We used to do something similar, and all us kids did it. You would use the thumb and index finger, and move them up and down, as though it were a mouth. But you also move the fingertips close to somebody's face without touching them. You wouldn't believe how irritating that was, mostly just because it was so stupid.

They were confused and he went on set with them on his fingers, and did this scene in one go and tried to save it, and he did flawlessly.. They were holding back laughter on set but tried to keep serious. Walter thought this scene was genius and kept it in. David thought of the idea because when he was a kid, he had Bullies who would knock on his window and yell "DAVID, COME OUT TO PLAY-YAY!"

During a break in filming at Coney Island in New York David Patrick Kelly states as he walked along the beach he saw all these mini beer bottles strewn on the beach and he came up with the now iconic scene. Also of note actor David Patrick Kelly also plays in the movie John Wick 1 albeit with less hair as the head of the clean up crew who come to John Wicks house to professionally clean up the bloody mess and bodies of guys John Wick kills.
