Definitely tinn eye tus
I know I had always pronounced it as "tinn-eye-tus" probably (I presume) because that was how it was pronounced the first time I ever heard the word (and, yes, I am from the US). When I first learned of the debate on pronunciation, I reasoned that all other similarly named afflictions (i.e., arthritis - bronchitis - cystitis - poliomyelitis - colitis - hepatitis - appendicitis - meningitis - laryngitis) were all pronounced as "...eye-tus".
However, when I searched further, I realized my error. Those are all disease due to inflammation, thus the "-itis" spelling. Whereas, tinnitus has the slightly different spelling because it is not from inflammation, but from a neurological condition. Then, the whole debate made sense.
Out of habit, I usually still pronounce it as "tinn-eye-tus". But if I think about it, I try to pronounce it the medically preferred way, even though both are now acceptable.

Funny. This has been a topic my whole 30 year career in Audiology. According to the American Tinnitus Association, it is not so much geography that determines pronunciation but role. ATA says: Tinnitus has two different pronunciations, both of which are correct and interchangeable: ti-NIGH-us is typically used by patients and laypeople while TINN-a-tus is typically used by clinicians and researchers. But I also prefer the latter!

Tinn-eye-tis. Been told by different audiologists / doctors I was pronouncing it incorrectly. USA. FWIW Merriam Webster has both prononciations.

Tinnitus is a medical condition in which a ringing is heard, but there is no obvious cause (may be hair damage in the cochlea, though, from loud sounds - one of the causes but far from the only one). An -itis - is used to denote an infection, eg. "rhinitis", Tinnitus IS NOT an inflammation and should, therefore, be pronounced "tin-i-tus" and not "tin-ite-is".
