Excellent question. I would still have the access from the interior. Yes, it matters a lot from an energy point of view. My crawlspace is part of my heating and cooling space. the thermal boundary (insulation) is at the dirt level, not the joists. If you were building different, with insulation in the joists, then having a door through the foundation wall would be more efficient. I hope that helps, let me know if I'm unclear.
