Realism is one context where the M16 is more useful than the AK. The higher rate of fire and better accuracy of the M16 makes it easier to go for headshots, which is more important on realism if you don't just use shotguns, magnums and melee weapons.
Getting the achievement for finishing all campaigns on expert wasn't enough for me and my group. We're now suffering through it again on realism. I expect to see at least one friendly incapacitation with a shotgun. i'm smiling ear to ear while watching the suffering. (also we don't count expert unless you have all 4 human players) This is because expert is super easy to cheese with only 2 players and 2 bots.
