Once we become an adult we move out and try to live on our own. Our parents I assume die and we get the house in our name. When we are evicted out of our apartment we move back into our childhood home which is across the neighbors now burned down house. Moving back into there made us think and remember everything from when we were a child living there. Causing us to relive it all again

That silhouette is your fear of the neighbour. The neighbour also sees the same silhouette (seen at the end of act 3) but in child form and that is his fear of his last child dying.
