Thank you Ben. I'm an 83year old female and started HRT at 50 as I was having severe panic attacks as well as hot flashes which interfered with my work and life generally. Over the years I've several times stopped at the instance of Drs but have felt so bad life was unbearable. I still use a bio identical cream and will continue to do so. That, in conjunction with a low carb woe and active lifestyle has kept me slim and positve as I age.
Thank you! You are an inspiration. I am in my mid 50’s and started HRT a few years ago. I am low carb like you and love to stay active like you with running, plyometrics and heavy lifting. Still lean, strong bones and want to stay this way for as long as possible.
Excellent discussion, Dr. Bikman, thank you. However, testosterone is another very important hormone for women. Most people are surprised to learn that women actually have more testosterone than estrogen. The ovary converts some of that testosterone to estradiol. Testosterone, even in female doses - about 1/10th of a male dose - helps with mood, libido, and energy much much more than estrogen. Also protects against breast cancer. As a physician I offer bioidentical testosterone, estrogen, and progesterone (not progestins) to all my female patients.
You have to have testosterone to convert to estradiol in women. Yes, testosterone is very important in women. As a family practice professional, I know about this and test it in my women.
I went through menopause rather early, in my very early 40’s. I used to take pregnenolone and DHEA for many years. Since addressing my adrenals specifically and going carnivore recently, I no longer take either.
I’m on HRT since 7 yrs now, I did go off of it for 3 months this yr, what a mistake. My hot flashes and night sweats returned, sleep was awful and my vertigo came back. Had a vertigo attack at Easter. Learnt my lesson back on and loving life again. Great video.
this. I'm 45 and had joint pain and it has all gone away. The itchy ears is going away and I'm finally losing weight just with my same exact, healthy lifestyle. I pray it is always available to me, and that I can always afford it. I'll take it until the day I die.
I’m in my 60s, got breast cancer E/Pr+, they put me on anastrazole, an aromatase inhibitor (steals your estrogen since this type of cancer feeds off estrogen). He is correct about the diabetic trend after losing estrogen. Even though I changed my diet, lost weight and exercised regularly, the medication to keep cancer at bay was causing me to become diabetic. I had to request a CGM and dial my diet and exercise timing in even more to be able to stay on the life saving pill and clear up my pre-diabetes. It is a narrow road I have to walk now. ❤ Thank you for validating what I suspected about how the loss of estrogen was actually responsible for my A1C of 6.1. I was eating right, exercising, etc and my doc said the pill was not responsible. I have science background as well and have researched it now and yes….. it Can cause diabetes if low in estrogen❤.
You can take natural testosterone (female dose is about 1/10th of male dose) and it will greatly restore your energy, mood, and libido AND testosterone has been shown to decrease volume of breast cancer masses.

Any time I let myself have a lot of meat, my blood sugars spike. I have emotional episodes too. It’s frustrating to know I have to go through this for at least 2 more years. I want to quit these medications but know the cancer is worse than the horrible way I feel.
check with a well-versed doctor on this subject. Dr. Heather Hirsch has an online practice. I have heard of women who have previously been diagnosed with breast cancer being on HRT and risk of recurrence was not that much higher. I am in perimenopause and started HRT 3 months ago and it has cleared up all my joint pain and my clean diet and active lifestyle is finally paying off. Changing nothing about how I eat and move my body, I have lost 12 pounds in those 3 months. I was low carb. But now that my body is finally working for me, I decided to go keto just this last week. I lost two pounds just in that week. I had been keto for years, working out even more than I am now, and not a single pound would shed. But now that my body is working with me on HRT, I'm back to attempting to shed pounds again. We shall see.
Nobody ever addresses the SUDDEN AND CATASTROPHIC effects of a pan-hystero-salpingo- oopherctony on a women's body. When done at a young age, I was 31, you are thrown into instant menopause and everything changes instantly. Oral HRT does little to nothing to ameliorate the cascade of metabolic changes. If I had been better educated in metabolic medicine in med school, I would have been better prepared to correct and prevent the insulin resistance, weight gain, muscle mass loss. Now at 73, carnivore for over 2 years I'm finally metabolically healthy again with a fasting insulin of 2. I'm normal weight, strong with increased muscle mass and a normal DEXA scan. So I hope women are listening to your channel and they can prevent long term destruction from life saving surgery that will also degrade their health overall if they don't do things to fully recover. Thank you for all this updated education.
Perfect timing! I’m 62 and recently started bhrt. Sleep-right out the gate improved. With testosterone I have more energy and focus. Why oh why were we fearful of this? My doc never offered 10 yrs ago when I needed it most 😢. I went to holistic to get figured out. However bc I’m type 1 diabetic I’ve not had increased insulin sensitivity. In fact I’m taking a bit more🤨
I have been studying HRT for the past year and half. It is one of the best explanations I have heard. The high protein section taught me something today. Great job. I will save this and past it with my dr.
I am 70, no menopause symptoms ever. I have been eating carnivore for 3 years. I weigh the same as I did in high school and have no health issues. I also lift very heavy weights several times per week. HRT does not seem to be necessary for me.
@Thecatladybooknook_PennyD No aches and pains. No arthritis. Sleep is good. Skin is good. I should get bone density checked so I can not speak to that. I do lift heavy weights several times a week. For example, I squat at least half my body weight and steadily increasing it. I also do deadlifts, standing press and chest press. I take no medications. Never run to bathroom since doing carnivore. No incontinence.
I would like to see a much more detailed discussion of progesterone and testosterone since there are progesterone and testosterone receptors all over the body in women and men.
I am 45 and just started estrogen patch and oral progesterone 10 days ago. My sleep is improved as is my energy im back getting so much more done. My menstrual cramps this month were 80% improved. So far loving it.
I have found that optimal estradiol levels without sufficient progesterone can substantially increase histamine production which for me raised fasting blood glucose into the 120's in spite of a fasting insulin level of 3. Oral progesterone often does not provide sufficient progesterone because of so much being converted into allopregnenalone.
Great basics on the topic. For women in menopause and peri-menopause who should consider HRT they may want to read the wikipedia articles on pharmacokinetics of estradiol and progesterone and testosterone. Women typically need all three in menopause.

Can we have more information on the insulin resistance inducing effects of progesterone- as evidenced by getting fat and nearly diabetic when pregnant? Now needing to take double dose as part of my HRT to protect the endometrium I am fighting a losing battle against weight gain and hunger. Previously low carb keto would easily shift weight/fat but not anymore and the mental struggle to not overeat is huge.
Maybe you are on too much progesterone? If you are menopausal testing may be a good idea. I only take 100 mg of progesterone for .075 mg patch of estradiol. Perhaps your doctor needs to lower it? I'm losing weight on both P1 and E2.
@ i know I’m on too much progesterone for what I’d like but nothing else stops the bleeding so I don’t have a choice - I’m never having a mirena again as I thought it was perforating and I was dying with the pain for 3 days last time!
@@claudiawilkinson8769 I’m so sorry. That is awful. I get that. I’ve had many friends say the same. And the synthetic hormones have way more side effects. I hope you find a solution. Maybe an ablation? Talk to your doctor about that. I have had friends go that route. I haven’t dealt with the excessive bleeding- just everything else, it seems. All the best to you.
You may want to switch delivery methods if you are on oral progesterone because much of it converts to allopregnenalone in the liver. Prometrium capsules can be used as suppository and will result in far higher blood levels without the sedating effect of allopregnenalone.
Kindly elaborate on the term: later in life. How many years after using n then stopping HRTs, or do u mean when they are still on HRTs for the last edge of their life?!
I'm 72. Went through hormone positive breast cancer, taking Arimidex, in 2004. Before that, at 54, my monthly cycle was STILL every 28 days. Chemopause ended that for good. Only had mild hot flashes and moderate weight changes. I've upped my muscle work and protein and am at a healthy weight. I am interested in whether HRT would help my hair AND not increase any risk of recurrence. I now have male pattern baldness- very thin and receding in front and top.
I'm no expert, but have (like many) had to educate myself with my own research and due diligence. I know the thinning hair component can be a thyroid element. Thyroid may be worth looking into.
@@giselleturner5687 Oh right! I had a friend back when we were in our 40s whose great hair got so thin and straggly which turned out to be a thyroid problem. She did get it straightened out and hair came back along with big drop in weight. My last check was 2021, normal so I don't think that's it, but adding to next physical just to be sure. Thanks.
I’m 64, 15 years into menopause, never had HRT, and have been a Vegetatian for more than 50 years. The only symptom I’ve ever experienced was low sex drive, which doesn’t bother me. Could I start HRT now to avoid hormonal diseases and for longevity?
54. Still perimenopausal. I don't have many uncomfortable symptoms, however, my mom had vascular dementia/alzeimers. It's a catch 22, as with vascular dementia, arteries are obviously more of an issue, clotting. But with Alzheimers you can benefit more from HRT.
Dr Bikman I heard an RD say on a lecture about vegan keto diet, that the ovaries are the most metabolically active tissues in women (and They are also more metabolically active than any tissues in a man). Can you comment on this? It might explain why women gain weight after menopause Despite not a changing diet.
Please explain your understanding of how phytoestrogens affect cells compared to estrogen. Does phytoestrogens behave the same was as estrogens? Would phytoestrogens still feed/support cancer cells that are estrogen positive?
60. Keto-ish for a year. No menopause things that bother me. Lost my mom to Alzheimers so I am working hard to prevent that in me. Had never considered HRT. Now, I have a lot of questions. Is it too late? Would it help me? Doctors tend to be useless. Down the research rabbit hole I go...
Thank you for this. I hit abrupt medical menopause as my ovaries were removed due to breast cancer. I’ve been suffering ever since but am not supposed to take HRT. Any suggestions on non hormonal interventions I can use for similar benefits/protection?
Do not drink alcohol and do not eat ANY SUGAR or sugar forming foods, and fruits at all costs. Being metabolically healthy will improve your symptoms dramatically. Limit total carbs to 20 grams or less every day.
I am 80 - and have never had a normal production of estrogen in my life. I had very few , maybe 5 or 6 periods as a teen and had a complete hysterectomy at age 23. HRT was never discussed with me and I remained totally uneducated until many years later. I have had many health problems throughout my life ranging from bone degeneration at age35, a brain tumour located.near my pituitary gland which was golf ball sized removed at age 56 and many other issues. Is it too late in my life to use HRT to help me now with my very extreme conditions of today. Extreme chronic inflammation- kidney disease, weight problems, it’s all so frustrating. Help
That was very informative. Thank you. Is it too late to start this treatment when you started menopause at 45 and are now 66, but still experiencing flashes? I was experiencing this at the peak of the HRT fear of cancer. I am experiencing many of the symptoms you mentioned, but did not know about the estradiol connection. Is it too late for me?
I'm 63 and have been dealing with severe hot flashes for the past 6 months. Not one of my doctor's will put me on HRT no matter how much I beg. I'm a T2 diabetic for the past 10 years. I've lost nearly 70 pounds due to Trulicity. As I've lost weight I've become very cold except when I'm having a hot flash.
That is unbelievably cruel! You need a hormone specialist, preferably female. My hormone doctor is 80 years old and obviously practices what she preaches. She’s amazing!
My doctor stopped prescribing it for me after I was on it 10 years.. I wish I had insisted or went to another doctor. Partial hysterectomy @ 25, now I'm in my 50s with all kinds of issues
do yourself a favor and go with Midi instead. Winona doesn't rx patches and those are more effective than their biest cream. Estradiol patches are also cheaper. Compare your insurance prices and good rx. I get it cheaper through good rx and can pick up at cvs.
The problem is finding a doctor that doesn’t immediately dismiss you when you discuss HRT. I am 49 and do not have heat flashes but approached my doc to discuss HRT for the metabolic/bone benefits, and was immediately dismissed “I don’t prescribe HRT prophylactically.” When your insurance dictates who your doctor is, and your doctor unilaterally dismisses the possibility of HRT, then what…
I'm 56, made a decision at 48 to start hrt for perimeno symptoms. At that time I had insurance that paid most of naturopath visits. Now I pay private about $200 twice a year and pay for estradiol and testosterone from compound pharmacy. I've significantly reduced my trt since going carnivore because my levels raised buly 140 points. I changed my health plan to the HSA where I can put in pre tax dollars to pay for health care. For me, I just made it a priority. But I get it can be difficult to pay for. I get my progesterone at a pharmacy so insurance pays most. My friend opted for the estradiol patch which can be paid by insurance at a pharmacy and it is the same as the cream, you just can't customize the amounts needed.
Very informative, but the doctors available to me (even the one I like) don't seem to be versant in this area. I am very discouraged with the quality of healthcare available to me.
Mine seems to have/ esp on tops of hands snd arms to elbows. Oddly, i started4thousand iu of vit D and 150mcg of K2 at same time so am not certain if its the combo thats helping
Stryggling with hyperplasia in uterus since leaving hrt behind. Have gained huge weight and cant move the needle with diet and exercise. Any siggestions?
Despite over blown fears of clots and cancer, if someone still doesn’t want to go for HRT, can a healthy keto- high fat low carb diet, some walk and upper limbs dumbbells a day save her day?
I am also interested in knowing this. I have chosen to stop taking HRT after 7 years of use. I am 60, insulin sensitive and lean. By eating very low carb (keto)- low carb, keeping insulin & glucose under control...can that prevent the fat storage change and keep the brain healthy. Should I check the genetic biomarker for Alzheimer's to make sure I have made the correct decision?
The biggest mistake I made was stopping bhrt for a year and gained 30 lbs in that time. Since starting it again, I've lost 20lbs in two months without even trying. I'm hoping that my lipid profile improves too. I'm high-fat ketovore, and my diet hasn't changed, I'm 68@moniqueshenefield6698
That’s exactly what I’d like to know. I’m 49 and I know menopause is coming. But I’m not keen on the idea of HRT. Then, I’m concerned though that if I don’t take HRT, I could put myself at risk. So I’d also like to know if diet and lifestyle can make up for, or offset any risk?
Very good questions. I am searching. I want to believe that eating a proper human diet (Dr. Berry or following Ben's book How Not To Get Sick) supports hormonal health and processes that lead to disease. Many cultures do not supplement with hormones for menopause (for example the Taiwanese), so are the women in Taiwan more prone to develop dementia? I lived there 4 years and watched women over 90 doing tai chi in the park... This is behind my interest in going off hormone replacement. I will look for studies in Asia on this topic. @@deafish4577
@ what sleep supplements did you use? I’ve only been getting 5/12 hours sleep now for a couple of weeks, and I’m so worn out. I feel like I’ve tried just about everything. I did just find out that my sodium blood level is too low.
I am 56 and T1 for over years, now post menopausal. High fat keto/carnivore helped some symptoms but weight loss stopped and insulin resistance remains. I’ve been doing so much research and now have an appointment with my GYN and I’m going to start HRT.
I have to laugh when you say hot flashes are "uncomfortable". Spoken like a man. They are HELL, Painful, make you anxious, short tempered and make you want to crawl into the freezer! 😮
While metabolic health certainly needs addressing, there is a need for estrogen cream for skin health which gets greatly neglected. Also doctors completely ignore health in that area!!!!
Please do not use Premarin, which is an abbreviation for pregnant mare's urine. The production of this product is extremely abusive to horses. The mares have to be pregnant to produce the hormone replacement. Their foals are byproducts and get set to auctions where the majority of foals are sent to be slaughtered for the meat. The mares are forced to stand in stalls so narrow that the horse can't even turn around. They are kept locked up in this manner for 11 months, the length of their pregnancy. They aren't allowed out.The mares wear an uncomfortable contraption around their hind ends and back legs in order to collect their urine.Next their foals are born and the whole cruel process starts again.
I am extremely disappointed that this is so biased for HRT and the information found in the nurses health study which followed a massive amount of nurses for Many years and found that all the benefits that were being touted did not hold out in real life, but rather resulted in a significant increase in breast cancer. You really downplay the real life findings. Personally, I trust that God knows what He is doing.
Bio-identical all the way, because of it's excellent safety profile. Conjugated mares' urine based hrt, isn't bio-identical, and just look on youtube for the appalling, cruel conditions the mares are kept in. Listen to any of the excellent Dr Louise Newson/Newson Health vids on youtube to get a much clearer knowledge of all aspects of peri/meno/hrt.
Post menopausal gluteal tendinopathy is common and horrific. Why does nonone speak about this. About 30% of postmenopausal women get this. It is debilitating.
Thank you! I understand the concept and appreciate the medical benefits, but I believe that God, in His infinite wisdom, designed us the way He did for a reason. I don’t want to mess with that, but I understand and respect my sisters who choose HRT.
I hear you but I also remember we live in a fallen world. I do BHRT and see it as a gift from God. It improves my life dramatically. Helps me live and not just be alive. Without BHRT I was extremely sleep deprived from chronic insomnia. So I see it as a blessing 😊

Well balanced discussion on hormone replacement therapy Kind Regards. John Stewart
It's not. There are many many nuances to this.
Thank your for an interesting, relevant and very clear lecture
If it isn't broke, I don't fix it. I am 70 and have never had a menopause symtpom in my life. I eat a carnivore diet.
@@mamabear9389good for you, n=1!
Thank you so much. There is still a lot of raided eye brows when I tell doctors I'm on cream HRT.
This applies to perimenopausal women too. That's anyone between 35ish to menopause.
Help

You are preaching to the choir sir. I can't eat anything since menopause.
are you on intravenous feeding then - I hadn't realised that was an option.
No it's not too late. I'm in the uk. I've done lots of research on this as I've bern on HRT for almost 10 years, I'm 55. Goodluck 😊
No. Not too late. Read the books “the new menopause “ and the latest edition of “estrogen matters”
Very good!
She’s amazing!
check out Midi health online. Another clinic is called Defy. Midi takes some insurance. Defy is self pay.
@@donnahelps5680where is she located?
My doctor stopped prescribing it for me after I was on it 10 years.. I wish I had insisted or went to another doctor. Partial hysterectomy @ 25, now I'm in my 50s with all kinds of issues
Very interesting
There has been so much misinformation around heart disease risk and menopause, what is your view on that? I would love to see something on that
This is timely! I have been looking into Winona and I just haven’t pulled the trigger yet
do yourself a favor and go with Midi instead. Winona doesn't rx patches and those are more effective than their biest cream. Estradiol patches are also cheaper. Compare your insurance prices and good rx. I get it cheaper through good rx and can pick up at cvs.
Danke !!
What is the average age science recommend when we have to suspend HRT? Thank you for your wonderful lectures ❤
Never 74 yrs old started hrt at 50. I would fight you for it
Went through menopause 15 years ago. I didn't have a single symptom.
Benjamin Bikman Please set the translation to Polish. We in Poland also want to listen to you. On RUclips and Instagram
About to listen! Please of please talk about testosterone. *crosses fingers
Why didn't you mention testosterone replacement therapy in women aswell? Doesn't that also drop dramatically in menopause?
Not dramatically ) but depends on personality
Does exogenous estradiol prevent the brain from building new estrogen receptors?
please talk about trt for men.
I wish my doctors would prescribe HRT. How many symptoms do I need to get it? What do I have to say to get it??!! I am experiencing MANY symptoms.
Can a women start hrt post menopausal? In case missed the pre menopausal?
yes. It's just most protective in the first ten years after menopause onset, or before that.
@@MelissaC269 thanks 🙏
I started at age 72. My gluteal tendinopathy ws debilitating
Can women that are 15 years past the beginning of menopause start HRT?
I want to know this too!!!
I would like to know this as well.
What about using HRT post menopause? Does the skin thicken again?:
Mine seems to have/ esp on tops of hands snd arms to elbows.
Oddly, i started4thousand iu of vit D and 150mcg of K2 at same time so am not certain if its the combo thats helping
Stryggling with hyperplasia in uterus since leaving hrt behind. Have gained huge weight and cant move the needle with diet and exercise. Any siggestions?

what would you recommend to girls after hysterectomy? no one talks about it, and it's clear that it's an epidemic today :(
Despite over blown fears of clots and cancer, if someone still doesn’t want to go for HRT, can a healthy keto- high fat low carb diet, some walk and upper limbs dumbbells a day save her day?
I'd like to know too. Went through menopause early. Switched to keto hot flashes so so much better. But still get them and go on estradiol
How does estradiol effect heart palpitations? And sleep?
My palpitations stopped and after using some sleep supplements for a few months I was able to go off of them and now sleep much better
@ what sleep supplements did you use? I’ve only been getting 5/12 hours sleep now for a couple of weeks, and I’m so worn out. I feel like I’ve tried just about everything. I did just find out that my sodium blood level is too low.
5htp 50 mg by vital nutrients and L-theanine , I think it was 200 mg or less
The low sodium I’m sure is effecting you too. Hope you get some sleep!
How does all of this affect type 1 diabetic. Post menopause. Age67 Ty
I am 56 and T1 for over years, now post menopausal. High fat keto/carnivore helped some symptoms but weight loss stopped and insulin resistance remains. I’ve been doing so much research and now have an appointment with my GYN and I’m going to start HRT.
@@PedsRn all the best to you.
So, if you DIDN'T do HRT, and now you're almost 65, should I still be considered?
I started bhrt at 65. Start very low & slowly increase until you feel good. Game changer, life changing. I will take it forever.
I have to laugh when you say hot flashes are "uncomfortable". Spoken like a man. They are HELL, Painful, make you anxious, short tempered and make you want to crawl into the freezer! 😮
The Family Proclamation on your wall.
What about testosterone?
I’m taking testosterone.25 mg compounded cream. More focus, energy 👍🏻
😊❤
Cream, oral, and don’t forget Shots! Haha!
Post menopausal gluteal tendinopathy is common and horrific. Why does nonone speak about this. About 30% of postmenopausal women get this. It is debilitating.
Hot flashes are awful…
Cold flashes in my case 😂
I’m on HRT but my fat still stores on my belly and not on my hips 🤷♀️🤣….
Gestational Diabetes and PCOS and then menopause boom
High insulin
It's more nuanced than this.
Thank you! I understand the concept and appreciate the medical benefits, but I believe that God, in His infinite wisdom, designed us the way He did for a reason. I don’t want to mess with that, but I understand and respect my sisters who choose HRT.
What a stupid, ignorant comment!
I hear you but I also remember we live in a fallen world. I do BHRT and see it as a gift from God. It improves my life dramatically. Helps me live and not just be alive. Without BHRT I was extremely sleep deprived from chronic insomnia. So I see it as a blessing 😊
