Hi Amy - to do this, you would need to use Power Automate and have a business account for Microsoft 365. Forms has an option under Settings to get an email notification when a response is received, but I don't see any way to re-program that email so it would go to the email address of the main account that you are logged into. That would be a nice enhancement idea! Are you familiar with Power Automate? There are some template flows that you could use to do this if you have access. Thanks for watching!
Hi Michael! I believe you can include a URL link when you customize the Thank You page. Click on the 3-dot menu in the top right of MS Forms next to the Send button and select "Settings." From there check the box next to "Customize thank you message" and try entering your link and any other messaging there. I believe this should work. Thanks for watching!
Been suing Forms for about 2 years. My issue is when I put in the option of a picture to be taken, that when i go to look at the Excel file, it shows a link to the picture but I have to use Smart Lookup in order to get the text as a link
I'm planning to make one for field day volunteer but i want to separate each grade level for example for Kinder & 1st grade volunteers, 2& 3rd the 4& 5th. Is will be the same activities but different time for 2 grade levels. Do i have to make different links for each groups?

I work at a hospital as a phone operator It is the duty of whomever is on night shift to make what we call a pass down sheet. Briefly, the pass down sheet tells the telephone operators which nurses or doctors are on call for a given department. The way we do it now, is to fill in a Word document and we have to enter each nurse's or doctor's name, and in some cases their phone number(s). As you can imagine, this is time consuming. I want to create a new form that will allow the phone operator to choose the given name from a drop down list, and auto populate the phone number. Here's the catch. Some departments will have multiple employees on duty. When it comes to those, I want to create ONE list and repeat it on multiple lines. For example in my department--the phone operators--I have to fill in a day shift person, a night shift person and do this for two operators each. Can I create a master list for each department and just link them into the appropriate parts of the form?
I am new to ms forms, but it's not clear whether or not I can create an application form where I need to collect personal data of each candidature and other information. If possible how can I add buttons, text fields, upload field, drop-down item and so on? I'm not interested in creating a survey form. Please let me know. Thanks
