  • Many patients wonder what to expect on the day of Cataract Surgery. There's normally some anxiety and uncertainty. Perhaps your doctor didn't go over the details and you have many questions. In this video, please join me as I help a wonderful patient with their Cataract. We will implant the Light Adjustable Lens (LAL) as he wanted the best quality far vision.     What is a Cataract?
    Within a healthy eye is a clear lens. As we become older, that lens becomes cloudy. The cloudy lens is called a Cataract. In cataract surgery, the cloudy lens is removed and replaced with a clear artificial lens implant.

    Phenomenon, may I ask which camera you used to shoot the video?

      The surgery was recorded with a Sony Alpha 7C camera. The third-person view shots were recorded with the DJI Pocket 3 with auto-tracking.

    What were the marks in the eye. I expected completely uniform color after removing cataract. Looks like some remnants left is ok? Can you remove floaters while cleaning?

    Excellent video! I was wondering what kind of setting do you have to record your surgeries? Is it a dslr camera? What kind is it? And what settings do you use to record cataract surgery? Thank you! Great video once again!

      Air bubbles. They go away / absorb within an hour or two

    Doctor, a couple questions from a fellow doctor who had corneal edema (now resolved) after the first cataract surgery and is losing sleep over going through the whole thing again in the near future:
    - do you routinely do a pre-op corneal evaluation (AS-OCT and stuff, not just a slit lamp exam) for every patient?
    - at what point during the procedure does a vigilant surgeon look out for a PC rent and what are the cardinal warning signs? Can he be 100% certain that he hasn't missed one before he implants the IOL?

      Hey Doc! There are many factors that increase the risk of corneal edema, including increased cataract density, shallow anterior chamber (less space to work in), weak corneal endothelium (Fuch's Dystrophy), imperfect surgical technique (cataract pieces hitting the cornea or performing phacoemulsification too far anteriorly), a tear in descemet's membrane near the incision, high eye pressure, retained lens fragment in anterior chamber angle, insufficient viscoelastic use, excessive use of ultrasound energy, etc. Excellent surgical technique can minimize corneal edema, but it can still happen. A good slit lamp examination can expose factors that increase the risk, and anterior segment/corneal OCT is not performed routinely prior to Cataract Surgery. Corneal OCT could be performed if a corneal abnormality was noted on slit lamp exam, and the etiology or view was questionable. Corneal topography & Retinal OCT are more commonly done prior to cataract surgery than Corneal OCT, but also not necessary for everyone. As for a posterior capsule tears during surgery, a vigilant surgeon would be monitoring the capsule the entire time during surgery and detect it almost immediately. Direct visualization and paying attention to how cataract fragments float around the anterior chamber (they get trapped in vitreous) are of utmost importance. There is a lot going on during the surgery and occasionally diagnosing a capsule tear is delayed for a few seconds or even a minute, which can be problematic. The surgeon should be certain the capsule is intact prior to implanting a lens, and this is USUALLY the case. One is typically 100% sure, but surgeons are human and can make mistakes. I hope that helps!
