    Hello Doctor. I am 63 & had a stroke March of 2022. My pain is controlled by Cymbalta (and my mood). The pain is better, BUT now I have SUCH STIFFNESS (aka spasticity?). I don't remember the spasticity before taking Cymbalta...Could that be a side effect of the drug? I have tried to get off the Cymbalta twice, each time the PAIN comes hack with a VENGENCE. My pain mgmt nurse keeps switching muscles relaxers, NONE have worked. Nothing seems to help. I am so confused 😔 I would appreciate ANY suggestions. Enjoyed this video (1st). Liked/subscribed.

Maybe do a case review that would be of educational value for medical students, GPS and nurses. Can you do a video about using cymbalta for anxiety & depression while also using methadone. My doc knows I'm on the methadone program and recently prescribed me cymbalta but I don't feel right. I feel really fatigued, foggy head, almost feel like its making my methadone not work as well. Its really weird and I dont like it. My doc is not knowledgeable so please don't say "ask your doc" i want to hear your take on it. I have already stopped the cymbalta because I dont feel its safe for me to continue but I really would like to hear from anyone who has knowledge. Btw I was only on it for 5 days at 30mg before I stopped otherwise I'd of had my doc help me stop.

    • @ShrinksInSneakers
      @ShrinksInSneakers  3 года назад +4

      Sure, we need to think about drug interactions. Methadone and Cymbalta do interact, and methadone can increase blood levels of duloxetine when the medications are combined. This may be the cause of your adverse side effects, although you were on a low dose, it may have been much higher due to the interaction, and this is because duloxetine is metabolized by CYP2D6 which methadone is a substrate for and may inhibit as well. Basically, you had higher serum levels of duloxetine due to the interaction worsening the side effects. I would have started with one of the more traditional SSRI like Lexapro in this case, the side effects are mild, and it works well for depression and comorbid anxiety. It still has some risk with methadone but much lower than the risk with Cymbalta. Hope this helps clear things up, appreciate the comment

  • @dfgdfg_
    @dfgdfg_ 2 года назад +1

    Thank you for this. Based on the half life, for GAD, would you suggest 30mg once a day or 20mg twice a day?
    Can it be prescribed alongside bupropion to help offset the sleepiness?

    • @ShrinksInSneakers
      @ShrinksInSneakers  2 года назад +2

      Based on the half life you would split the dose. Although many people dose it once per day and I have not seen a significant differnace in the effect either way. There is some risk of increased blood levels and more side effects when these two medications are combined. Bupropion is a 2D6 inhibitor and cymbalta is metabolized by 2D6. It can be used but dose adjustment is needed and you need to watch for increased risk of seizure as well. Hope this helps, if you haven't subscribed please do and spread the word about what we are doing here.

    Hi my Doctor started me on this 2 days ago. I've spent the 1st two days in bed completely exhausted....is this within normal boundaries? I'm not very keen on Antidepressants but im at the point where I need to try. I'm doing 30 mgs a day for a week then 60 mgs after a week.

    • @ShrinksInSneakers
      @ShrinksInSneakers  2 года назад +3

      This is not medical advice you should always talk to your doctor, this is general information for educational purposes only. Sedation is a potential side effect, the way I usually get around this is to have my patients take the medication at bedtime when feeling tired is a goo thing. Many people acclimate to the medication over the course of 4-6 weeks but I would talk with your doctor if it persists or interfere significantly with things like driving. My Dr just prescribed this for me 30mg but I also take a pain killer prescribed by same Dr..not methadone but is a controlled medicine....is this safe? Dr says of course it's safe, pharmacist said be careful about the cns affects of both meds but I don't see anything in my own research that says it has an interaction on the cns like a morphine or similar drug might have. Can you offer insight on this? I woukd be taking it for pain management and anxiety and depression caused from the pain and discomfort I deal with.

    • @ShrinksInSneakers
      @ShrinksInSneakers  2 года назад +2

      It really depends on the pain medication being used. The CNS depressant effects comment will come up as an interaction when combining these classes of medication but it’s hard to say if it’s relevant or not unless you check it. • @ShrinksInSneakers

      Withdrawal is known to be worse for venlafaxine specifically. I do not think an exact mechanism has been discovered for these withdrawal symptoms as of yet. I try to use extended release or avoid venlafaxine if possible because of this and the increased risk of death in overdose.

I know it's alot but it's worked for appox 20 years. I was falling apart before I got on adderall 25 xr mg 1 daily, 1 adderall 25 ml. Short acting, 1 70 mg vyvance, daily, (Cymbalta 2 60 mg & pantroprazole SOD EC TB 40 MG X 2 DAILY (upset stomach everyday) & 40mg for the undifferentiated connective tissue) lamotrigine 25MG, 3X DAILY. compounded testosterone and estrogen for well being. A tiny bit of trazadone for sleep about 5 mg from 50mg broken tablet. Everything keeps me functioning except I'm postmenopausal and I think adderall doesn't work as well these days. Med.s go in and out of my body quickly. I know it's alot, I'm balanced for about 20 yrs. I would like to try to go off some of my rx. But I know I would go back to very disorganized mentally and physically. Have any idea of how to do this without pain and agony? I don't want to be disorganized again. I know of course I could talk with my dr.s. I know I need a Dr's help. It seems too hard for me to just go down in meds by my self. Thank you.
