To make the morphology simpler :
Parenchymal caseous necrosis = Ghon focus
Ghon focus + lymphadenitis and lymphangitis = Ghon COMPLEX
Ghon complex + multinucleate giant cells and fibrosis/calcification = RANKE complex which is healed lesion.

Adam is a solder in his late twenties; come from Kassala, he is off work for a long time because of his chronic cough. Two weeks later he started to have weakness in his lower limbs that is increasing in the intensity. He has back pain also. O/E:
He looks ill, cachexic.
He is pale.
Chest: signs of chronic infection.
Back examination: there is vertebral swelling, which is tender but not hot.
My Question : why the swelling is not hot although there is infamation
Probably a cold abscess (because he’s immunocompromised).

I have a question : Why there is no hilar lymph node involvement in secondary TB?Example : young adult, positive for immunodeficiency virus who had tb scrofulosis, that means he had primary tb ? i understand the immune system can better locate the bacilli in secondary tb, but then how can be scrofulosis explained ?(maybe a very immunosupressed state? ) thanks
