@@andrewbrown3156 The only situation in which a warhead creates a nuclear detonation is when the warhead was primed and detonated on its own. A secondary explosion would set off the primary, non-nuclear explosives, which would ruin the warhead and turn in into essentially a dirty bomb.

Yes, all modern destroyers has Nuclear, biological and chemical (NBC) to washdown the ship. Considering the Nuke detonated at higher altitude and was maybe low yield, there shouldn't be any tsunami waves.
It would of been fine. Far enough way from the detonation to be out of range of the thermal pulse, maybe it could have had its masts damaged and windows broken from the shockwave but otherwise it'd be completely fine. The crew might have temporary flash blindness if there was anyone looking in the direction of the incoming warhead.
Depends on the size of the warhead and the distance of the explosion. Interception seemed to have occurred well above 10 kilometers. North Korea allegedly has a warhead of 250 kilotons, according to a blast wave calculator from MIT that could shatter windows at a distance of just over 10 kilometers from the explosion. If a ships windows are not stronger than regular windows in your house this could lead to shattered windows and related injuries, but otherwise it shouldn't cause much damage. There is a high likelihood for first-degree burns for anyone with a direct line of sight to the fireball, though Note: The effects as depicted in the scene are way overblown.
