Epidermoid Cyst Removal INTACT| Auburn Medical Group

  • Removal of an epidermoid cyst without rupturing it is done by Dr. Vaughan to relieve a patient of continuing episodes of draining from the cyst in his back. These are often inaccurately called sebaceous cysts by many doctors. Many RUclips doctors will squeeze out the sebaceous material during removal. This can sometimes allow a large cyst to be removed through a smaller excision, resulting in less scarring. It is often more difficult to completely remove the cyst capsule if it is ruptured during removal. We had the convenience of working on this patient's back where scarring was not as much of a concern.

    Great job doc! This is the best way to remove a cyst. You avoid the cyst from coming back, no cyst debris, just perfect. Congrats on a job well done. You made my day!!!

    Hey from England ! That was fantastic. I'm a fan of dr lee aka dr pimple popper too, sometimes they rupture by accident sometimes they have to be ruptured to leave a smaller scar as possible . That was nice to see dr Vaughn ! Encore ! Xx

      I agree about them popping regardless of technique. I am new to this channel, one of Dr. Lee's popoholics. I need to defend her for a moment. She does try to excise cyst whole. There are many occasions that the cysts are just too big to be excised intact. I absolutely love it when she pulls a ginormous cyst sac out of a little bitty hole. She does debulk the really large ones first. She does pull some out intact and that is cool too. There was one gentleman who had a tattoo. She did her best to not damage the tattoo. She did debulk the cyst, but then she just kept working on it and this enormous cyst sac finally appeared. She also had an arm cyst do the same. The sacs were probably a good 12cm and 8cm, respectively. Now, her husband, Dr. Rebish, has a very different approach than she does. So, the mural is to each his own. Different medical schools teach different techniques. So, whichever one the doctor is comfortable with then that's best.

    Nice to see an intact sac removed! I appreciate the medical aspect more than a pop for the sake of popping. If it pops in the process, so be it. Beautifully done.

    Was surprised at how round (and intact) that cyst was...especially since it had been squeezed and or inflamed before. Great job on getting it out whole. I think there's room here on RUclips for all of you Drs who remove cysts...I know there's room as far as I'm concerned. And whilst techniques vary, it certainly doesn't mean one is more correct than the other. Thanks for sharing Dr. V!

    Dr. Sandra Lee only ruptures the cyst purposely because she is trying to minimize the length of the scar. And it's not Hank Williams Jr.; it's Waylon Jennings.

      Becky Ray of have to disagree with you, she used to, in the early days, take them out whole, but as she got more of a following, she started deliberately opening them and squeezing them. You can tell it's an audience entertainment thing, as she often does a slow motion replay of the contents coming out and she never does whole removals anymore. Well except Pilar cysts, but even those she's tending to squeeze more now.

    First and foremost: EXCELLENT camera work! I don't know if it's the kind of camera you have, the person who's operating it or a combo of both (what I think it is!) This was so sharp and clear...kudos! Second: while I am a popaholic, my OCD lives that you got this cyst out intact! And, still attached to the skin! Amazing to watch! I really enjoyed your clear descriptions step by step. I find this so fascinating to learn about! You explained it all in terms I understood and made the whole video that much better! The camera used for most of the close work is the iPhone 6 using the Filmic Pro app. It still surprises people how much professional video work is currently being captured with mobile phones.     A lot of times, I don't think Dr. Lee ruptures cysts for the wow factor. I think she just tries to get the cyst out of the smallest incision possible.

      +mindy573 I agree. It is a good technique when the cyst is located in cosmetically sensitive parts of the body and the operator removes all of the capsule.

      mindy573 given she rarely ever removes then whole anymore, I think she actually does rupture them for the wow factor these days, she's even started rupturing Pilar cysts.

      Dr. Sandra Lee has many different clients that do not want scares on their body for self-confident reasons, in many of her recent videos you can see her taking out the whole thing intact. Especially on many older clients such as senior citizens who longer no longer care about the scars. Do not make assumptions that someone else’s technique just by watching a few of their videos... she may sometimes do it for the wow factor (when the client wants it as little scar as possible) but that’s usually towards the end when she pops this is when she pulls it out hole.. so maybe be kind to mindy573. I really did enjoy your video I didn’t like the comments I’m reading. I think you did a fantastic job... i’m not a medical physician of any kind. Lol

    Several points on this one: His back skin was THICK! I've watched so many of these excisions and I've never seen anyone with that thickness of skin, even on their backs. lol It's always satisfying to watch all the contents get squeezed out, but this was just as satisfying in it's own way. It was like a beautiful pearl. And yes, that last stitch you put in was going to bother my sense of symmetry as well if you had not put it in. lol Loved that this procedure was a longer one with a follow up the next day. :)

    I've seen several other physicians do this exact same procedure but WOWOOWWW!!! I have NEVER EVER seen it done this way where the skin and entire sack, including the surrounding areas have been removed!! I literally couldn''t take my eyes off the screen!! KUDUS Dr. V!!! I only wished you were local as I have a horrible lipoma that's started giving me trouble lately and my doctor wants it out! I don't know of/trust anyone local. IF you know any good physicians who can help me in Palm Beach County, FL, I'd love to hear back from you! Again, AMAZING JOB!

    Correction, Dr. Pimple popper does not squeeze out the contents just for us viewers. She often does this in order to remove the entire sac from a small incision, which produces a smaller scar. Just fyi! *^_^*

      Elisha Barrows, you could go for your RN. I worked in surgery for 16 yrs. Or you could be a scrub tech. They start out pretty good. You won't make as much as a nurse, though. There are plenty of jobs in the medical field.

    Nicely done! It's extremely satisfying to see the entire sac removed and even intact. I thought the missing stitch was because the dissolvable suture was used...no? Sure would've like to see that cut in half though (but I understand).

    Great job getting that out whole, I thought it would have been in a lot of scar tissue, you struck lucky ! I notice another scar next to it too. I wonder , if the skin on the back is under so much tension, why you may not have undermined the skin before putting in the deep suture ? Also, I don't think the " missing stitch" was a mattress stitch, I think it may have been a white one, as you used that to finish off when you ran out of the dark suture .

    Is there a reason why the skin on top was patchy and dark or was that just a fluke?

      It was from trauma to that area because the skin was pushed outward by the cyst, causing it to rub against things.

      Yes. Cutting parallel with Langer's lines allows for less tension on the wound to allow healing with less scarring.

    I noticed you did not remove all the scar tissue from past ruptures, what is your reason for leaving some behind?

      We removed everything associated with this cyst. Other lesions would be done as a separate procedure.

    Sir, This is the first time watching your videos and keep doing what you are doing! I agree with you that taking a cyst out whole is just as if not more satisfying than popping it!! keep them coming and I hope i will find at least one video where you take black heads out of the inside of peoples ears! and solar comedones out of the wrinkles in guys foreheads :) my two favorite pops

    Im thinking the reason the skin is so thick on the back is it is to protect the spine. Much like the ribs protect the heart?

      Peggy Helton possibly an evolutionary hold over from when we walked like chimps. That layer of thick skin would probably be a great insulator, and yes protect the back as it likely would take a lot of any injuries from fights etc.

      It also depends on how "thick" we are. People that are overweight have a thicker layer of subq.
