I'm in a similar boat, with a higher margin product. I've started hiring part-time team members. Wish I could hire full-time, but Ken is right. he has to start delegating. Beware caller: Hopefully, this isn't a problem where he feels like he can't trust anyone to do the jobs he's been doing well already.
I thought this guy called in to brag, but considering he has 500 clients and is only pocketing maybe 50K from 300K revenue is nothing. Especially if the guy is getting up at 2:45am. He’s not even at the level where he can hire employees yet - there’s no money for new employees yet. He does need to come up with more efficient systems of doing things, because he’s not at a level where he can hire folks. At least according to the numbers he mentioned.

This guy needs to hire staff...at the very least a bookkeeper and a receptionist that makes the appointments

Simple: Demands that you work as a bureaucrat instead of a craftsman.
This dude takes forever to get to the point, so annoying.
He’s exhausted. It sounds like he has trouble thinking clearly because of it.
