- Two words that strike fear into the hearts of patients and surgeons alike.
In #facelift and #necklift surgery, sensory “#nervedamage” is normal and expected.
It is unavoidable when lifting the skin in front of each ear and under the chin. Like rolling up sod off of the soil - the tiny roots get disconnected. That skin goes temporarily numb.
This is totally normal, and resolves over 3-6 months’ time as the “roots” reconnect.
This can sometimes be accompanied by the feeling of little ”zingers” or ”lightning bolts.” These are good signs, and suggest returning function.
“Motor nerves” are the nerves that control the motion of facial muscles. Damage to these nerves is what most concerns facelift surgeons and facelift patients.
When they are injured, we see changes in the motion of the face such as smiling, blinking, or pursing the lips.
With experienced surgeons, permanent motor nerve injury is almost unheard of - fewer than one in 1000 cases.
Temporary nerve injury is much more common - about 1 in 30 patients (3-4%).
Temporary injuries are most likely due to stretching of the nerves, bruising of the nerves, or partial injury to the nerves by use of electrocautery to stop nearby bleeding.
These temporary injuries are not 100% avoidable, and even the most experienced surgeons encounter these a few percent of the time.
Permanent nerve injury is due to having the nerve actually cut. Even these injuries often improve with time As nearby nerves sprout and work overtime to make up the gap
With experienced surgeons, permanent nerve injuries are almost unheard of (fewer than one per thousand cases).
While it might be tempting to think that certain techniques are safer or riskier than others, in reality, it is certain surgeons that are safer or riskier than others.
Choose your #plasticsurgeon carefully!

Hello, Hopefully by now you'll be back to normal. I had this nerve problem on my upper lip for 2 months and a half, it was extremely difficult for me but please know you'll get well and beautiful. it's only a matter of time, and of being patient. I did some face exercises that were recommended by my Dr. but mostly it's just time. You'll be ok believe me. @@lauheid

My mom had a deep plane facelift 6 weeks ago and has problems with her cheek function not properly lifting her right upper lip, leaving her with a crooked smile. Thank you ❤

Dr. Nayak, you have completely given me hope! I literally just had my surgery 5 days ago and by day 2, that my smile began to look paralizyed like the beautiful lady in the video! neck liposuctioned with Renuivon.-I think I'm extra frightened because I don't know enough about Renuvion and whether this is the "direct" reason for the nerve injury. I am a 56 year old female and I'm just worried about nevve regeneration. I also had other body work done from same doctor who was phenomenal-I do not blame his skill. He did say to me, "Looks like a nerve was hit with the Renuvion". He then explained that Renuvion has not been indicated to cause permanent paralysis. -I went out of Canada to the US to see a top docotor featured on Real Self and just truthfully, he is incredible at what he does. I just keep getting nervous that mine (which is even slighter than that pretty lady you featured), that it may not heal becasue of the Renuvion. -Can you please let me know if you think that that is not an issue (the medium used)? Mine was more crooked on day 3 but now on day 5, with me just doing gentle massage with tallow balm, I feel like it has slightly "picked up". Thank you so very, very much! Isn't it amazing how one doctor such as yourself, can go out to the whole word via a short video and make them feel reassued?
Is there any exercise or treatments that can be done to expedite the nerves recovery ?

Is there a facial nerve in that area? Ive seen alot of images online regarding the temporal nerve and they are not consistant. I cant find the answer to this question.

Thankyou, this video has put my mind at rest, I am nearly 8 weeks after surgery and was getting worried I would permanently end up with a crooked mouth even though my surgeon told me to be patient, I wish he gave me an idea on recovery and that this is quite common. Thankyou once again for this information
I'm 7 days out of face;neck lift surgery with a crooked smile like your subject. My surgeon is discussing nerve stimulation therapy but I was an out of town patient and have no idea how to find such therapy should it come to that. So all in all I'm shocked and worried with no idea that crooked smiles are as common as you suggest. But thank you for your thoughtful explanation.

Dr. Nayak, I was involved in a motorcycle accident in Thailand and had to have my face reconstructed. My nose and jaw were broken, and my checks were crushed. L have server numbness in all of that area. It has been 2 1/2 months since my surgery and no improvement yet. The surgeon didn't explain it to me as well as you have, maybe because on the language difference. Thank you for your explanation. It gives me hope. I hope to see a plastic surgeon when I return to the states in a couple of months. Are there any exercises I can do to help my nerves heal faster?
Hello, doctor! What do you say about the effects of hyaluronidase? Many write about side effects, including on the nerves, as well as the destruction of connective tissue. What is your experience in this matter?
I have the nerve damage and smile issue after neck platysmaplasty and submandibular gland reduction. It’s 2 weeks post op and I’m hoping it will improve soon. My surgeon did briefly mention this risk to me before the surgery but I didn’t think too much of it at the time.

Hey, Dr. I had Pall’s palsy for 4 months and I got better by 70%, however, my lips are still not moving normally and I have difficulty when I want to pronounce some letters; especially the lower lip. Is that normal??
Hi doc. Im not sure if ur still replying. I had a chin surgery to remove the silicon. And also “undesirable” unknown according to my surgeon. Fats or tissue to make the shape nicer. The insertion cut is beneath my chin (exterior) now passed 3months. Keloid had formed and healing were ok. But when i smile. One side is not moving. Its been 3months and my doc insist its not nerve damage and got to excercise my mouth n lips to get the lip muscle back. But i don’t trust him. Pls advice😢its giving me much anxiety n sleepless night

Thank you for your information! I am still hopeful that my left side of my smile will return! I am 9 months out, there was also work done on submandibular glands. I know my surgeon (Dr. Steven Cohen in San Diego) is wonderful. This was the risk I knew of.. Hopeful my nerves will wake back up the rest of the way.
Dr. Nayak can you please help me answer to my question about mandibular nerve. It was hit with syringe and in that place I also have a mini spasm. After that I developed neuralgia, the pain originate in my right ear. Can this be correlated from injection needle? Does the mandibular nerve regenerate after being hit by syringe? Thank you for this! I currently have motor nerve damage and my smile is crooked. I’m 6 weeks PO from buccal fat pad removal and chin Lipo. I can’t wait to have my smile back again!

@@moskam1670 he believes it’s the inflammation in my chin/neck that is causing the muscle to not work as well. He says it should heal with time. It is much better now but still not 100%. I’m 3 months post op now
@@elsaweisel5344 Aha ok. Glad that it has improved :) Im 2 weeks post op and I have crooked smile, numbness and can’t control one side of my mouth area. I got so scared that it might be permanent but im happy to hear that it will heal.

I got mine when the surgeon was giving me local anesthesia before he even started. It has been 10 days and no change. It is worse than your patient and others notice it when I talk forget about smiling.

I had a facelift 15 months ago, and since then, my left eyebrow has been immobile. It has a little movement at the end of the brow, and that’s all. I have lost my facial expression, and I feel so worried that it will stay that way. I started physical therapy and electrotherapy. I have used Proserin and Nuclo injections, but none of these have helped. What can I do? I do not want to get botox on the right side
I'm 3 months PO from deep plane and neck lift. My entire surgical field burns, tingles and at times hurts. My surgeon has never had a patient with my problems. He is sending me to a neurologist, does this sound like permanent nerve damage
I had a medpor jaw implant shaved down on the right side of my face. That was three months ago. Immediately after I had a crooked smile. In the last few weeks my chin is starting to look deformed. Today I bought an acupen to electrically stimulate the marginal mandibular nerve. On my injured side the nerve does not respond at all. On my non-injured sign I'm able to get my chin and lip to twitch when I stimulate the nerve with the acupen. I'm afraid that my nerve has been cut and is permanently damaged. Have you had any luck? Thanks!

4weeks post deep plane facelift/neck lift looks same as 1 day post op marginal mandibular damage. Are there any excercise or electro stimulation that helps, or would those impair or delay healing?

I did a mini lift and double chin lipo 6 weeks ago. One side of my face got very bruised and paralyzed. The other a little paralyzed at the beginning and not bruising and now has a lot of movement but I can tell it needs to keep healing. The paralyzed side took long time to become normal in color. it was purple, green, yellow. I still have a big part of my cheek , side forehead (next to my eye) and side cheek with some dull pain and almost no sensation. I can see tiny improvement every day. The doctor said I will get back to normal and to be patience. However there is one part of my face which has no improvements at all and I'm getting nervous. It's the muscle or nerve that make you pull your lip up. like if you are grossed out, you will pull your nose and top of the lip. Ones side I can do no problem but the other doeskin even try. Also, I cant move my eyebrow to do an angry face, is if I had an intense botox in there. I can pull my eyebrow up but getting angry stays in neutral area close to the 11s is like it those nerves doesn't exists.The other eyebrow works ok. The numb area on the paralyzed side gets smaller little by little but I don't understand why those specific areas don't move at all. If there is any past experience with this I will appreciate it. My doctor keeps saying everything is ok but anxiety and worry is going high . 😭

@@mrf741 im on week 12.. several nerves suddenly like magic started to work around week 10. But I still have few ones that are not working. At this point if i have a rested or smiling face you can not notice anything weird in the face, not even asymmetry.. but if I smile showing the teeth or if i try to make an angry face as in lowering both eyebrows, you can tell in one side i can no do the normal expressions. im just hoping those 2 or 3 nerves that are still paralyzed suddenly start working like all the others.

@@isabel1614 I'm over 6 months after surgery. I think I'm about 90% recover. I still have some inflammation in one of the sides. You can say I look normal but my smile is not symetric yet nor other little details. Went to the doctor today she said it will go back to normal but it is taking long time. We are not sure why I had such bad reaction and inflammation. It has been a long journey but overall It is better than before but it was not a big change because I was nearly starting to have jowls. I just hope it slows down starting to show again.

Thank you for your clear and reassuring information - I have lower lip weakness after a deep plane face and neck lift with chin implant 3 weeks ago. I have been feeling extremely anxious about how I look and speak and dont feel so hopeless now. My surgeon says he expects to see it resolved in up to 12 weeks and I do trust him but its hard when you look in the mirror every morning and dont see what you were hoping for -Ii keep thinking I'm being punished for being vain :(

That is so scary. My surgeon said if it is not fixed by the 2 year mark it is pretty much permanent at that point.
