David Gilmour wrote the music and Roger Waters heard it and put his own lyrics to it from two occurrences in his life. When he was very young he had a very high fever like 105 fever and was so delirious he remembers speaking to people but it was like nobody was listening to him. Then in 1977 at a concert in Philadelphia he apparently had hepatitis and he had to go on stage. So a doctor gave him a painkiller/ muscle relaxer to take the pain away and he could barely move his arms and had to go play. While on stage he said it was the longest two and a half hours of his life and he thought he was playing terribly but people were cheering and having a great time anyway so he became "Comfortably Numb" and made it through the concert. From that occurrence and the one when he was young he wrote the lyrics for this song. So it's not about taking drugs to get high or to get through the day, it's about the aforementioned two occurrences in his life!!

You're right, of course, but you know very well that Pink Floyd's lyrics are almost always ambiguous. I think it might as well be about drugs. Given Syd Barrett's and Rick Wright's unfortunate drug experiences...
"Comfortably Numb" is from the double album "The Wall". It's the story of the fictional character Pink Floyd, who wants to grow up and become a famous rock star. Over the course of the two records, he achieves this success, but in order to do it, he walls himself off from the people who cared about him (hence the title "The Wall"). As he realizes what his success has cost him, he becomes despondent, and does indeed start using drugs to allow him to escape his lonely, miserable life, and become 'comfortably numb'. "Comfortably Numb" represents a pivotal turning point in Pink's story.
At the beginning of the song, Pink is too drugged out to perform. His manager comes knocking on his dressing room door, telling him it's "Time to go!" but Pink is too out of it to respond. Caring only that Pink go out and play the night's concert so the manager can make his cut, the manager calls in a doctor, who gives Pink a shot to "keep you going through the show", and sends him out on-stage. The shot doesn't take effect right away, and Pink's first solo reflects this. It's brief, beautiful, pure musical bliss. But by the time of the second solo, the shot has sobered him up, and he has to face the life he hates. So the second solo is filled with pain, anguish, and self-loathing, and David Gilmour's magnificent guitar playing reflects this.
Previous commenters have pointed out the two incidents that are the sources of Waters' lyrics. But while the events did happen, they aren't what the song is about.
Finally, two items of note. For most of their run, Pink Floyd didn't write "singles". Instead, they crafted concept albums, where each composition flows into the next, with breaks only so listeners could flip the album over. This is why many Floyd songs seem to begin and/or end abruptly; they are supposed to be listened to as an entire album.
Secondly, if you enjoyed listening and reacting to this studio version, you REALLY need to listen to the Pulse Concert version. Pink Floyd's last tour was to support their last studio album, the Division Bell. Roger Waters had left the band almost ten years previous, but while the rest of the group was still intact, by 1994 they were growing old. So for this last tour, as they say, Floyd blew out all the stops. The created a MASSIVE stage (180 feet across, 130-foot arch, 30+ trucks, 700 tons of steel, a truly DAZZLING light show, 3 separate stages leap-frogging each other around the world, three and half days to set the stage up, another two to break it down) and put on an INCREDIBLE concert. They videotaped the shows (in old 3 x 4 video format) and released it, first as a CD and then as a DVD. The name "Pulse Concert" comes from the gimmicky red LED light in the spine of the CD, which 'pulsed'. The concert was AMAZING - you can literally pick any song from the concert, and it will be fantastic. But the song that brought down the house was "Comfortably Numb". It's been called arguably one of the greatest concert performances ever recorded, and David Gilmour's second solo is just.....well, you really have to experience it. Please consider reacting to it.

I believe I’ve heard it is about the distance between our minds and the perception of reality because of artificial relief. He had an interview back in the 80’s about a doctor giving him a shot for pain and he couldn’t feel his arms during the whole two hour show. He said that a lack of communication is a form of pain. We don’t need to be just seen, to be perceived, we need to also be heard and actually listened to.

When he says, "it's just a little pin prick" he's talking about the needle poking through your skin. He's talking about shooting heroin. Comfortably numb is a good description of how it makes you feel. He then says, "there ain't more AHHHH, but you may feel a little sick". He's basically saying all the pain will go away, but you may feel a little sick because heroin can make very nauseous, especially if you're not used to it.
I just read some of the other comments and learned a lot, so I had to come back and add this. I'm sure the other commenters are correct, but I think people interpret lyrics based on what they've been through or are going through. That's one of the greatest things about songs, they can often be interpreted many different ways. So, without any of the knowledge that I just learned shooting heroin is always what I thought it was about.

I recommend checking out Pink Floyd's live concert performance of 'Sorrow' from the Pulse concert, 1994. 'Sorrow' is an intense, amazing experience, especially live.
Do you know where the idea behind the album The Wall came from?
Montreal June 6, 1977, last stop of the tour (In The Flesh), in front of an audience of more than 80,000 spectators, partially intoxicated by more than four hours of waiting. The crowd is noisy, which greatly exasperates Roger Waters (bass player and lyricist), to the point of inviting a young spectator to approach the stage, in order to spit in his face. Subsequently, Roger regretted this unsightly gesture, but the idea of separating the band from the crowd by a wall came to him. In the 1980s and 1981, Pink Floyd presented their The Wall tour. The scenic arrangement of the tour is particularly impressive, notably with the live construction of a 12-meter-high cardboard brick wall separating the band from the spectators during the concert.
Hiding behind a wall never solves problems, it only further separates the sufferer.
In a radio interview around 1980 with Jim Ladd from KLOS in Los Angeles, Roger Waters said part of the song is about the time he got hepatitis but didn't know it. Pink Floyd had to do a show that night in Philadelphia, and the doctor Roger saw gave him a sedative to help the pain, thinking it was a stomach disorder. At the show, Roger's hands were numb "like two toy balloons." He was unable to focus, but also realized the fans didn't care because they were so busy screaming, hence "comfortably" numb. He said most of The Wall is about alienation between the audience and band. Peace out.

Afaik, you were right about the protagonist in the song having a lot of struggles with drugs and two of the voices in the song are his agent and the doctors voice speaking to him.
But to me it really resonates with feelings of mental dissociation, just checking out completely and withdrawing in. Nothing speaks to you, nothing sparks connections anymore and you just hide behind that mental wall.
One of my favorite things about your videos is that I hear things in songs I have never noticed cause I listen too them so much closer during them. And this song is 100% talking about heroin.
What I like about Pink Floyd, the ability to distort time and space, they seem to slow down time, while staying in pitch. Also, there is the moment that the songs ends, and you think, wow is this masterpiece done? Let's play it again.
I was obviously very young the first time I heard this song, but even then I understood that he's talking about the way it feels when someone is trying to speak to you, but your lost in the darkness of your own thoughts and can't quite understand them... its a scary place to get stuck.
