Question 3: If the requirements are not there, we may issue a change request to add it, and analyze its impact on project objectives. But the question stated that the requirements were overlooked, which means the requirements were there but ignored. Why I need to issue a change request for something that didn’t change ?! Note that the question didn’t mentioned whether there is defected deliverables already built or not. ------- Even if we need a change request, why we don’t log the stakeholders concern as an issue, then analyze the root cause of the issue then issue a change request with corrective action for approval, then provide the approved change request reference number or result summary as a decision to close the issue?!
Thanks Aileen.. the 3rd example is one of those that can be confusing, as you could argue that both A and C are correct. Ultimately you are right that the change request will be needed to implement the requirement, but it very likely needs to be logged in as an issue as well, specially that issue logs carry on to lessons learned. So both answers are correct, but one is more important than the other.

Hi Aileen, I have come across similar question no. 2 in one of PMI free exam. I've chosen "Update Lessons learned register" instead of "Update Issue log" since issue says "already resolved" and it says "issues may occur in other similar project so i thought why not use the lessons learned" but I got wrong mark. The PMI free exam explanation are not clearly defined/explained. So I have read it carefully, and thought of I should capture first all the action i made to resolve the issue and make sure it is recorded in Issue log to make sure that all issues are investigated and resolved, before i can use it in other similar project through "Lessons learned register" so "Update Issue log" always comes first before "Update Lessons learned"
Hi Aileen, for Question 1, I picked C initially, because the question said the project may no longer be viable, its still uncertain yet, that's why I think first, to update the issue log, and secondly to discuss with the project stakeholders to see if there are any solutions. May I know your thought on my logic?
I also think answer C should be the one as without analyzing and knowing for sure the project is not viable, it is probably an over reaction to inform all the project stakeholders

Because the remarks from stakeholder need further review and evaluation by the Project team, and based on that decide if requirements were missed while preparing requirement documentation or is it missed while execution by project team? Please shed some light on this.
I think "issue a change request" is the best option from the options provided. If we had another option, "analyze the impact of overlooked requirements", then we could have chosen that option because we always analyze the impact before issuing a CR. But this option hasn't been given so we choose the closest/ best option available i.e. "issue a CR". Hope this clarified your doubt.
I thought this as well. The stakeholder only remarked that requirements have been overlooked. We aren't sure the requirements were really overlooked yet to issue a change request. Plus according to my studies, usually the best answer for the PMP exam when a stakeholder makes a comment is to document it or seek clarification/evaluation unless additional information is provided.
When you issue a change request, what you are doing is providing a notification to the stakeholders that you want to make changes to the BASELINE products in this case, the requirements document as determined by the Collect Requirements process group. Because the change request will be evaluated through the integrated change control process, the stakeholders will also evaluate the impact at this point in time. And depending on the impact the change will be either approved or disapproved.
