one week. So i guess that week you lose collagen. I guess one could take compensatory measures to grow collagen back eg redlight, microneedling, rf.
Also i think most filler is injected where the fat is and it does not grow collagen there. So if they inject in the fat you would not lose collagen.
xen ia i think there is a definite possibility of permanent loss of something - the way courtney cox looks now after dissolving her fillers is clearly not a natural outcome, the dissolving has definitely taken away some of her natural fullness. there are plenty of reviews and photos on realself.com to that effect as well. i think the question is, why does that happen in some cases (injected too deep? amount used?) and how to prevent it. i hope dr. chang does an updated video on hyalurondaise and proper technique/dangers to help educate us all with new knowledge that’s been gained since this video
I had filler dissolved a few weeks ago and it definitely disfigures your face. I’m not sure why doctors are saying that it doesn’t. It’s kind of crazy actually. In the reviews I read on realself a common answer from doctors that I see is that your face is probably reverting back to what it was before the filler and you probably were used to seeing the filler and now that it’s not there the returning hollowness is revealing itself. this is wrong. i literally have photos of my face before doing anything and what it looks like now and there is loss of fullness from what my natural face looked like before doing anything. I had 1 ml of restalyn under my eyes and this was the only thing i’ve ever done to my face and I am now left with my face looking aged and trying to cope with the fact that my face will never look the same as it did before. I have two large indented lines now going down my cheeks that were 10000% not there before. To anyone thinking about getting filler: DO NOT GET FILLER unless what is bothering you on your face is literally ruining your life. It was the biggest regret of my life and I would do anything to go back to what it was before. Hyaluronidase does effect your natural tissue/hydraulic acid. Please don’t let people tell you otherwise

I received fillers back in 2007 and I got a bump in each location. The doctor didn’t help me afterwards those bumps are still there. Do you have any suggestions? I cannot reverse mine completely. 3 syringes injected rabdomly into deep dermi, so not too deep. Do you think rf microneedling will get rid of it? Some drs say it dissolves filler.

Don’t do RF micro needling to dissolve filler. The only thing it melts is your own fat because filler is too deep. It will melt your fat before it melts the filler.

No we don't usually find filler migrating that far. Just in the local area usually.

I would so appreciate your opinion on the following: 2 years ago I tried lip filler but along with hating the result started to develop lumps and so I had it all dissolved. The doctor seemed to use a lot of hylase and since then I have been struggling with dry lips. Could my native HA have been permanently damaged, or is this coincidental?
I dont know the answer to your q but there is a strong link between b vitamin levels esp b12 and dry lips. mine were so chronic for months it gave me above lip wrinkles. When I found this out and got my blood tests back that I was low in it and hopped on vitamins my dry lips and chapping went.

How localized can it be used? I have certain areas if my face where I want filler to be removed (lower cheeks) but areas where I want the filler to remain (jawline). Is it possible to dissolve smaller areas?

Thanks for your instructive videos! I had a dermal filler on my nadiokabial (actually I didn’t want it but the dicta made it without my consent). Unfortunately it looks very unnatural thought it’s only one ml. I want to the doctor and he says he doesn’t see the filler, and that the extra volume on my cheeks is fat…. This is a hard one... I will be doing a video on temporary vs permanent fillers soon. Other options for granuloma include 5-FU injections, but you will need the guidance of a doctor experienced in this. Otherwise... surgery!

I had filler in a scar but didn’t like the result so I decided to dissolve it and I did its been for 6 days and there is a yellow color in one area where the hyaluronidase was injected. I was injected in another area close to it but it’s normal. Is this a common thing that happens when using this injection? Im just worried

What about the same mishap occurring with *RADIESSE* - it's *not* reversible - so what happens if it enters a blood vessel?
I think the chances of recovery no matter what filler you use - permanent, temporary, hyaluronic acid or non-hyaluronic acid filler, are extremely slim. I believe there have been very few reversals of blindness once the filler enters the arteries supplying the retina.

I had a doctor use Volure for tear trough which destroyed my collagen. Has taken 6 years to look ok. Still has a bluish color. Bad mistake.

My dr suggested microneedling with prp and erbium skin resurfacing plus zo skincare ( retinol twice a week for starter, growth factor serum, daily defense). Is it possible though that your filler was injected in the dermis and thats what you feel? Because thats what i feel and i was injecting hyaluronudase in the subcutaneous then the idiot dr told me she injected in the deep dermis, which does feel like thick and a but plastic/ rubbery
@@italianlessonsnyc1176 Yes, it feels like plastic or leather texture, very think and my make up doesn't blend, instead as I have lost the skin texture I can see how the make up get inside my pores and that looks very bad because it looks like of I have white spots :( The skin is also thick and red/purple colour. My doctor recommended me to use a cortisone cream but hasn't improved. Did your skin improve after all that treatments you are recommending me? It is a lot of things and treatments to be done, must be expensive. Some Doctors say that Hyaluronidase damage your skin although many just deny it. To be honest I don't know if I will ever be able to get my skin back. Did you get red/purple skin colour where it was injected? x
Myself86 i never heard of skin thickness as a side effect. It usually makes it thinner albeit temporarily they say and on me it creates tiny wrinkles that take a long time to get better, dryness, discoloration and enlarged pores. That is what i got at least. I got a dent once. Is it possible you have filler left that causes induration? Bc i have tmthis thick rubbery feeling where the filler is still present. Alex Gzz dont be scared, if they know what they are doing no damage from hyaluronidase. Go to a dermatologist for this. Nurses and plastic surgeons are not good at dissolving in my experience.

This is very useful video and this doctor is so informative. Although I dint have any fillers, it is good to know potential side effects so that no one can full me in the future if I maybe decide in my wiser age to do something with my face
I now swell up badly after hyalauronidase when I didn’t used to ever have a reaction. I don’t have an allergy to it that I know of. Do the products vary that much? Very odd. Any thoughts on why this could be? Thanks so much 😊
Panda1122 A I’ve read up on it and hylenex is derived from human tissue while others can be bovine, sheep or pig. Don’t know if that makes a difference

This is a good question. Our body's own hyaluronic acid is temporarily dissolved by the hyaluronidase, but because our body's own hyaluronic acid is turned over every 24 hours it doesn't dissolve permanently and therefore does not cause sagging. Interestingly, hyaluronidase is often mixed with local anaesthetic and injected into the face, including behind the eye prior to cataract surgery. This helps to temporarily breakdown the body's hyaluronic acid and spread the local anaesthetic. Often large doses of hyaluronidase are used in these instances and it causes no sagging of the area injected.
@@viccosmeticinstitute It caused sagging and dents in my face. Not enough research yet to establish that it doesn't dissolve your own tissue - that is a dangerous message to put out there. The fact there are hundreds of patients that report similar damage from the dissolver should provide some evidence that it can dissolve your own tissue.

I had Juvi injected under my eyes 1 year ago. Three rounds of Hyaluronidase later and only my right side looks normal. The left side is still elevated. Any suggestions?
sometimes Juvederm can take a few goes to dissolve, but usually 3 is enough. Have you compared your previous photos? Could it be fat that you are looking at?
It's somewhat dissolved. When I wake up...my under eye is puffy. Goes away during the day.

Thank you for the informative video. I recently had Juvaderm Voluma filler placed into my cheeks with less them optimal results and I am considering getting the fillers dissolved with hyaluronidaise. I had a few additional questions from your information. You mentioned that haluroniaise can temporarily reduce some natural hyaluronic acid, but if this is the case that the natural hyaluronic acid would regenerate or turnover quickly. Are there any instances in which the natural hyaluronic acid wouldn't regenerate? Also, are there any particular risks or special considerations associated with hyaluronic injection in the cheeks as opposed to other locations? How would one distinguish between a granuloma or fillers that as been placed superficially? (I have a palpable hard feeling lumpy area on my left cheek and I am not sure if this is just filler that was placed to close to the surface or if it is potentially a granuloma). Thank you.
Jelena JM I would say let it dissolve by itself. There are so many bad cases where people got their filler dissolved and their faces aged 20 years in 2 weeks. Are you sure u want to take that risk. I know I’m no doctor. But I trust people’s experiences more than what the doctors say. Because honestly these doctors act like they know everything but they don’t. Go on real self see how many doctors claim that Hyaluronidase won’t destroy your own collagen. Almost all. But if you ask people’s experiences with that dissolvant. They will tell you that it melted all there collagen and facial fat. And now they have ruined there faces. Doctors lie to you. At the end of the days it’s your face. Make a wise decision. Make sure you do your research.

there's no other way. Why do you not want to use hyaluronidase? It is quite safe.
