    Rest In Peace, Jennifer R. Condolences to her family and friends.
    I had no idea that this problem happened originally in 2016!! Southwest and the FAA should have acted on this original failure quickly and throughly. Not waiting to inspect the fan blades via ultra-sound would have, most likely, found the stress fracturing in-progress on the engine that exploded in 2017. It is a shame that only $$$ stood in the way of this preventative inspection that could have saved the life of this lady.
    Once again, this channel is outstanding!! The best aviation channel on RUclips. Thank you for your hard work and dedication. I’ve told all my friends to subscribe to the best channel on YT!!

      @HORSE SIX ZERO that is absolute 100% correct . Tombstone technology is an interesting part of the recreations, I just watch alot of these kinds of investigative discovery type shows , as well as forensic files, etc. My father-in-law was in the Air Force and Navy as well as worked at Lockheed. He worked with several 'high profile now but at the time more secretive" type aircraft designs. He got me really interested in this type of thing, but yes it is quite sad that it appears there has to be loss of life before any real action is taken !

    I was at Pensacola Airport picking my aunt up from her flight back from North Carolina when this happened. I watched it land. Glad everyone was okay. The pilots did an amazing job.

      Well, I can read checklists too and they left the radio channel open by mistake twice and they did not declare an emergency (as far as I could tell from this video). So I am not impressed. They did not make vital mistakes, and that is good.

      @@ronaldderooij1774 They declared the emergency when they set the transponder at 7500!! SOP! That does not require verbal declaration! They may not have left the radio channel on by mistake, a good way to record their actions to back up the CVR! If it was a mistake, the button on the yoke functions for both radio and intercom communications depending on which way it is toggled! In the intensity of the situation it would be easy to toggle that button the opposite way, even for you!

      @@user-xf2no9kq8m Thank you! Years ago a mentor gave me some sage advice! He said in an emergency here are the three most important things to do! Fly the airplane, fly the airplane, fly the airplane! That's exactly what this crew did! Here is how this works in the real world! An unexpected event suddenly happens. The first thing to do is secure control of the aircraft while assessing the situation! That should not be a snap judgement! In the meantime, the Captain assures control, the F/O will dial in and squawk "7500" or "7700" depending on the situation. One is for in-flight emergencies, the other hijacking! ATC will inquire about the emergency in a manner dependent on which squawk is displayed. At that time the crew in this case did communicate the type of emergency which should by now be confirmed! When confirmed the appropriate abnormal checklist for that particular emergency will be called for and executed. This crew demonstrated coolness and professionalism in the face of a stressful event! For armchair quarterbacks to fault them for not calling MAYDAY, MAYDAY and communicating the checklist on the radio frequency shows a lack of understanding of the real world of in-flight emergencies! Situations like this seldom happen, in approximately 30 years of airline flying I experienced only 2 short lived in-flight emergencies, neither of which involved an engine failure or decompression! That crew did an exemplary job!

      Being calm and aware is crucial in almost any career or anything one would do in life from pilots to a person driving down the highway. A calm person is a safe person and that goes for anything and everything.

      CFM engines compare with Russian Models , after looking at the number of similar problems they appear to suffer, why did they not stick with the previous engines agh. yes, money comes into the equation. CFM 'donkeys' appear to be somewhat like The well known Chevrolet Corvettes of some years back. NBG.

    Why is it that the FAA “recommends” and doesn’t “require” - when you tell a company we recommend something, more likely than not they will not act on that recommendation.
    I guess it takes a fatal crash to make “requirements”

    Wow, amazing video!! I love The Flight Channel. Really makes you think about the air worthiness of any given plane you fly on AND the airline's knowledge of that fact. So sorry for the passenger who died on the flight 20 months later. That did not have to happen.

      Exactly. The executives should be held criminally liable. Send them to jail and seize their assets. There's people serving 25 years for silly marijuana charges (I'm not advocating drugs), but these people made decisions that led to a person's death (not to mention all the other passengers who were traumatized).

      The airline has hundreds of decisions like this to make. Air fare Tickets would be double or triple if airlines managements make every single test ASAP or issue. It's a hindsight type of mistake! No intentional negligence on Southwest. It wasn't conveyed how important it is to get it done ASAP! Fan blades are checked several times, just not scanned with a ultrasound. I feel for the woman who lost her life, her family and friends. Things break. If I was the individual that was killed and knew somehow of this I would be frustrated but not to the point of taking freedom away from a decision maker. Although I hope my family would win a civil suit though.. Btw I ABSOUTLY love this channel! I would love to play the program! Can you do early crashes like the one that crashed into a building decade before 9/11 due to low cloudy visibility. I think it's the Sears tower or something like that!,

      As with all things, decisions are often made based on risk and reward. The company "bet" that a critical failure would not occur again and was slow to take action. Not the right bet. As Arishia and bigace mention, there is a cost factor involved and when people way risk v. reward, they don't make the best decision. Think about the time when one said, it's just a few blocks away I don't need my seat belt; or I'll only be inside for a minute (while the kids are playing by the pool). Not excusing the airline, just providing perspective.

    I work in a CFM56-7B engine shop, and for many years. I clearly remember when this EAB hit my desk. I'm not sure where the amount of $170 came from but I can tell you that I make $60/hr and It would take me a lot longer than 2 hours to perform an ultrasonic inspection of a fan blade. *ESPECIALLY* If the engine is still on the wing. The CFM56-7B has 24 blades that each need to be inspected. Per the EAB directive, it takes about 15-20 mins to properly inspect ONE blade. This will get you approximately 6 to 8 blades in 2 hours @ a cost of $120 just in my salary. I spell all of this out to say, you're basically inspecting every 3rd blade per this EAB. Luckily, my airline and many others, usually always add additional testing beyond what is required by the FAA. In the case of this particular EAB, we inspected every single blade. Engine on wing or not.
    I never could understand why the FAA didn't issue an AD, (Airworthiness Directive), which is the *end all, stop everything and check this IMMEDIAYTELY or else* in the aviation world, considering how serious this was. Unfortunately it cost Jennifer R. her life. Continued prayers for her friends and family!

      It appears that this problem was due innate engine failure. Lately it seems that the CFM engines are structurally defective and by the nature of the beast, they have been been on most planes in this current era of flight problems.

    First off, RIP Jennifer, second, fantastic informative video, as always, third, well done the professional response of the pilots, fourth, too slow a response by the regulators and operators.
    On a lighter note, if that is possible, the first report was to Houston, reminiscent of Apollo 13, "We have a problem".
    Finally I think many potential passengers will not want to fly on the 737 max if and when it returns.

    Folks iv worked for SWA since 83 and only one fatality on a SWA flight in all that time ,one other fatality was on the ground, our crews are the best of the best no hot doggers ( well at least not anymore lol ) but seriously I don’t think any other major airline can match that record. American Delta Continental Eastern SwissAir Air Canada Air France United and on and on SWA dose not cut corners and most employees are dedicated professionals, that’s across the board.

    2 Hours and $170 is non-sense. The plane has to be taken out of service for two days. The fan assembly removed. Remove all retainer clips. remove each blade into a rack to ensure they all go back into the exact same slot since each blade has a unique weight, dynamic balance must be preserved. Inspecting one blade might take 15 minutes. There are 28 blades. Then the whole fan assy must be reassembled and the engine test run. Expect balance issues for one in 5 engines so serviced. If any blades are defective the whole engine must be replaced and new blades applied via computerized balance mapping at CFM. The FAA and Boeing are one in the same. People hired back and forth. The lack of oversight or appropriate response is the same failure mechanism that doomed the 737 Max planes. Problems with MCAS were known in 2012 but never fixed. Now Boeing Commercial Airplane Co may be sold to Airbus. The other half - Military - will be all that remains. This is what happens when a technology company is run by bean counters and DOCUMENTED CONCERNS OF ENGINEERS ARE IGNORED. 2nd Qtr earnings went from positive $2.4B to minus $2.9B. So the 737 Max MCAS stupidity cost 2 aircraft and $5B in one Quarter. Boeing managers show no evidence they can learn from their mistakes. Is it time to short Boeing stock?

      Not all of the money goes to the airlines, most goes to the airport ground services, runway usage, and lounge fee.

    Wow. So this was the previous one before flight 1380. They should have fixed the problem before. They just waited for another accident to happen. What the heck. Anyways, I love this episode. This is better than Air Crash Investigation. Because it does not show the crash animation. I love this video. It's a masterpiece. Can you make an episode on what happened on Qantas flight 30?       Ronald Harrison, The CFM-56 has a superb safety record...it’s s well-engineered product. Maintenance is the issue here. Aside from risks of ambivalence, nailing acceptable intervals of inspections is in part a guessing game. And it will always be a balance between conservative prudence vs. the almighty dollar. Everything mechanical resides in that world.

      @@Snookynibbles The first fan failure could have been a "freak" occurrance, but since they couldn't pinpoint the cause of the fracture, it would have been a no-brainer to check other fan blades to see if such fractures were more common but "hidden". Maintenance is NOT the problem however, the maintenance regime required was spelt out by the FAA but the fact that it wasnt acted upon by the airlines. Simple greed over concern for the passengers.

      There is a term for this behavior. It's called "regulatory capture" and it goes on all the time. The regulators get too cozy with the people they are supposed to be regulating and safety becomes a secondary concern.

      Just looking for anything to make it political. The fault here is with the airplane manufacturer, not the President, regardless of Republican or Democrat. So does that mean Jimmy Carter did nothing before or after American Airlines flight 191 out of Chicago killing 273 in 1979? Your a fricking idiot.
