But yeah needle chest decompression is a ridiculous thing to carry civilian side unless you're an *on duty* paramedic. I'm trained to perform NCDs and surgical crics and a variety of other procedures, but I'm absolutely not doing anything that invasive if I'm in civilian clothes and operating under Good Samaritan laws alone.

Some dude was having chest depression and was having trouble breathing (due to just nornal health issues, not because his throat was closed) and the kid put a traech in after cutting his throat. Not only did he put it in wrong, but he gave the poor dude permanent damage to his throat tissue.

If you carry a gun you should also carry basic emergency medical supplies. Just my opinion. If guns come out, people are likely getting shot, and you can’t predict how, when or where something might go down. You need to be ready to take responsibility for what you are prepared to do with your gun, as well as for the aftermath of whatever situation caused you to use it. Seems like an obvious necessity to me. You carry a gun to be responsible for your safety (and the safety of those around you if you so choose) injuries are intrinsically part of that and also simply more likely to be encountered in life regardless of if a gunfight occurs. Its not sexy but its part of taking responsibility for yourself.

Everyone has their opinion and I would never discourage someone from having these conversations but things have changed. Blood lost from extremity wounds and amputations is the most common killer for the battlefield. The concept that a TQ is last resort is not true. In fact, in most cases it’s the first thing you do. Combat medicine has exponentially grown in the past couple years. While a TQ is not a permanent fix, there is usually follow on medical from doctors and surgeons who know hot address the situation. The TQ just keeps you alive.

Given body armor should be covering most the chest your main areas of concern are your arms and legs. If it squirts and is below the shoulder throw a tourniquet on. If it oozes or is above the shoulder stuff with hemo and then throw on a dressing. If it’s oozing and doesn’t need stuffed or is long more than deep then use a pressure dressing. The tourniquet is good for stopping any kind of extremity bleeding even if it’s a little or a lot so it’s reliable and light. All gunfights and gear for them is playing a giant statistics game. Statistically tourniquets are effective for the most common fatal injuries in combat so why not have one or two when all that other stuff is so small it’s easy to carry anywhere.
