He probably has very large buccal fat pads. Genetic trait. Very difficult to to reduce even with intense dieting. He'd probably need to have them removed surgically.

Me as a natural with good genetics, even before I knew about carbs, proteins etc this was in the early 2000s when I was in high school. I subconsciously knew that carbs were the go to nutrient to make me look fuller, and never sacrificing any definition. At the time I could literally eat whatever I wanted and not lose definition. Even till this day (esp knowing now) if I feel flat ill eat some white rice, and in an hr or two I start looking fuller, hydrate myself as well so that my veins look fuller and more dilated.
