    What would happen to a patients eye if they decided not to have further DMEK surgery after having full thickness graft plus 6 DMEK surgeries on R eye, 2 full thickness on L. Do you know if there is any connection of cornea rejection and Sarcoidosis.

      As every case is specific, there is no answer simple answer here.
      Prolonged swelling of the cornea will lead to loss of the eye.
      The questions to ask include, why was a full transplant done, why did it fail, and why did subsequent cell transplants fail?
      Something is going on...

      @@DrAnthonyMaloof Thank you for taking the time to answer my question and for your honesty regarding eye loss.
      Original full thickness transplants were completed due to Kerecotonous. Over 20 years ago. In 2016 I started having other health issues was tested for MS which was negative. Was told problems due to old back injury. My right cornea rejected and first DMEK was done in 2016. I had mild swelling in left eyelid which i was told was cellulite. Eyelid eventually became so swollen was basically looking through a small slit November 2020 part left eyelid removed and biopsied, results showed sarcoidosis. PET Scan in 3/2021 showed multi organ Sarcoidosis. From 2016 to 12/2022 six DMEKs compketed. Oct 2022 both the left eye and right eyecrejected at the same time. Last DMEK on right eye done 12/2023 Full thickness graft completed left eye 3/2023, which now has a very thick cataract. Cannot see my hand 15cm away from that eye. Last stitch to come out of left eye graft @ 3/2024 cataract can be removed six weeks after that.
      In my mind it has something to do with the sarcoidosis but no specialist can confirm. Medication for Sarcoidosis us 25mg Methtrexate weekly, 40mg Humira fortnightly.

      Thanks for filling in the details. Sarcoidosis is a serious condition which is why you are on serious immune suppressant medications, and any form of transplant to restore vision will require continued immune suppression therapy. Close cooperation between transplant surgeon and immunologist is critical.
      Prolonged corneal swelling is very damaging to the eye and reduces the success of further surgery.
      This is a very testing problem. Rejection is why I rarely offer full transplantation to keratoconus patients…..If I cant perform a lamellar procedure, I suggest not operating.

    I had SMILE 11 months ago. I think there is still some haze in my right eye, can this be remedied? it causes glare.

      Unfortunately, the direct answer is no. Glare is not always due to haze but the optical change created by laser surgery.

    I use losartan eye drops, for corneal clouds in a some weeks. It’s a test.

    HI DOCTOR. HOPE YOU WILL BE FINE. I GOT CORNEA; SCARS IN MY RIGHT EYE AFTER transPRK AND MY THICKNESS IN THIS EYE IS 280. WHAT I HAVE TO DO NOW,? I ALSO GOT EYE FLOATERS IN RIGHT EYE AND LITTLE BIT FOGG IN IT. IM POSITIVE FOR HERPES TOO. DONT KNOW WHAT CAUSED THESE PROBLEBS EITHER HSV OR LASER SURGERY. PLEASE REPLY ME. LOCAL SURGEONS HAS GIVEN ME EYE DROPS AND MULTIVITIMIN AND SAID SCARS CANT BE FIX BY ANY SURGERY DUE TO THINESS IN RIGHT EYE.

    Hello Doctor, I scratched my left eye on 31/08/2024, I have corneal abrasion , my Cornea is healed now, I don’t feel pain anymore since 3 weeks ago. I consulted a doctor who has 32 years experience and he is a cornea consultant. Both doctors said I should use eye drops lubricant and eye gel. I used also eye drops which contain NaCl. I tried everything, both doctors said I need more time until the blurry vision goes away. One of the doctors said I might need another Lasik surgery , while the other one kept saying I just need 2 more weeks. I don’t think that the blurry vision will go away. By the way, I had 1 lasik surgery on my left eye in 2016. Please Advise me doctor.

      It is really hard to advise as it sounds like several things are going on.
      The rule of the cornea is give it time.
      The surface takes 3 to 6 months to settle.

      @@DrAnthonyMaloofThank you Doctor. At least now I know that it takes time. Today is 21/10/2024, my left eye still didn’t recover completely. The cornea healed, however I still can’t see clearly. The doctor advised me to wear glasses for two months from now. Then he said he will apply laser to my left eye to make the surface rough. Finally, after 2 months from now, he said that he can do the laser/lasik to correct the vision. Before the incident it was -0.25, now it is -1.5

    What kind of surgery do you need for a central haze and scar in one eye? Will that surgery cure you? Could I do a cataract surgery after treating a haze in cornea? Will I ever be able to stabilise my vision? I’m really desperate here, really distraught: the surgeon told me I had the haze and a scar on cornea when I came to discuss a cataract surgery. Now I’m waiting to see a corneal doctor and surgeon. I’m scared of what he’s gonna tell me. The first surgeon wrote in his report that scarring looks inactive and long-standing. Please give me some idea how serious it is. My eyesight started deteriorating last summer in that eye and I was told in December that it’s a cataract, but I could not have imagined I had something else there!!!

      It depends on the cause. Deep scarring can be reversed with stromal transplantation, replacing the structural layer with clear tissue. See this link ruclips.net/video/ZmLXvt4KcCM/видео.html

      @@DrAnthonyMaloofThe corneal doctor of 30 years experience said that the topography was acceptable and forget about the scar and the haze is natural to my eye. No treatment for cornea needed. So carry on with cataract surgery back to the original doctor who said I had scar and haze. And they both know each other. The first doctor is not a corneal specialist but now I’m worried to have an operation with him although he is a renowned Vitreoretinal Surgeon and does these operations all the time. Will see.

      @@DrAnthonyMaloof Dear, Doctor. Thank you for asking. I am still waiting for my cataract operation. I went to another clinic and did all tests, the optometrist told me that the scar is like deposits or some residue in the cornea, and said it may affect a little bit my vision: it’s like cataract affects 90% of the vision and the scar 10% but he is also positive that I don’t need to do any operation on my cornea. I may update later what happened.

    Is it normal that my eye with the recurrent corneal erosion is so blurry every time it attacks and when in pain? Erosion is like every other week, it becomes blurry then slowly my vision tends to improve very very slowly. Then erosion occurs again

      Thank you for your question. It's not uncommon for your vision to become blurry during a recurrent corneal erosion episode. The pain and blurriness typically accompany the erosion as the surface of the cornea is affected. After the initial discomfort, it's normal for vision to slowly improve as the cornea heals. However, frequent episodes like yours can be challenging. It's essential to follow up with your eye care provider to explore treatments that may help reduce the frequency of erosions and manage symptoms more effectively. Take care!

    Was cleaning yard and u think some debris may have gotten into eye. Optometrist put dilating drops in eye. It now appears as if there is a haze cloud in eye and around the peripheral the light has dimmed. An adverse reaction to the drops. Are these symptoms consistent w cornea injury. Don't want anymore drops. Whatxare your recommendations. Is there an eye test to check cornea without drops

      This is very tricky, and not something that can be answered electronically.
      Haze around an object in your field of vision implies a distortion, and if something happened with a foreign body, may reflect a change in shape of the cornea.
      This can be picked up on topography, tomography and aberrometry without drops.

      ​@@DrAnthonyMaloofthe optometrist now said the macula. Are there tests for this without using dilating drops. I reacted so badly to drops I think it further damaged my eye. Is this reversible. Can it be treated once diagnosed. If they use more drops i wonder if they will do more damage. I have other health conditions.

      I am unable to answer your question, and do not wish to misguide you.
      I am aware that some machines are able to scan the macula through an undilated pupil.

      ​@@nuelsimon9647seeing an eye specialist on Thursday. Still see hazy cloud in front of images and periphery is darker. Specialist insist on drops to dilate eye. Not sure what to do because the optometrist put drops in initially and it caused the cloudy hazy and darkening of light in eye. Don't know if I should allow drops again because caused much more damage buy I need to get a diagnosis as well

      @@DrAnthonyMaloof I went for eye testing and doctors said that there will be no remedy, but they adviced me to take good care of the other eye. They, the doctors said that you can also go overseas for surgery but i can't because of financial handicapped..‘

      I am not sure how the heath system works in Africa. You will need to seek out the equivalent of an optometrist or ophthalmologist to see where your options are for this type of surgery. After you do some searching, let me know how you went and how the system works in Africa.
