It includes the bonus level from the Master System version, includes autofire without needing a turbo controller or messing around with autofire settings (which are a pain to get working in retroarch), and depending on what platform you're using, genesis emulation is typically better and more accurate than mame.
The MD version is looking great... but I'm not fond of their rendition of the music. I much prefer the arcades music and don't know why they couldn't have just tried to copy that as close as possible (the MD is capable of generating music pretty close to it), instead, they went with their own interpretation and slightly altered iteration.
