Honestly, I think they capitalized on leaving that unsaid, as given his current emotional state one can also interpret that as a solitary moment where Homelander understands that the people closest to him, fear him, and it's causing him to lose their support. I actually really love that line mixed with his mournful facial expressions. He doesnt imply that "'course I would kill them" with anger and hatred, he implies it with actual remorse and understanding, that's a powerful bit of direction. Homelander doesnt have a lot of moments where he clues into his own failings, but I think that's one of them, and the best in the season so far. :)

I think he didn’t kill Sage because deep down he knows that she’s right. In his own words “I’m smart enough to listen” - so when she said it’s all apart of her plan. There’s dramatic irony present. We know her master plan (or at least a part of it). But she kept it hidden from homelander and that’s pissed him off in combination with A-train’s betrayal.
