With many muscle tests you move the patient to a gravity neutral position for Grade 2 testing, in the reference used (Daniel's and Worthingham's Muscle Testing) for this muscle test they chose to have the clinician support the weight of the arm for the patient which does the same thing as moving it to a gravity neutral position. We hope that is helpful and thanks for watching our videos.

Isn't shoulder adducted instead of abducted?

There is a difference between shoulder adduction and shoulder horizontal adduction. Horizontal adduction involves moving the arm across in front of the body. To be able to perform this movement the shoulder must start from an abducted position. The angle of starting abduction helps determine which fibers of the pectoralis major muscle are emphasized. I hope that is helpful, and thank you for watching.
