i guess the fact it's fiction, and not history helps....GRRM made a lot of efforts to make his world as realistic as possible while having a drop of fantasy in it (extinguished dragons, a world of fantasy beyond the wall with skinchangers, giants, others and children of the forest, and a bit of human sorcery in the east) But, with the continent of westeros being almost ridden of magic, it's supposed to be a realistic medieval set up, but it has flaws : the fact that the targaryens were able, once their dragons died, to maintain an unified empire with a purely feudal structure, of landed knights pledging allegiance to their lords, who pledge allegiance to their liege lords, who pledge allegiance to the lords paramounts, who pledge allegiance to the Targaryen king, with each paramount lord being a de facto king in his own fiefdom, and each lord a small kinglet on his own domain, doesn't realy makes sense. Westeros should have split apart the minute the last targaryen dragon died. Feudal european kingdoms were purely theoric structures originally with the king being a symbolic figure, with no real military power beyond his own personnal fief, and no mean to levy taxes and to implement a real governance. European kings were able to eventually unify their kingdoms, by granting autonomy to increasingly bigger cities and towns, who got independant from their lords, and the kings could there implement a royal administration in the cities who progressively took over small rural lords and landed knights while high lords were subdued by war or marriage. Kings played the lords versus the bourgeois (non-poor urban commoners) while using, and later submitting, the superstructure of the Catholic Church, who had monopole over not just religion, but also science, architecture and education, and owned a lot of fiefs in his own right. The catholic church in Westeros is basically incarnated in the septons and the maesters, but maesters don't seem to answer to the king in anyway. No royal administration either. Plus, it's not realistic that there is one single language in Westeros. GRRM Matrin described it as as long as South America (although far thinner) and populated with twenty millions inhabitants, and with different ethnic backgrounds (first men, andals and rhoynar) : there should be tens of different languages....

Despite not being a carbon copy of our real history, I still think its incredibly grounded and realistic with fantasy elements. This is actually what I like about it the best. Its humans squabbling in a world that is believable, they don't behave how we wouldn't etc. But these elements like Euron being insane, the Wall, the Walkers, oily Black stone, the mysteries.. its just so interesting.

From what I read and from official artwork, it's a yes and a no. It seems like a few of the kings are stereotypical when it comes to muscle or being out of shape, but other non king men that are mentioned are feminine coded with a usually slender build and long hair.
