  • How can a profession with amazing potential for human connection fall short and fail to offer compassionate care? And why do nurses too often go in the other direction-and do harm?
    Registered Nurse Kathleen Bartholomew dissects the hierarchical culture within the nursing profession, and examines why nurses who are devalued and shamed are much more likely to make mistakes and relay critical patient information to a physician.
    Kathleen Bartholomew, RN, MN, has been a national speaker for the past fourteen years. She was nominated in 2010 by Health Leaders Media as one of the top 20 people in the U. S. changing the culture of healthcare - specifically for calling attention to the dangerous impact on patient care of disruptive behavior by medical professionals, as well as the critical need for better physician-nurse communication.
    Kathleen is the author of five books and is best known for her pioneering work, “Ending Nurse to Nurse Hostility” (2006), which offered the first comprehensive and compassionate look at the etiology and impact of horizontal violence on both patients and nurses. As a health care culture expert, Kathleen now speaks about patient safety, communication, leadership and power to hospital boards, the military, senior leadership, and front line staff.

    As a nursing student , I am constantly witnessing this bullying and have experienced it first hand myself. It is so toxic. I have it in my mind to make sure when i graduate and earn my license as a registered nurse, that I can find a unit to work on with emotionally healthy leadership. It is SO so SO SO important. We need to take care of each other. We need to lift each other up. I loved this Ted Talk. Thank you!

    I always tell my staff that no one is more important than anyone else: we are all equally important and we all must work together to function. Titles are thrown out the door.

      teams need leaders, and thus a hierarchy. some severe medical mistakes have been made because nobody took charge.

    As a longtime nurse who has worked in L&D, Med-Surg, step down, ICU, and ER, I could definitely recognize the hierarchical stepladders you showed. I have definitely found myself buying into them.
    It was actually only after I moved away from the bedside (to Infection Control) did I truly get the necessity and importance of every single person on the team, from Security, Housekeeping, Dietary, and Facilities, to Respiratory, Imaging, Lab, Nursing, and Medical Staff. It’s only when everyone recognizes the importance of every other piece of what the hospital is here for, that we can give every patient our best efforts.

    I wish she was my boss or nurse admistrator. All RNs Physician cafeteria workers housekeeping secretaries patient observers everyone is a valuable asset to a hospital or long term care health care facility. Unfortunately healthcare has become business oriented industry. Short staffed where RNs on average have 6 to 10 pts. One housekeeper night shift available for entire hospital.

    I just randomly came across this video today. I'm an Athletic Trainer and currently applying to nursing school.     YOU'VE NAILED IT!! I have been bullied and back stabbed ,set up for failure, but I persevered in this non -altruistic field, for 38 years. Nurses can be HORRIFIC to each other. The hierarchy exists on many different levels. What facility do you work for or skills you have, much schooling, what school, what title? What license? All make you feel welcomed or alienated or shunned. In the meantime the Job alone is stressful. Then add in that toxicity. A good freind said, " a nurse can be stabbed a thousand times by her /his co workers." And the patient or resident suffers! When will it end? I , like others can't wait to retire. I will run like the wind!

    This is an excellent lesson about humility and lateral violence. I'm starting my nurse residency in the Neuro ICU, and I'll remember this lesson as I continue learning to be a good nurse.

    Wow this just made me cry. I’ve been in school for over 8 yrs finishing up my prerequisites & just a few weeks ago I decided no matter what Iam going to become an RN. I know I have a calling but raising my kids going through losses and life I’ve been keeping my dream aside. This just made me say Yes I can do it, thank you so much. I know it’s hard but what a way of giving back to the world by helping others 🙏🏻

    This makes me miss my old unit so much... and reminds me why I did not stay at that hospital... I worked as a nurse aid at a local hospital for one year and it was by far one of my favorite jobs until they shut down the unit, and it was my favorite because of the people on my unit. The nurses treated us aids with a lot of respect and reminded us that we were important to the unit. If the unit was overwhelmed the charge nurses were right there to step in and help without blaming the other members for not being on top of things (it was a fast-paced admissions unit). The doctors always expressed appreciation for what we did. Our housekeeper was one of our favorite people, and while she didn't specifically work for our floor she was nearly always assigned to us and we once voted her employee of the month for the unit because she was awesome, and we would help her out whenever possible. Our unit regularly did "family dinners" where we would all get together for cookouts.
    Why were we shut down? Our unit didn't make as much money as another, so they eliminated our unit and expanded a more profitable one. We all lost our jobs and were told to re-apply to other floors if we wished to continue working for the hospital. Made moving to a company with such a clear hierarchy and disconnect really difficult.

    I love this concept. I'd like to see it carried to the schools too. No matter if a student is on the football team, a cheerleader, the brainy science team, the reading club, or special ed,... every single student in the school is just as important as the next. It seems that adults like to promote "anti-bullying" at the schools, but jeez, I can't help but worry that the kids learn how to bully from the adults. 🙄 Great message!!

    I needed this. It is so easy to get worn down. Why do we fear calling doctors, night or day shift? Why is it ok to be cursed at by co workers? We are nothing without each other. If we could treat each other with a little more kindness, a little more humanity, I think it would show it's benefit in patient outcomes and employee resiliency as well.

    We need to improve power dynamics in healthcare in order to save patients lives, but also to credit nurses the value that their work has. No health institution functions without nurses and that fact says a lot.

    I needed this talk indeed. I am at the moment through the worst horrendous experience as student nurse. I could not find consolation why things went wrong on placement. What I did wrong? but now I can understand that without support, team job and honest approach nurses are nothing.

      @Fernando Rego it sounds like one particular placement was bad in her case. There are so many types of nursing. A bad placement on a bad floor will be horrible and make anyone question their career choice! But that does not mean they shouldn't be a nurse or that it "doesn't get any better." They need to find a non-toxic environment with supportive staff and in an area of nursing they find interesting/enjoy.

      Nursing isn't all about sunshine and rainbows. its a bloody hard job to do. Sometimes we all have incredible shifts where we think 'yep I love this job' then we have a bad experience which makes us say to ourselves' why am I putting myself through this'. But this is why we reflect. Having bad experiences makes us better practitioners, it helps us find answers and challenges when we reflect because it helps us realise what went wrong, why it went wrong and what I could do different in the future. Sometimes we honestly do need a bad experience to help us think. Im a student nurse in my final year, and ive had some bad experiences, some emotionally challenging ones, some morally challenging ones but ive also had some incredible ones and theyre the ones who keep be going. But reflect upon your bad experience, its great for self-awareness, for recognising your weaknesses and your strengths and ways you can change that which can better you and your practice. Keep going hun, you can do it.

      Patricia. Did you end up being a nurse?
      5 years ago I wanted to quit university so bad. Then I graduated 6 months later because I was mid 30's and felt stuck as to where to go next. Wanted to tear up the bachelor I received in the mail. Didn't go to graduation, I hated the whole institution. But I ended up going through motions of getting myself back up and now permanent in a great nursing job. Don't get me wrong. Yesterdays shift was thoroughly awful but the good outweighs the bad. And now I don't give a toss about some of the negative nursing 'personalities' we have to deal with. 🙃

    This is so incredibly accurate. I am a unit secretary and feel like I am respected by most of the nurses. Last week I happened to be in a patient's room when he coded and I called for help. He's alive so far. However the work atmosphere is definitely toxic. We all gossip about each other, staff, patients, and say things we would never say to anyone's face. I realize that it is a problem and needs to stop.
    Even just one person's attitude can affect a whole unit. The first time I was in treatment for anorexia, I sunk into the toxic, gossipy, and hopeless atmosphere of the unit. The second time I was hospitalized, I did everything within my power to spread an atmosphere of love, authenticity, and recovery. It made a difference and everyone benefited.
    Although I am shy and feel unimportant in the hierarchy of the hospital, I have to remind myself that I've done this before and can do it again. Tearing each other down does nothing for us nor for the lives of our patients.
    I am so unsure if I should be a nurse, but I wonder if my actions last week are a sign that I should be. I hate science though so if anyone has any tips that would be great 😅

One of them is a spot on description on how one of the most important core values in nursing is so ignored as a value in the New Public Managment system where healthcare is "industry" " .in the healthcare field, there is often a lack of recognition for important actions and services, such as the absence of a billing code for an elderly gentleman's grief. This leads to nurses devaluing themselves because their contributions are not given sufficient worth or cost. Instead, they are treated as something free and easily available"!

    Wonderful video! I agree whole heartedly! It is not as easy to gain the results you speak of on a daily basis. I was groomed with the attitude that everyone & their work was essential & my first chief of surgery would mop floors & transport patients. It took me 26yrs to write BSN behind my name because of peoples' attitudes. I will do anything to help make the unit work more efficiently.

    As amazing as nursing is as a profession, I would never make it an ultimatum that forces someone to be a nurse, especially if they don't know much about it. Through my career, I've known many nurses who hated their job. That hate becomes dangerous to patients and blocks the will to learn more about medicine. It is a tough job and it's really not for everyone. As with any career path, consider yourself lucky if you get to a point of loving what you do. Many nurses may never see that day.

    Many medical mistakes happen because of human factors. its more the culture than the heirarchy. Many airline accidents are the result of the captain and first officer not communicating properly. Either becaues the first officer is apprehensive to speak up and/or the captain is dismissive and treats their co-pilot badly.
    That is why they train to make sure they talk to each other and that subordinates input is welcomed and mandatory for saftey.

      I thought she was referring to a ward tech or phlebotomist. But a lab tech (Pathology department) are viewed in high regard (imo). That being said phlebotomists are equally amazing and are able to get the difficult sticks I can't do.

    Guys I left nursing bcos of Poor Adminstration.. let the doctors and administration do nurses work.. no point of fighting for this job!

    Still sucks seeing the system ripped off everyday while ungrateful bosses co workers try to throw you under the bus
