It took me quite a few trys but I got him. Power stance build. +9 Bloodhound Fang (R) and +9 Zamor Curved Greatsword (L). Wearing Iji's Mirrored Helm,, Raptor's Black Feathers and Beast Champion gauntlets and greaves.

Radagon Soreseal
Claw Talisman
Dragoncrest Greatshield Talisman
Silver Scarab (I forgot to put the Boltdrake Talisman back on)
Used Golden Vow once at beginning.
Drops entire Malformed Dragon armor set upon death.
