Start Taking Your Thyroid Medication at Night

  • Contrary to popular belief, you don't HAVE to take your thyroid medication first thing in the morning.
In this video, I discuss some newer studies which show that taking thyroid medication in the evening can be just as effective (if not more effective) than taking in the morning.
The takeaway here is that it doesn't really matter when you take your thyroid medication so long as you are taking it on an empty stomach.
And to do that, all you need to do is make sure that you take it 2 hours after (or before) a meal.
This leaves the stomach with 2 hours to empty which means that food will not interfere with its absorption.
There are two ways to transition to taking your thyroid medication in the evening and I call them the non-skip approach and the skip approach. Most people have no problem taking their thyroid medication in the morning and then again in the evening but if you are sensitive then you may want to try the skip dose approach.

I asked my endo about taking it at night and he asked why. I replied so that I could walk to the coffee pot when I got out of bed. His reply was “that’s a very adult reason so just wait at least 2-3 hours after eating”. My levels have actually been better since doing so.

I just tried this as a new thyroid patient and I swear I feel much better today! I hope it continues. My one question which no one seems to know because these meds are so new and what I think is likely impacting my ability to absorb all the thyroid I’m taking, is I am on Zepbound, a GLP agonist. This slows stomach emptying to a crawl so I don’t know that it’s possible for me to take thyroid on a truly empty stomach while on this med, but taking at night did seem to help at least!

Been taking levothyroxine at night for quite a few years now, and you shouldn't be eating so close to going to sleep anyway so the empty stomach thing has never been an issue. Sleep solidly most nights as well, so doesn't effect my sleep. Bonus as well, is you can eat straight away after waking up without having to wait 30 minutes and worrying about things interfering with the medication.
