Hi, what if in the Disability portion the patient is confused and he scored 3, what should I put in the monitoring? Should I put minimum 1hourly since it's single parameter or should I put weekly since the setting is in the Community? Thanks for the response.
For confusion in NEWS CHART,a patient can only be scored if it's a new onset of confusion,so if it's a new onset of confusion and the only place the patient has a score of 3 is in disability the frequency of monitoring should be 1hourly.However if the scenario is for a known dementia patient that's not a new onset so the patient will score 0 for disability.

I thought wearing gloves and touching patients document at the same time is a red flag? Is Apron and glove really necessary for assessment based on the marking criteria?
If a patient repetedly say i was a nurse before after 1min she will say again i was a nurse did i told you before again the same..and when i reached acvpu i asked date and time she told me incorrect date but correct time can i write it as confused
@@oluyedeeunice6940 Once the patient is suffering from Alzheimer's that's a chronic case. An Acute confusional case would be due to a fall meaning it just happened recently.
Hi Emily, yes if you are using NEWS chart the standard of the monitoring frequency will be applicable. A residential home is still in the community, Nurses and health workers are available to do that.

Hi, during the procedure, it is ok to write with your gloved hand, it is considered a clean procedure. But you can remove and discard your PPE , do hand hygiene at the point that you will interpret your NEWS chart and ask questions on Activities of daily living.
Use of PPE is not compulsory for Assessment based on the marking criteria, ensure you do hand hygiene as required. But if you choose to use PPE, you will not be penalised for that.
Hi, based on the marking criteria, PPE is not stated. You have a choice to use it or not. If you use it it is acceptable, if you are not using PPE please do hand hygiene as required.

Hi, Re-orient the patient, try to have a critical look, ask more questions to establish that the patient is confused. But be mindful of time.
