I found getting my hormonal acne in control first with spironolactone, tretinoin, and aczone helped with further PHP. Also with religious use of mineral sunscreen, vanicream (on a 3mnth break), and cerave moisturizer my skin looks amazing. I never thought I could ever say that in my whole life. So blessed to have found my Dermatologist Dr Sax last year and the wonderful advice and knowledge of Dr Dray. Everyone should see a Dermatologist in my opinion.

i have quite a bit of persistent hyper pigmentation mainly over the forehead area. One thing I found effective but you need to have patience and you ll get results over time is: The juice of onion mixed with apple cider vinegar and applied over the dark spots. It won't make them disappear completely but I found over the period of one month they became less perceptible to the eye and easier to conceal with less cover

Thanks for the info I wanted to do a Lazer treatment because I have black marks/scars from trying to unclog my skin which didn't work and my dark spot seem to get worse then I found out it was fungal acne, I'm sure not too sure of the chemical peels since I have sensitive skin too I'm screwed.

I've been able to clear up my hormonal acne using tretinoin and other products, but was told by my esthetician the other day that I now have hyper pigmentation (I can't seem catch a break with my skin, something is always wrong!). So glad you made this video, hoping some of these solutions can work out for me... :)
Hydroquinone is safe to use upto 16 weeks twice daily but break is important as it can also cause discoloration , sunscreen gets more important during the time of use....it can also cause irritation

Wendy F I loooove my Dr. Pen. I do have Melasma. But I use my pen for acne scaring and texture. Makes my skin smooth, helps my products penetrate deeper and my make up goes on sooooo much better! Although this might not be effective on Hyperpigmentation, you may find it beneficial for other areas of concern.

Thanks so much! Like I say, the vitamin C is very unstable, but as an antioxidant, i suppose that makes sense so as to theoretically scavenge any free radicals

Hi there, the first thing you can do is to use sunscreen to prevent further worsening of your melasma. Try cerave pm and a retinoid in the evening and see if it works. Kind regards!
Hi Dr. Dray, thanks for another great Q&A. I tried the Neutrogena Ultra Gentle Hydrating Cleanser Creamy Formula, and I really like it. I was trying out the CeraVe Renewing SA cleanser prior, but I was getting some redness, sensitivity and dry patches from it (could have been over using it, so instead I'm going to give it a shot on some KP areas). The Neutrogena wash had a really soothing effect after using it on my skin. It does have that Cetaphil cleanser look and feel, but I seem to like the Neutrogena more - feels like it cleanses better I guess. Thanks for the recommendation, and I'm going to try the Ultra Gentle foaming later. Thanks again, and hope you have a great weekend!!!!

What you say about it is so true but my question is....why the dermatologist don't explain to the patient that this ingredient helps for a little while but after all, the Melasma will come back even more strong? I would love that a doctor can tell me this, directly. I prefer to know it has not cure that believe that I can do something with this kind of ingredients that on top of that, has many side effects. No good at all.

I have very stubborn melasma and light areas too. Recommend Estee Lauder Camoflauge make up. I mixed it with tinted sunscreen. It really covers for about 6 to 8 hours. Nothing stays forever. I am adding the soy and fern. I already switch back and forth from hydroquoaine to vitamin c with Rodan Fields skincare. (take breaks). I use 30 spf and put bullfrog on if I hike, plus reapply sunscreen. Melasma is nasty stuff. The light areas too (from removing acne scars) areas on my face don't help much. It is maddening. Tried laser and it did nothing. 5 treatments from a reputable dermatologist and paid more than 2.5K. My point - thank you so much for your no nonsense clear layman's terms video on this subject. The amount of babbling 20 minute videos with bull$--- out there drives me off youtube from time to time. Other times, doctor dive deep deep into what causes it, but offer NO solutions but "sunscreen". Thank you thank you! Merry (early) Xmas Dr. Dray!

Dr Dray hello! 👋 In the 90's i went through a very stressful and traumatic situation. I noticed soon after that when I went out into the sun, mostly in Summer, any skin exposed would break out in hives, including my face. So I guess the stress opened me up to a sun allergy. Before this happened I used Ambi sometimes. But then Ambi started giving me hives too. Do you know what happened? And what can I use besides hydroquinone to lighten dark spots that won't give me hives? What would be the best product for me of the ones you mentioned? Thank you in advance. 😊

For a natural remedy- do you think a pure Aloe Vera plant the gel applied over post inflammatory hyper pigmentation could help being that pure Aloe vera contains traces of Vitamin C, A and Zinc?
Thanks, this was really informative. I've tried the Timeless Vitamin C serum, and a similar one by Skin Deva, but no lightening of age spots. I've also tried a product from Paula's Choice with licorice extract, and alpha arbutin from The Ordinary, and neither lightened my spots. I did have a little success with a drugstore hydroquinone. but I'm concerned about those dark spots you mentioned, that are permanent. Is there a way to avoid those? Thanks again for the video!

Thanks! Unfortunately, all the drugstore versions of hydroquinone have fragrance. Paula's Choice has two options without fragrance, but they have either BHA or AHA in them.
So glad you made this video I spent over a thousand dollars on laser treatments for acne scars and saw no improvement.. i actually think my skin is worst... I have now found a new dermatologist that prescribed triluma but was unsure about purchasing because it’s so expensive and not covered by insurance.. I’m so glad you mentioned it because now I feel more comfortable trying it out.. do you know of any coupon sites for triluma?
cassandra dixon I used alovera for scar works well but not for dark spot once you put on your face it very sensitive to the sun will make your face darker happened to me and my man.
Have you considered doing a video on acanthosis? If you have, would you remind me of the video. I don't recall seeing it. I know it's an endocrine and metabolic (or genetic, malignancy...) issue, but so many who have it think that scrubbing the areas will make things better. Would products (retinoids, topical vitamin D analogs) be helpful? It would be a great video to address the underlying issues and what one can do at home in addition to lifestyle mods. Thank you!

I have a patch on my face which was smaller. I have been using the Lancôme Genifique serum, and Lancôme visionaire moisturiser for multi coreection of skin. My spot has got lighter. It's getting larger though . I see small patches appearing in different spots around the Area too. I do not have acne. Will it help me if I used the Differin gel.

Very informative! I'm having a lot of success using Paula's Choice 10% Niacinamide booster and Differin for treating my dark spots. Unfortunately, Vitamin C didn't help me. I've been trying to figure out if I need an AHA or BHA in my routine. Does the Differin encourage enough exfoliation on its own? Thanks so much and thanks for sharing about the Paula's choice niacinamide. I like that brand as it seems fairly affordable. I don't know why everyone is telling everyone and their mother they "need an AHA/BHA," lol! Yeah differin is a keratolytic so adding more just increases the chances of a) inactivating other active ingredients in the things you are already using that seem to be working, b) increasing irritation and worsening acne/rosacea/redness c) increasing photosensitivity, d) worsening or contributing to post-inflammatory hyperpigmentation, e) complicating your life f) draining your wallet g) robbing you of time away from the things that actually bring you joy in life. ;)
Dr Dray it's crazy all the things companies try to convince us we need! LOL. I did see a lot of benefit from Salycylic Acid before starting Differin but honestly the Differin and Niacinamide have completely taken care of my acne. I've tried to include AHA in some way or another thinking it would help with my discoloration and it hasn't done anything for that. Thanks for saving me some more money:-)

ammra hellohi1 I used it it does not get darker at first after it lighter don’t keep continue to use it. Take a break for three months then use it. If you see it get dark stop use it. The long term dark spot get darker so I stop using it. The fresh new dark spot fading fast.

Hi there, the first thing you can do is to use sunscreen to prevent further worsening of your melasma. Try cerave pm and a retinoid in the evening and see if it works. Kind regards!

Hi Dr. Dray, thanks for another great Q&A. I tried the Neutrogena Ultra Gentle Hydrating Cleanser Creamy Formula, and I really like it. I was trying out the CeraVe Renewing SA cleanser prior, but I was getting some redness, sensitivity and dry patches from it (could have been over using it, so instead I'm going to give it a shot on some KP areas). The Neutrogena wash had a really soothing effect after using it on my skin. It does have that Cetaphil cleanser look and feel, but I seem to like the Neutrogena more - feels like it cleanses better I guess. Hi Doctor....Your video is very interesting :) I have a question. I have Melasma, I know about hydroquinone. What you say about it is so true but my question is....why the dermatologist don't explain to the patient that this ingredient helps for a little while but after all, the Melasma will come back even more strong? I would love that a doctor can tell me this, directly. I prefer to know it has not cure that believe that I can do something with this kind of ingredients that on top of that, has many side effects. No good at all.

I have very stubborn melasma and light areas too. Recommend Estee Lauder Camoflauge make up. I mixed it with tinted sunscreen. It really covers for about 6 to 8 hours. Nothing stays forever. I am adding the soy and fern. I already switch back and forth from hydroquoaine to vitamin c with Rodan Fields skincare. (take breaks). I use 30 spf and put bullfrog on if I hike, plus reapply sunscreen. Melasma is nasty stuff. The light areas too (from removing acne scars) areas on my face don't help much. It is maddening. Tried laser and it did nothing. 5 treatments from a reputable dermatologist and paid more than 2.5K. My point - thank you so much for your no nonsense clear layman's terms video on this subject. The amount of babbling 20 minute videos with bull$--- out there drives me off youtube from time to time. Other times, doctor dive deep deep into what causes it, but offer NO solutions but "sunscreen". Thank you thank you! Merry (early) Xmas Dr. Dray!

@@ghostofroom9384 I didn't over use ut and it ws on my legs.which were covered so I didn't uses sun cream. I was told that certain people can have this reaction I just so happen to be one of them. I only used it 3 times thinly on my legs. But thanks for the suggestion and reading my comment

In the 90's i went through a very stressful and traumatic situation. I noticed soon after that when I went out into the sun, mostly in Summer, any skin exposed would break out in hives, including my face. So I guess the stress opened me up to a sun allergy. Before this happened I used Ambi sometimes. But then Ambi started giving me hives too. Do you know what happened? And what can I use besides hydroquinone to lighten dark spots that won't give me hives? What would be the best product for me of the ones you mentioned? Thank you in advance. 😊

Thanks, this was really informative. I've tried the Timeless Vitamin C serum, and a similar one by Skin Deva, but no lightening of age spots. I've also tried a product from Paula's Choice with licorice extract, and alpha arbutin from The Ordinary, and neither lightened my spots. I did have a little success with a drugstore hydroquinone. but I'm concerned about those dark spots you mentioned, that are permanent. Is there a way to avoid those? Thanks again for the video!

Thanks! Unfortunately, all the drugstore versions of hydroquinone have fragrance. Paula's Choice has two options without fragrance, but they have either BHA or AHA in them.
So glad you made this video I spent over a thousand dollars on laser treatments for acne scars and saw no improvement.. i actually think my skin is worst... I have now found a new dermatologist that prescribed triluma but was unsure about purchasing because it’s so expensive and not covered by insurance.. I’m so glad you mentioned it because now I feel more comfortable trying it out.. do you know of any coupon sites for triluma?

cassandra dixon I used alovera for scar works well but not for dark spot once you put on your face it very sensitive to the sun will make your face darker happened to me and my man.
Have you considered doing a video on acanthosis? If you have, would you remind me of the video. I don't recall seeing it. I know it's an endocrine and metabolic (or genetic, malignancy...) issue, but so many who have it think that scrubbing the areas will make things better. Would products (retinoids, topical vitamin D analogs) be helpful? It would be a great video to address the underlying issues and what one can do at home in addition to lifestyle mods. Thank you!

I have a patch on my face which was smaller. I have been using the Lancôme Genifique serum, and Lancôme visionaire moisturiser for multi coreection of skin. My spot has got lighter. It's getting larger though . I see small patches appearing in different spots around the Area too. I do not have acne. Will it help me if I used the Differin gel.

Very informative! I'm having a lot of success using Paula's Choice 10% Niacinamide booster and Differin for treating my dark spots. Unfortunately, Vitamin C didn't help me. I've been trying to figure out if I need an AHA or BHA in my routine. Does the Differin encourage enough exfoliation on its own? I haven't had an acne breakout in months so I don't necessarily need a BHA for that. Thanks so much for all the wonderful advice you give us! Hope you have a great weekend!!!
Thanks so much and thanks for sharing about the Paula's choice niacinamide. I like that brand as it seems fairly affordable. I don't know why everyone is telling everyone and their mother they "need an AHA/BHA," lol! Yeah differin is a keratolytic so adding more just increases the chances of a) inactivating other active ingredients in the things you are already using that seem to be working, b) increasing irritation and worsening acne/rosacea/redness c) increasing photosensitivity, d) worsening or contributing to post-inflammatory hyperpigmentation, e) complicating your life f) draining your wallet g) robbing you of time away from the things that actually bring you joy in life. ;)
Dr Dray it's crazy all the things companies try to convince us we need! LOL. I did see a lot of benefit from Salycylic Acid before starting Differin but honestly the Differin and Niacinamide have completely taken care of my acne. I've tried to include AHA in some way or another thinking it would help with my discoloration and it hasn't done anything for that. Thanks for saving me some more money:-)

ammra hellohi1 I used it it does not get darker at first after it lighter don’t keep continue to use it. Take a break for three months then use it. If you see it get dark stop use it. The long term dark spot get darker so I stop using it. The fresh new dark spot fading fast.

You talk in this video about hyperpigmentation and I wonder what sort of treatment is advised for hypopigmentation, like the light scars that are left after cystic acne and never seem to fade.
