For us brits, a "toilet" is a derogatory term for some place that is really nasty, low rent and squalid, so in this case Matt's basically talking about how like most great bands they started out playing in dive bars and underground clubs.
Here one might say sh1t hole which is obviously the same thing. For her not to immediately figure out what he meant is absolutely hilarious.
It means the same in North America as well. Toilet = shithole/dump/run down piece of shit locale. It might be a generational thing that she doesn't get the slang.
"Toilet" is basically a "sh!thole" with extra steps, so yeah, it's some dirty bar or something.

The "I have played in every toilet" line comes from a UK term. The toilet circuit is a nickname for the sort of small dingy pubs and bars that small, unsigned bands play at.

Those vocalizations at the end are him, into a megaphone, into the mic. God, these really take me back. Honestly, top 3 live concerts. I have been to a handful of Muse concerts and they have all been epic. I went to a Rage Against the Machine concert once where Muse had a set before them... I felt bad for Rage because Muse was a step above at Rage's own concert.
Playing in every toilet is british slang for saying you've played in the nastiest places. London, for example, is full of pubs where starting bands can perform to reach new audiences. Those places have mini stage that are pretty much next to the bathroom, hence the toilet name. This song is actually about how they struggled in the beginning.
In addition to the other comments on the toilet line, don't forget that over here we don't use the term 'bathroom', unless it is the actual room with a bath in it, we also don't use 'restroom', if we want a rest we would go take a seat somewhere comfortable.
If we are out at a pub or restaurant (or somebody's house) and need to use the facilities it is just called going to the toilet. This might add confusion to those folk overseas where this direct terminology is not used.
So playing in a toilet just means those little unknown venues that often feel like the size of a public toilet room. The toilet tour of the UK ironically has projected some of the biggest acts to stardom & is almost a right of passage.
