This is the funniest thing I've seen in a long time. No joke, as a large black man myself I am very very very reluctant to help a white lady, I just stare at the situation and think to myself "This Is A Trap!". :)

Like even when nothing is “wrong”, as in there’s no “external issues” to point to to say “that’s the reason I’m not happy”. It’s not being able to get out of bed & do things you normally would without reason.

Sometimes it's purely physical. Intellectually you know you're okay, but your body is freaking out and feeling like it's going to die. Trust me that if it were just a case of mind over matter there would be many less anxious people in the world.
I think part of the misunderstanding is because "anxiety" can be a medical condition or a feeling caused by worry as you say -- there's a wide range of meanings for a single word. It's like saying someone was mad -- were they mildly irritated or were they insanely homicidal? They could be either as the word is so general.
Please try to have more compassion the next time someone tells you they're dealing with anxiety. If y'all are close, ask what they mean. If they're worried about something, your advice is great and may be just what they needed to hear; if they feel like they're having heart attacks, but it's "just" panic attacks encourage them to see a doctor or just commiserate and move on.
