I'm guessing either COPD from years of smoking or lung cancer; it couldn't have been TB 'cause if that disease took a man like Arthur in less than a year then an aging man like Hosea couldn't possibly made it past the extreme conditions back in the Colter chapter.
@@Amarganeitor We have to consider that Arthur could survived several years with TB but due to crashing in Guarma, not moving somewhere warm as was recommended to him, and get beaten near death by Micah was what accelerated the TB to fatal stages.

The gang wasn't just recruiting members to assist in robberies or whatever, they'd also save them and give them shelter and family, they were out in the Grizzles before RDR2 and that's why there found Charles as well Lenny and Jenny.
He was a veteran survivalist before he fell in with Dutch’s gang , he talks about it in chapter 1 early on when he teaches Arthur to shoot a bow , he says that Dutch is different and that most of the gang members treat him fair which isn’t normally the case with an Indian father and Black woman’s son.
