Pediatric Resuscitation: The EMS to ED Hand-off

  • The chain of survival is critical in order for communities to improve neuro-intact survival in cardiac arrest (adult and pediatric). The EMS to Hospital Hand-off is a critical step in this process and it often is structured in a way that disrupts the flow....blood flow that is.
    We believe that EMS should be an active participant during ED CPR instead of simply transferring care and walking away. The airway device (i-Gel) doesn't always have to be pulled and the monitor can stay on for at least 5-10 minutes. EMS personnel should stay in the triangle and perform their roles so the ED team can focus on other items.
    Let's widen the lens and begin to train with hospital staff so that outcomes can improve.

    It pains me to not see every pedi code run like this. Fantastic!!! Dr. Antevy, what is your opinion on swapping the supraglottic airway for an ET tube after transport is initiate? Would you be ok with letting the crew pull the iGel and take a quick look and intubate if they can easily visualize vocal chords?

      I do give them that leeway yet if the O2 sat and ETCO2 are wnl then they won’t swap unless needed.

    Personally I’d go for a fetal Stimulation to see if there responsive over pressing down on a baby’s chest yelling “ baby baby “ but I guess that works, in EMT school rn & I feel you’d get a better chance at reaction from the bottom of the feet

      There is no age limit in either direction.

    June 2020. Height of Covid. ALL hospital staff wearing masks properly. FF instructor not wearing a mask at all. First FF walks in wearing the classic "right below my nose wasting everyones time" mask. Like what's the point...

      June 2020 was not the height of covid though, thats when we were first being told to always wear masks in public, so people were still getting use to it at that time. Last quarter of 2020/First quarter of 2021 would be more the "height" or "peak" of covid, thats when everything was definitely in full swing and people were told not to gather in large crows and keep your masks on
