    This is me!My cholesterol is off the chart, however my triglycerides are non existent.Doctor was very concerned with my cholesterol, also said it was the lowest triglycerides she has ever seen.Im 67 year old male, carnivore.

      Very skinny, very low triglycerides in the 50s, very high HDL of 112 or more, LDL of 150 or more and consistent over time. Seems to be high HDL in both parents and no heart disease. Dad died at almost 91, Mom is very healthy at 86. She was told last year that they had never seen such a high HDL in an elderly person. Hers was 120 and she is quite thin. She does not restrict carbs.

    I gave up on tracking cholesterol about 4 years ago when I went low carb. I've never felt better and I'm never going back. Thanks for all of your coverage of the latest science!

    Perfect timing!
    My husband had blood drawn recently. He had switched to a carnivore diet after being on a low-carb, zero processed food diet for 7 years.
    His triglycerides had cut by a 3rd from 90 to 60 his ldl went up from 60 to 90. His doctor panicked and wanted him on statins, despite the fact his overall cholesterol was 190...I told the dude no thanks 😂

      @@jimg6970 It's not the number itself, but the fact his LDL increased by 50% from his previous number that most likely caused his doctor to panic.

      90 LDL is low. Maybe there was a third digit missing. I wish mine was that low.

      He went from unhealthy to healthy. Low LDL is correlated with illness.

    Same thing happened, went low carb, LDL went up 180, triglycerides down to 50, HDL up to 70, total cholesterol 280, lean athletic feeling great. Before this my total cholesterol was always about 180, and I didn't feel as good as I do now. I'll keep my higher healthy cholesterol !

    My doctor attempted to push me on statins because of high cholestrol...despite my being a very lean, muscular, athletic 70 old...I declined...citing my triglyceride to HDL ratio of 0.5, i.e., indicating very low inflammation etc. None of the attending doctors seemed to have any knowledge or, at least, appreciation for this key marker of heart health and (low) inflammation. And when I asked for the breakdown of the lipid panel as to type of LDL I was told, 'Oh, we don't do that'.

      What's you apoB--if that's high, you've got problems. HDL is not causally related to heart disease (they figured that out over a decade ago, so TG to HDL levels don't make sense).

      @@karlwheatley1244 If LDL is high then APOB is high, and yeah, when there are more small particles then APOB will be relatively higher. The TG to HDL ration is a good indirect indicator for insuline sensitivity (but do your own research on this ratio). In practice people with CVD tend to have high triglycerides and low HDL.

In western countries such as America, almost everyone has early signs of developing heart disease by age 8, but most men have CAC scores of zero until their 40s and most women into their 60s.
      People can have lots and lots of soft plaque in their arteries and still have CAC scores of zero.

      Same here....but I was already aware that I'm a lean mass hyper-responder, and this would be normal and healthy for me. I do my own research, thank goodness.

      I’m in this boat, TC: 300+, TG: 73, HDL: 80+, LDL: 200+, CAC: 0.
      Low carb, competitive Oly weightlifter, weekly zone 2, consistent weekly rower. VO2 Max 40+, 50yrs
      I added 100g carbs and saw a reduction in LDL, lower LDL when added 150-200g carbs.
      I feel better on low carb, high protein and higher fat.

      @@TomHaile Sounds fantastic what you are doing! I have added in some carbs and lowered LDL too. I do all the same things you do, exercise wise and that is what counts really in the long run.

      @@joybecker2335 I added carbs (sweet potatoes and butternut squash) and tested cholesterol using a home kit week to week and more carbs resulted in lower LDL. I tested 150g a day to 200g.

    I searched for this -- because I exactly experienced the same thing. I started weight training about 4 months back (5 days a week). HDL improved from 39 to 48. Triglycerides from 137 to 118. But LDL increased from 120 to 157. My BMI improved from 25 to 22.5. I'm on low-carb, protein rich diet. Drastic increase in LDL in 4 months was concerning to me -- so I started doing some research and stumbled upon this video.

    I know 2 co-workers. Super skinny, both from China, eating very healthy. They both have super high cholesterol levels. Based on their situation, I start to think that cholesterol is not necessarily a bad thing. The high cholesterol is probably there to compensate their low body fat in order for their body to function properly.

    I was just looking for videos like this, had some labs yesterday that showed some high cholesterol, even though I'm very lean right now. I've been under eating from being lazy with cooking, and most of what I've been eating in the days before my lab was meat/eggs/dairy. Was encouraged to do a "Heart Healthy Diet", but was fairly familiar with the fact that LDL isn't a great indicator in cases like mine.
    I'll keep up with what I'm doing, and if my doctor bothers me about it, I'll try putting forward this study.

    So basically if you are fat adapted and using fat for fuel, LDL will increase as required by the body, and as in the Havard study you could drop this LDL back down by eating sugary biscuits OR statins. So this LDL stuff looks like a nothing burger just to flog statins.

      BUT, the increase only occurred in people who were on a no/low carb diet. One of the likely reasons is there was very likely an increase in the consumption of animal protein to compensate for calories/energy. BTW, I’m a fan of animal protein. However, I understand the importance of carbs.

      @@RBzee112 Healthy lifestyles should include some form of cardio for cardiovascular health. Our bodies require glycogen in our muscles for energy to use when we burn through glucose during cardio. Carbs are the best source. Just consuming protein or fat doesn’t work. 90% of people who subscribed to zero or very low carbs struggle with energy levels after about 3-4 weeks with no carbs. This impacts gym performance and some people have hair loss or other health issues. Also, most of the initial weight loss success is from the loss of water weight. Glycogen stores water in the muscles, which is needed for peak performance. The goal for weight loss should be fat, not water. The zero carb crowd doesn’t understand that.

    I LITERALLY had this discussion about my lab LDL bloodwork this year vs last year at my physical this morning!! I said I was eating less carbs, more meat, and running/exercise more (but hardly excessively.) All my ratios are fantastic as well. She was like can’t you eat less meat? to which I responded - I’m not doing that. My hdl, triglycerides, etc were perfect. No genetic predispositions to coronary diseases etc either.

    When I went low carb, triglycerides went way, way down and HDL went up. Total cholesterol is a bit high so am pondering how to handle the PCP. She thinks it is too early for statins, thank goodness.

      There is no good reason to take statins. The drug companies quote *relative* risk reduction (of heart attack etc) and not *absolute* risk reduction. So, for eg, if my risk of having a heart attack is 2% before taking a statin, and 1% after taking a statin, the drug companies will say that my risk of having a heart attack has been reduced by 50%. In reality, it has been reduced by 1%. The absolute risk reduction figures are miniscule . Also, statins are implicated in many negative health outcomes - diabetes to name just one.

    This happened to me when I had my values drawn 2 years ago. I am 6’1 with 19BMI. LDL higher than ‘normal’, but Trigs and vLDL was low and HDL was high. Funny, I was already going in for a blood draw tomorrow and I expect the same. Glad I caught this video.

      ​@@amfohrit is each person's individual body that functions differently. We are not all the same cut with a cookie cutter pattern and doctors should not treat us as such. Oreos can kill you yet make you have good readings on doctors tests. Does that make sense?

    This confirms my since long time theory that these lipids is just a fat/energy transportation system. With less glucose in the blood stream you need to transport more triglycerides and cholesterol so the LDL is needed for that. If you have plenty of fat deposits you will start to burn off that fat turning it in to ketones so you would expect less increase in LDL. Pure logic!

    I was pretty worried about the VLDL. I got bloodwork done when I was doing True OMAD diet with only meat and veggies like kale and avocado. I had been keto for about 4 months. Started out not fasting, went from 16/8 down to 2 meals a day, eventually ended up doing 1 meal. I was around 10%bf. Pretty lean and very muscular. My HDL was low, LDL was high, triglyceride were high and VLDL was also very high. So was my cholesterol particularly count. I had no answers for that.

    I've never put much value in the BMI chart because it's just height and weight. Muscle is heavier than fat per amount. Two people can have a weight of 150 lbs and height of 5' 5", but one be fat and one be lean. Am I missing another chart or way of measuring that takes this into account? I'm 20 BMI, but I don't believe that reflects my health.

    You can have a bmi of more than 25 while being lean if you possess a lot of muscle, many people do.

      I’m 5’4, and weigh 162. I’m all muscle. My waist is 27 inches. My Dr wants me to lose 17 pounds. It’s ridiculous!! I use to be 145 and everyone use to ask me if I was OK. I looked sickly!!

    This is also what anabolics that have fat burning properties, like Anavar and Masteron also increase LDL. Many BBs freak out about their LDL while taking these.

    Of course we need to figure this anomaly out. It’s obvious to me that it is due to a dietary difference. What kind of fats and proteins are the high LDL group consuming? Are they no carb at all?, are they low carb but low fibre carb? Are they nutrient timing differently?

    My TC is 222, and LDL-C is 146..I am not worried about my TC and LDL, because what really predicts heart disease is the triglyceride and HDL levels and their ratio--which indirectly measures your level of insulin resistance or risk of heart attack or stroke.
    Despite my elevated TC and LDL-C, my Triglycerides are 66 and my HDL is 64. So my TG/HDL ratio is a very heart healthy 1.03, which signals insulin sensitivity or no insulin resistance. Thats why I dont care how high my TC or LDL levels are, your TG/HDL ratio counts the most and the best way the predict heart disease. Have a great day all. 😊

    BMI 16, as LMHR, what I do to make my primary doctor happy is to eat more Quinoa for 2 weeks before my blood work, and this decreases my LDL to a less alarming range (still elevated though) 😂

    My total c is 349/ ldl 281/ HDL 47/ triglycerides 51. 2 years ago it was 203/143/56/98x glucose down to 80. Was 105. A1c 4.9 was 5.1. 8 weeks on carnivore. Down 30 lbs. bp normalized . Skin disorders cleared up

    The problem with the study is that severe exercise will raise LDL especially if your exercise is in the top 5 per cent and severe low carb even at bmi 31.

    Ugh, it seems obvious. When your using fat for fuel, your body needs more means of mobilizing it, more LDL. When your taking in high exogenous cholesterol and high carb you get the toxic, unnatural, mix. Our bodies were built experiencing seasons, unmolested fruits and minimal unprocessed grains, with fruit and feasting in the fall and summer and copious meat, fat and fasting in the winter and spring. We're trying to do it all all the time.

    My question is whether the keto diet itself might not be solely responsible for the spike in cholesterol levels that some people experience when they begin this diet. It's possible that the increase in LDL cholesterol is more attributed to the higher intake of saturated fats, which often accompanies a keto diet. By replacing some of these saturated fats with other options like olive oil and including fiber-rich foods such as avocados, you can potentially mitigate this rise in LDL cholesterol.

    Thank you so much for presenting this study. I might be LMHR with LDL of 223, trig 88, and HDL 61? My doctor wants me to consider changing my diet I eat meat, eggs, dairy, fruits and honey. That’s like 90% what I eat and my testosterone increased 250ng/dL too. I think the cholesterol increase might be correlated to the testosterone?

    Yup, my LDL is 250 when doing keto. I have 10-14% BF and 20.5 BMI. Im a bit worry, but decide to not take statin.
    We need more studies about this. CMIIW, as I know people who live long (80++ yo), tend to have high cholesterol.
    So maybe Cholesterol indicate protective mechanism in niece case???

    Does AST/ALT, high ferritin combo give anyone pause at all?
    Seems like she is dealing with signs and symptoms of systemic inflammation, potentially fat deposits in liver?
    Also TG is low but optimal imo is below 70

    I'm lean but don't fit LMHR model because my LDL is high(164) but my HDL is low(33). Also Tri-G's, A1C and glucose are optimal. So, I'm a little perplexed.

      Work with a metabolic approach functional med physician to get specific details. Low HDL is associated with systemic inflammation. It makes sense if you're still on high carb diet or if you are low on omega-3 lipids and may be eating foods that have high omega-6 oils such as processed foods.

      I agree. You should look at the quality of fats in your diet and make sure you're avoiding seed/soy oils and getting enough omega-3s.

    Im definitely not lean - BMI 30.
    3 months keto, 2 months carnivore. Ldl 259, Hdl 46,Trig 38.
    Ldl before keto - 146. And i used to eat a lot of oreos before keto 😊 so they definitely will not help.

    Your graph suggests the fatter people’s livers are just pumping out so much cholesterol they start maxing out. Thinner people’s rise more to start approaching the same max out levels. That doesn’t sound good. Low carb looks possibly disastrous.

      If you're really concerned about high LDL cholesterol levels, it's important to look at your intake of saturated fats and fiber in a low-carb diet. Consuming too many saturated fats and not enough fiber can adversely affect your LDL levels. Switching to other fats like olive oil and including fiber-rich foods such as avocados will lower your LDL cholesterol.

    The point of interest that he doesn't get to until the end is that it's small, dense LDL, which the liver doesn't recycle, that are dangerous. Saturation of the LDL receptors in the liver, reducing LDL clearance from the blood, is the danger. LDL that hangs around in the blood too long and gets glycated and/or oxidized gets readily absorbed by macrophages, which is a problem when those macrophages are in or under the vascular epithelium in a chain reaction of escalating inflammation/immune response. How to check for small, dense LDL? Somewhat helpful proxies for measuring small, dense LDL level: triglyceride level, apo-B level, remnant cholesterol level.

    When I look at that female patient's blood panel, the most worrying is a high ferritin level. Possibly as a result of eating a lot of meat. I wonder if that was being treated. High ferritin levels are associated with increased mortality. Treatment is easy: periodic blood donations will solve the issue.

    I am on a low carb diet. My triglycerides is 40mg/dl, hdl: 71 mg/dl, LDL 198 mg/dl and total cholesterol 278 mg/dl. My fasting glucose in 98mg/dl but my hba1c is only 5.1 percent. My urea nitrogen in the blood is slightly elevated at 26 mg/dl(cut off of the lab was 20) - too much protein load?, serum creatinine: 1.2 mg/dl. Is this all right? My non low carb friends freaked out when they say my lab results. I exercise 3x a week.

      could be the protein, i would cut down on protein and stop creatine supplement (if consuming) and repeat test in about a week. if its normal than all is ok and i would go reintroduce creatine and up my protein intake to what it was before

    I had only high ldl and had a heart attack. All my other numbers are good. Two arteries are clogged. I do not have any other problems. Blood sugar is good, triglyceridebis good, weight is excellent...i have low body fat. I do not understand it. Hdl is good.

    Have little high cholesterol with total at 220 and hdl 110. BMI at about 20. Lots of exercise. Eat mainly fish and vegetables. But vegetables high with carbs at 35% total - so not following theory exactly mentioned here that low carbs pushing cholesterol high. Maybe higher cholesterol more related to low BMI then low carbs but just here N of 1 so not same for all.

    I have just received my cholesterol blood tests results. Total cholesterol = 6.7 mmol/L. HDL cholesterol =1.6 mmol/L. Non-HDL cholesterol = 5.1 mmol/L. TC HDL ratio =1.6 mmol/L. Triglycerides = 1.3 mmol/L. LDL cholesterol: missed marker and unrecorded. I have asked for a copy of the specific blood marker chart and analysis, who actually did the test, and where was it conducted, and I am still waiting for the evidence as they now want me on statin prescribed medication? Laughable!

    I really like your content, however, humans adapted to eat carbohydrates to survive, and while I agree we eat too many carbs, lean mass hyper-responders are ridiculously obsessed with using ketones constantly, being lean and never eating carbs. There are several people I know who are extremely lean, they excercise like crazy and don’t have diabetes but they have cancer (and they are generally miserable and NO fun!). Humans became omnivorous for a reason! To survive, to thrive! Being black and white and obsessed about diet to extremes is counter-productive! Low body fat is nice for wearing a nice dress, but it’s not necessary not to have any fun!! 😅😂 (or a sense of humour!)

      For one, you never have the same ldl particle number (it's not colesterol you messure but LDL particles in the blood) many don't understand this! This can vary greatly infact. The bodycells don't need the same amount of colesterol at any time. So this will vary quite a lot. So to even messure this i quite stupid! It's 100% a medicin to make money thing! And that's a fact

    I’m somewhat confused, are we saying LDL cholesterol is tied to low body fat or low carb diet? Low body fat and low carb diets are not related. Yes, there people who choose to eat less or no carbs and lose weight. However, all they are doing is removing from their diet what they were over consuming or eating to much causing fat. People can lose weight consuming nothing but carbs. It’s calories in vs calories out. People who go to a low/no carb diet often increase their animal protein intake, which can create an issue with cholesterol. No/low carb diets cause most, not all, people to lose energy and they struggle with cardio and other fitness activities. Eat a well balanced diet, with high volume and low calories and you’ll be fine. All these fad diets create issues and aren’t maintainable by the majority over time. Data shows the majority of no/low carb diets result in people gaining weight back and then some over time.

    As time passes, I am beginning to see LDL as a good guy in the heart disease court room. For myself I have already pardoned LDL. I realize not everyone agrees but nonetheless I view LDL cholesterol in the human body as the maintenance guy fixing this and fixing that like a handyman around the house. He was convicted of a murder he didn't commit, while his accusers lined their pockets with money. I don't think Mother Nature made a mistake.
