I watched your video today before my cystoscopy wondering what the hell to expect. I shouldn't have read the comments because they scared me more than anything. The procedure wasn't that bad. The worst part was when the doc pushed the scope through the prostate. The pain only lasted for a second. Your video was very helpful and I'm grateful you did it. Thank you!
My urologist told me his teacher required everyone in the class to have a cystoscopy done twofold to learn how to do it and to know what it feels like.
I've had 2 cystoscopies while awake and they were both horrible. Definitely a 10 out of 10! I especially like how doctors describe the cable as being so thin now... it sure felt like a very large diameter to me!
Urethrae come in different sizes due to genetics. My half brother had bed wetting issue as preteen and had to undergo a procedure to make his larger to solve that issue. Like most things with the body there is variation in sizes. And yes it was not comfortable for me, but it was bearable. I can see that some would be very painful and others no pain at all. Pretty sure the same would apply to catheters.
😡I had 1 done years ago, 4 blood in the urine & kidney stones. My scope was not a "small diameter"..it was the thickness of a magic marker!Sedated...but when I woke up IT WAS HORRIBLY PAINFUL! Most likely because they dislodged the stone & it passed. I was passing blood for 2 weeks. It was 1 of the most Painful Experiences I've Ever Had...& I've had ALOT of Pain in my Life!😭👎
I’ve had a lot of painful procedures in my life time and I can say on a scale of 1 to 10 this is a a 100. The last time I told the doctor do what you have to do to numb things up after several shot numbing locations he started, his raised eyebrow and my involuntary pain response was wow you really can feel that.
I have a cystoscope every 3-6 months for bladder cancer. This has been going on for 5 years now. Only one has been uncomfortable because they had to really fill the bladder. None have been painful and only a little pain on the first urination after the procedure. Make sure they use numbing cream prior to the procedure. I had one Urologist that didn’t and he is no longer my Urologist, just saying. I’ve been awake for all my procedures and returned to normal daily activity as soon as the procedure is done. I’m in my 50s and fortunately have not had any issues, though I don’t look forward to my 6 month appointments I don’t dread them either.
After reading comments here I was terrified about my procedure yesterday almost to the point of cancelling. However, I went for it and to my relief it was nowhere near as painful as expected. I must have had a very good surgeon and nursing staff. The worst part was when I felt a cold stinging sensation on application of the gel but hardly felt the insertion of the scope only slight discomfort on it passing the prostate and it was all over in a few minutes. I relaxed my pelvic area, closed my eyes and wiggled my toes during the process and this helped a lot. The worst part was the stinging feeling when urinating afterwards for the full day which subsides. I hope my experience helps alleviates any fears for anyone undergoing this procedure and I would describe it as more uncomfortable than painful.
No, it was more uncomfortable with the gel and some pain on scope passing the prostate. Maybe I had a good surgeon and it was over in minutes. It was more the fear of it that I had and I was almost going to cancel due to comments on here but glad I had it done. Let me know how your procedure goes and good luck. Thank you Dr. Rena, I am not sure why my Urologist's office staff seem to have difficulty answering even the most simple questions patient's ask. Your explanation was helpful, and covered the basics most people are likely interested in knowing; not least the amount of pain involved. After having an in office uterine biopsy, I have a justifiable mistrust of medical staff who fail to be transparent. So, I really appreciate down to earth, straight forward approach.
I have had many cystoscopies, i am a male, when there is plenty of numbing jelly used it is uncomfotable but not painful. However for a couple of hours after procedure i have the feeling I need to pee. Even though bladder is empty. For me the procedure has become routine. Almost 5 yrs cancer free.

I had a cystoscopy done yesterday. Like many of you I was dreading it and wishing they would put me under. But it was mostly a big nothing burger. Very minor discomfort and it stung when I urinated the first few times but 18 hours later no pain at all. I got to look inside at the mass in my bladder and I'm scheduled for surgery in 2 weeks but the cystoscopy was no big deal. Like Dr. Malik says you can do this and you will be fine!
@@BobTheMan2 I urge you to reconsider. If I had canceled mine I would not have found a tumor. It was successfully removed a month later. I went for my follow up 2 weeks later and figured now what. Chemo? Radiation? I was told that because it was caught early none of that was necessary. Just follow up every few months and see how it goes. My cystoscopy may or may not have saved my life but it definitely improved my chances. if you really have concerns maybe your doctor can put you out but cancelling it could cost you your life!
OK I will give it some thought I had Prostate Cancer, and the PA thinks it damaged my Bladder, A PA said this The doctor said it is a muscle tightening problem causing urinating properly, so do I listen to the Doctor or the PA?@@MyPetGoat2U
@@MyPetGoat2U I cannot talk to my Urologist he has the appointment desk to see his PA. Nurses answer all of my questions. Urology Specialists of the Carolinas is the busiest in the US. Appointments are set out 3 months or longer. I hate it but they are the one that diagnosed me with Prostate Cancer so have to stick with them.
I had a cystoscopy at age 29. It was by far the worst pain I have ever experienced in my life. The painful part is when they push it through to the bladder. In that moment my entire body from head to toe was immediately engulfed in what I can only describe as hellfire. It lasted for 3-5 minutes and I felt this extreme pain for the entire procedure until they pulled it out. Any person or doctor that says otherwise is bullshitting you. The camera is not thin. I (regrettably) looked over at the camera before they put it in and I swear to god that thing was the size of a spark plug. Never again.
A proceedure at gyni was unbearable,so I insisted on anaesthesia for cystoscopy. I coudn`t face any more pain. I also have severe osteo arthritis causing pain everyday. The procedure with Cystoscopy was a dream compared to the Gyni hellfire. Don`t remember a thing and woke up ,no pain and felt on top of the world.
I don’t know what you experienced and you don’t know what I experienced so saying that I am a bullshitter is bullshit. Pain for me was a 5 tops and was very brief. If you have symptoms that need to be checked do it. I have a very close friend walking around minus a bladder and prostate and has a bag glued to his abdomen. A cystoscopy is a picnic compared to what he has endured. Ask for sedative or whatever it takes just do it. Stop scaring people away from a simple procedure that might save their life.
Interesting, I've had two of these in the last 10 years and I agree with the experience being described. It was more discomfort than pain. Perhaps it depends on the type.. Rigid or flexible? Mine were the later.
in my case what bothers me is the prepping when they put that thing that injects the numbing gel.after that is easier at least for me. not everybody is the same.
Hell yes, that numbing gel contains an irritating chemical, and it has an acidic PH. It only hurts for about five seconds when they squirt the gel into the urethra, but it is quite uncomfortable. And there is conflicting Data from various studies, as to how effective the numbing Gel is. Some people would say it hurts bad enough putting in the gel, that they would rather not have the numbing gel. But of course whether you have the numbing gel or not, you would still need a lubricating gel, which does not contain the irritating chemical but the numbing Gel does. Anyway, do your homework as a patient to find out what this procedure is all about for you. Ask questions, be informed, as to your options, for pain relief. Especially if you have a big prostate.
Great video, and much appreciated. I've got a cystoscopy scheduled for a few days from now and as you can imagine I'm a bit nervous. This has eased my anxiety about it. I'll follow up after the procedure for an assessment of the discomfort. ***UPDATE*** Had my cystoscopy today. I've experienced some high levels of pain in the past, so I knew it would be okay, but I was certainly nervous going into it. For me it was a non-event. There was no pain at all and just a bit of mild discomfort. When the numbing gel was inserted, I'm not sure how it was done, but it felt as though it was forced into the urethra through a syringe. That felt a bit weird, but not painful. Dr came in 10 minutes later. I chose not to look at the camera beforehand as I did not want to unnecessarily increase my anxiety. Of course I felt the camera being inserted but there was not any pain involved at all. Just an unusual sensation that was mildly uncomfortable. I think it was more mentally uncomfortable than anything physical. Before I knew it he was in the bladder and looking around. Very slight pressure, still no pain. As for the removal of the camera, I did not feel it at all. The actual procedure was over very quickly. The Dr. explained there might be a bit of blood in my urine afterwards, which made me apprehensive to urinate. So far that has not been the case, though there is just a slight discomfort like a mild burning sensation.

I had a cystoscopy prior to a Rezum. I don’t know which was more painful. They were both 10 on the pain scale. I was in agony as the Dr. Literally rammed the scope into my bladder. Dr. Malik, you say no patient has asked you to stop the procedure. Sometimes a patient is in such agony, they only want it to be over. You are willing to endure the searing pain (hopefully) knowing it will be over soon. Don’t use that phrase, “not asking you to stop” as reasoning there isn’t pain. It was incredibly painful…….. and that was with 5 mg diazepam on board.
My dr actually said that most pain will come from the final push through. You either have to relax or he will just apply pressure until the muscle can't fight anymore, which is normally 30 seconds on everyone. No, my pain was mostly upon entry. Lasted until it was out. 10 out of 10 through the whole process.
I have had a bladder cancer (non-aggressive, thank God) removed, followed by a cystoscopy to remove a stent between my bladder and right kidney, which was intense, but as you say, pretty quick. I've since had six weeks of mitomycin treatments, a cystoscopy that determined that I'm at present cancer free, and just had my first of three follow up mitomycin treatments, and then another cystoscopy in June. What a ride!! Thanks for this, Dr. Malik...I'm blessed that I have an amazing urologist and staff, and I appreciate all that you do!!
I am 71, male had the cystoscopy to remove a stent due to a kidney stone. Urologist said it would be quick and a bit uncomfortable but local anesthesia would be fine. I trusted him. Big mistake. I did my research and many people comment about minimal pain and I consider myself pain tolerant but no masochist. Nurse inserted the tube with initial numbing which was slightly uncomfortable. They waited a good five minutes. The cystoscope tube was lubricated as well. Urologist started the initial insertion; for me it was similar to having the needle inserted for an IV -- a pinch then over with. I did have some "ooos and ahs" as the scope moved along but was fine. Then we hit the sphincter muscles before the prostate and all hell broke loose. It was clearly a 10/10 pain level, I was at the end of the my rope. Assistant did tell me to breathe which did help. And eventually the pain stopped. I think this was about four seconds. They then grabbed the stent and pulled it out which did not hurt and all was fine. My body did not deal well with the stent and just getting the stent out of me was relief. The whole procedure was short about two minutes (I think). Do I want to repeat it again - "hell no" but since the pain and whole procedure is short I think it is better than general anesthesia. I do agree with an earlier comment that urologist like to cause pain. With a stent inside me and four weeks of constant pain and urologist would only say take over the counter and I would say it is not working and then never going over with me the pain I would experience during the stent removal was horrible especially when he knew I was the kind of person that liked to know what was going on which allows me to mentally prepare for something. I was glad to hear that it is a small minority that have the overwhelming pain and now don't have to ___ my urologist for the pain he has put me through and when he kept telling me that all his other patients were not experiencing the pain that I did. They all lie. I put hundreds of catheters in men years ago. If they have a swollen prostate, it hurts like a son-of-a-bitch! It is not Rare! They don’t want to give you adequate pain medication because of the overreaction to the so called opiate epidemic. It is BullShit! Find a urologist who cares.
Payne is a very idiosyncratic phenomenon, but I think you’re Payne was greater than what’s covered in the video because you were having a procedure. It wasn’t just a simple CYSTOSCOPY. Hope you’re fully recovered and doing well.
I've had this procedure done, and it was way more mental concerning than physically painful. I rate it a 2 out of 10. The only discomfort was when passing through the prostate. But it was all worth it to find out everything was ok. Thanks so much for all your work and effort. I find so many of your topics SO relevant. Please keep it up, I appreciate it.
Had two cystoscopies several years ago. While it’s not fun, the anticipation was worse than the actual procedure. The only part that I would classify as uncomfortable was when the scope was passed beyond the prostate into the bladder. If there’s nothing wrong, the procedure doesn’t take more than a couple of minutes. Definitely not worth it to have general anesthesia for this. Just relax and it will be over before you know it.
I would rate this procedure similar to yourself. Urologist used numbing gel and a flexible cystoscope. Bit uncomfortable in the beginning, but the procedure itself lasted less than two minutes. Typical burning sensation when peeing post procedure but that didn't seem to last very long either.
As for the burning sensation when peeing after the procedure, patients can and probably should, purchase an over the counter pill Called AZO, also known as, Pyridium, which is supposed to offer a soothing effect to the urethra. Be advised that the medicine turns your urine kind of bright orange. Anyway, this medicine should at least help with the burning sensation. Secondly, if the male patient has a very large prostate, this procedure can be extremely painful. Passing that scope through the part of the urethra that runs through the prostate, Just before the bladder, can hurt like there's no tomorrow due to the narrowing of the urethra caused by the large prostate pressing on it. So, if you know before going into this procedure that you have a large prostate, be forewarned! This procedure is not going to be any walk in the park. And while that pain may last for less than 10 seconds, you will remember it. Just saying. The so called numbing gel likely is not going to reach that part of the urethra. And speaking of the numbing gel, it is quite uncomfortable for about five seconds after they squirt it in there. And by all means, make them wait at least five minutes after putting the numbing gel in so it has a chance to at least try and take affect. The medical team may well want to start the procedure right away after putting the numbing gel in but that is not the proper way to do it. I have read several studies on this topic and it is very important to wait at least five minutes. Don't let them rush you, the patient is in charge in so far as they are the one who will be experiencing the pain if the numbing gel isn't given a chance to at least help. Lastly, it's pretty easy to find a dentist who will give you some level of sedation for a simple cleaning of your teeth, but yet various urologists, based on what I have read, I think it is OK to inflict this level of pain on a patient without any kind of sedation except for the topical numbing gel. It's crazy.
@@jackmeyhoffer5107 Same for me. I was more worried about the initial insertion, the numbing gel did it's job and I really did not feel anything until the prostate was reached. The pain I would rate a 4 but very brief, I have felt more pain banging a knuckle when a wrench slipped. Burned a bit for the next 2 urinations. Maybe some Docs have better techniques than others or because of my age 61, it just wasn't that bad. BTW I have absolutely no reason to BULLSHIT anyone.
Discomfort is a serious understatement as I had to have a double cystoscopy when I was a kid. In short, it felt like a flexible needle forcing its way inside and the pain was an 11/10. I then heard my family doctor had become furious upon hearing the procedure and insisted it wasn't necessary. Two days of painful peeing. But this was back in the 90s, I hope things have changed to make the procedure less painful.
i just had one done. it was the worst pain ive ever experienced by far, and ive had 3rd degree burns. nothing has changed drs just have no concept of pain
I’m a female and was told by the doctor and many others that it doesn’t hurt and just uncomfortable, it did hurt!!! A LOT!! It was more bearable once it was fully in, worst pain was it going on but it still hurt pretty bad! I think that people who says that it doesn’t hurt are woman who already had a baby, cause everyone who said it didn’t hurt to me were moms. Just had this done today. Was very quick and painless with some pressure. Not bad just had to breathe like I was lifting weights. The doctor guided me through the whole thing. Was nowhere near what I feared it would be.
I had the same experience last week. Really quick and painless. After seeing this video and reading the comments it makes me wonder did my doctor do the procedure right or should I get a second opinion because he didn’t fill my bladder up the way this doctors is describing.
Your experience is a ray of hope for other guys that might have to go through this procedure... Perhaps, you had one of the best doctors in the world! Lol. Please share who he/she was for the rest of us. I personally think that you should have the OPTION of being put (totally out/asleep) for any kind of painful procedure! Whatever the cost is, I'm sure men/women would come up with the money, come rain or high water! LOL.
@@vernonparrish7066 it was Chesapeake Urology in Maryland. I have an uncle in Connecticut who recently had it done and he also said it didn’t hurt but he had no interest in looking at the monitor when the doctor was trying to show him what his issue was.
I'm calling BS on the pain scale. I've had a bunch of pain inducing procedures over the years and this was by far absolutely the most painful thing I ever had done.
Very thorough and informative review Dr. Malik! As a bladder cancer survivor now for 6 plus years, I've been through this twice per year during the first five years and now just once per year! I've found that numbing jelly can help, but it's a quick procedure and the worst symptoms are the very next time you have to empty the bladder. They always issue five antibiotics and that helps insure you don't get any other infections.

I've had a cystoscopy done when I was 22 and although I've undergone various endoscopies, including colonoscopy and two gastroscopies while I was completely awake and without any sedation, I have to say that this one was by far the most painful. Turned out I had a significant urethral narrowing for which I had to go through a urethrectomy while awake with topical anesthesia which I also felt and all I gotta say is that I normally had to go back for an evaluation and I have been putting that off for years now. I think my narrowing has come back now but I am not willing at all to go back for another cystoscopy. My doctor said it hurts much more with male patients but any doctor who tells you that it won't hurt, they are plain bullshiting you.😬💯

Please, do not let ANYONE tell you that a cystoscopy is a "little uncomfortable." The pain was so intense that I almost fainted; I had to lie on my back on the cold floor with my feet elevated afterwards to prevent myself from passing out. It felt as though some little gremlin was inside of me with a dagger, stabbing me from the inside. The reason that no urologist will tell you this in advance is because if they did, you either (a) would not get the procedure done, with possible risks to your health, or (b) you would ask to be put under general anesthesia. Option B would make a cystoscopy an utterly impossible and incredibly expensive task and it would cease to be a diagnostic tool. I will be going under general anesthesia for my Urolift.
@@Shield954 Probably not in every country. I had one in Switzerland recently and I was not sedated at all. I only received the numbing gel before the procedure
@@Shield954 They do most of these routinely in the clinical exam rooms; when "sedation" is used, the procedure is done in the operating room (since an anesthesiologist is required) such as for TURBT, Lithotripsy, kidney stents, etc.
Thanks for the video! I don't remember a great deal of pain. The main thing that I remember is that my bladder felt full even after I'd emptied it after the procedure.
I had rigid cystoscopy with only local numbing gel last week. It was the most painfull and embarrassing thing that ever happened to me. PAIN 11/10!!!! I'm a 22 year old, healthy male. So i really can't even imagine what pain would it be if i would actually have something wrong with my urethra or blader. Considering that I had it done at the university hospital, I was also so lucky to have 3 medical students (all female) present for this procedure. So basically it was about 10-15 minutes of being fully naked with spread legs screaming in unimaginable excruciating pain in a room full of people (again females) around my age :) I WOULD LITERALLY RATHER DIE THAN DO THIS AGAIN.
All negative comments. I have had several and all went very well. No serious pain. Doctor talks and details the procedure. Thank God for doing such a great Doctor and nurses
When I was facing the need to have one, I was given the option to be put under general anesthesia. I asked my urologist what he would choose. He said general anesthesia for sure.
I had one done prior to a Rezum procedure. I have a Hank Hill narrow urethra and it was definitely a 8-9 on the pain scale. And I developed an infection afterwards. Needless to say that when I went for the Rezume, I was unable to stand the pain and had the Doctor stop. I'm no Sally when it comes to pain usually but this was more than I could bare. My Heart bypass surgery wasn't as bad.
Anything to do with sticking something *INTO* the urethra is a masive hell fucking no! Why any sane person would choose this torture instead of just having it done under general anasthetic is beyond me. And I’m someone with a generally high pain threshold saying this. Nope! Im not stupid. I aint falling for doctors lying saying it aint that bad. We BOTH know it’s unbearable! Tldr: to any man reading this… GO UNDER!!!
I'd definitely give it at least a 5 on the pain scale. It didn't help that I had a Dr that told me to "suck it up" very hatefully either. I wish I could find a urologist like you Dr. Rena!
I have to go next week to see a urologist and I'm scared s***less. Been deployed, shot at, through aerobatic flight training, etc and never been more freaked out than this lol.
I had the flexible kind a couple years ago. I found it incredibly interesting to watch on the screen. I think I was too engrossed in watching it to pay much attention to the pain. I would rate it as much better than a dental visit.
I’ve had several cystoscopies and whilst uncomfortable and psychologically worrying it’s not that bad. I would certainly take a Xanax or similar beforehand if offered. Pain wise I would rank it around 4-5, definitely not a 2-3. The burning on void is afterwards which lasts around 24 hours is also unpleasant.
I concur. I seriously doubt that most people consider this pain level to be 2-3 on a scale of 1 to 10. The number of people who weigh in on this message board, Stating otherwise, is evidence that it is no walk in the park for a lot of people who undergo this procedure while fully awake. More people should probably be given Xanax or something like that at least a day or two leading up to the procedure because it can be extremely psychologically worrying to be scheduled for this scary procedure. It sounds barbaric, and for some patients, it is. Not trying to worry anybody, but do your homework and be informed, as to your options regarding sedation, pain relief, etc.
I would say my pain level was a 10 out of 10. Ridiculous to indicate an expected pain level of 2-3. This was the most painful and miserable medical procedure of my life.

Read a lot of the comments and realized I was extremely lucky. No pain at all. Little burning when the nurse injected the Lidocaine before the procedure but other than that no discomfort. More anxiety than anything else. Fortunately for me the doc said I had the prostate of a 25 year old. I’m 73. So no difficulty inserting the cystoscope past the prostate. Diagnosed with a neurobiology bladder condition. Been doing intermittent self cath now for 6 years.
I’ve had several of these. My pain level was much higher, especially when they added the fluid….Doc says…”you might feel some PRESSURE” yikes! Difference between slight pressure and 1000 psi. Only saving grace (this was to surveille for cancer) was it didn’t last long…seconds really. I have kidney stones, so I anticipate more of these in the future.
I had a cystoscopy and I felt it go through my prostate and when it went into my bladder. Uncomfortable but no pain. Then I had a vasectomy right afterwards. No problems with either procedure. This is a good video of the procedure.
Just had this procedure yesterday. Incredible jolt of level 6-8 pain for about 3-5 seconds when they fill up your urethra with lidocaine gel to (ironically) prevent pain as they insert the scope. Several seconds after inserting the lidocaine they inserted the scope itself, which was pretty painless; only about a level 1, obviously because of the lidocaine. Another jolt of level 6-8 pain (very similar in quality to earlier) for 2-3 seconds when they go through the prostate. Once the scope is in the bladder pain level returns to about 1. Mostly it's the idea of the whole thing that is so disturbing - one wrong move and yikes! Despite there being next to no pain as they looked around, I was still sweating bullets and feeling a little nauseous. When it was finally time to remove the scope there was some pain, maybe level 3-4, but only for a split second because they remove the scope a lot faster than they insert it. If you must have this procedure, my advice is to NOT look at the scope before and certainly not during the procedure. You don't want to see the girth of this thing, trust me. But don't blow off getting the procedure either, as it's very informative and could save your life.
This was exactly my experience. I would also add the pain when urinating after you get home is pretty significant as well. I could urinate, but had to psyche myself up, and bite down on something.
After reading some of the comments, I was a little freaked out about have this done. I had mine today and was so happy that it was pretty much painless.
I had 8/10 level of pain. The numbing gel which they used liberally seemed to not help at all. The size of the tube used actually tore the end of the urethra at the pens tip.
The numbing jel didn't work for me either. The procedure stopped in the middle as i could not tolerate the pain. They say that they will carry out this procedure with anesthesia for me.
This was the worst procedure I have ever had. I had this done at a major hospital and not only was it painful but the bed side manner was inexcusable ! Believe me, I understand they do this all day everyday but the experience was new to me. Laying naked on a table for 30 minutes waiting for the doctor to come in. Several nurses and Associates coming and going the whole time and you're on exhibition. This table has stirrups so your legs are up in the air bent backwards and you are not I repeat not coverd up while waiting. You mentioned a small percentage experience level 5 or higher pain, I find that hard to believe! I have had many types of tests in the past but this one is inhuman.. Next time knock me out
It is outrageous that they have no regard for your sense of privacy and human dignity. Yes, I know they do this all day routinely, but comon now! Show a little humanity! I never had this procedure, but I know very well how certain offices operate. I personally experienced humiliation at a dermatologist when the female doctor had me first fully strip --- then she left the room and I do believe she instructed a couple of colleagues (female non-doctors such as secretaries) to "happen" to enter the room for a "free good look". In no way do I believe that should be accepted as normal protocol in a doctor's office. Would these same office workers like to be fully naked for us males to see fully on display for a "free good look"???
What health care professionals need to understand. The comment ive seen it all before does not give me encouragement. It makes me feel like an inconvenience to worry about what they seem as a trivial thing. Why on earth does there have to be so many people in the room at the same time as I'm lying naked. Then to be told that I'm blocking the pathway for students learning for not letting them in the room. The doc said they won't be performing just watching. I'm trembling at the thought of getting undressed at this point. All hospital departments are the same no thought to as to what the patient is mentally going through or why we feel like this to being with.
Things will never change the big money is in womens health so offices are set up to make them comfortable lots of female nurses.Even urologist who largely deal with males usually have female nurses who handle your penis during procedures mostly like it is a gear stick.Also leave males naked and exposed for long periods of time whilst women staff members walk in and out.All the time they say don’t worry they see that all the time.Try using that statement at woman’s pap or breast exams
Had this procedure and it was no big deal. I’m female. What I am seeing in the comments is this procedure is much more painful for guys. Maybe there needs to be a different pre-procedure protocol for males. My brother had this done along with Rezum. He was traumatized by both. It’s something the urology community needs to think about. I think for men it's just a very strange sensation inside the penis they never felt before I had experience with catheters before I got the scope so it was lot like inserting one and removing it.
And more embarrassing males are not as used to being naked from waist down as women are we do not have to display our genitalia on regular basis as women do.Also when they do get us in stirrups with penis hanging out there always seems to be young females hanging around taking notes or doing some clerical task
I wouldn't do it at all. And if I absolutely had to it is being done with regional anesthesia (paralyzed from the waist down ). That is not something I am gonna wanna feel at all. Also male only don't care what they have seen I probably have seen worse and been through worse.

I was nervous about it so asked for it under anesthesia. They called and scheduled it for 1 mo from now. As a retired RN i know time is of essence so I called and said I would do it w/o anesthesia. 10 out of 10 pain! I was told not to urinate before. I was not given a numbing anything. There was lube on the tube which was inserted immediately after I laid back. I know pain and handle it better than most. This was a pain I've never felt or imagined. Construction, mechanic, smashed finger tips, broken bicep smacked my shoulder, kicked in the testicles, welding cross bracing on joist and being zapped in the testicles while straddling a wet joist. Stepping on Lego's wrong!! Nothing compares to the pain I felt immediately upon entry. That burning, that uncomfortable pressure feeling.. yup, felt that too. Almost like a warm friction burn as it slid in. No, I'm not talking about that pain. I'm talking about the feeling of my penis being ripped in half from the inside out, at the tip. The very tip! The pee hole!! A feeling like two of my wifes acrylic fingernails were pinching the very tip of it as hard as they could squeeze. From the time he entered until it was finished. All while he was speaking over me to tell me it was my fault and I needed to relax. All in about the longest, most painful, maybe 90 seconds, of my whole life. While in there he blasted apart some scar tissue he found in the urethra, in hopes that it will heal better this time and no longer restrict my urine flow. The blast was a whole different pain. Still nothing compared to point of entry. I'm sure everyone in the whole office heard my screaming and the pain I went through. The words I was screaming that I don't use in front of my mother or my kids. It was an embarrassment to walk out of there. Today is 3 days and I think the blood has stopped. I quit the azo yesterday and it only hurts towards the end of urination. If I had known, I would have went with restricted urine flow for the rest on my life. It wasn't that bad!! Lots more details, but that's the most of it. If you feel there's no way to avoid having it done, take the meds! It's not worth finding out if you are in the 5 percent.
It's been 2 months, 3 antibiotics for infection that wouldn't go away, multiple doctors appointments, hurt to urinate for almost 6 weeks after. Still hurts today to sit on a hard surface too fast. Weird feeling in my taint shoots another weird feeling all the way through to the end on my penis like squeezing a hot dog balloon and it shooting out the end of your fist. If you think you have a small penis hole, you prolly do. Trust me. If I had known, I may have tried some stretching or something. Idk. It's a nightmare that I wish I never would've went to the Dr for. A feeling I will never forget and a scary feeling if my heart was gonna allow me to make it through the procedure without stopping. Never again!!
Yes, worst pain ever!! Rough Doctor in Columbus, Ohio, now moved to Florida. Probably got ran out of town. They could have use a child’s size scope on this little guy but today I would never do it again even with a smaller scope. Other guys have given the same pain descriptive experience. I can not explain how painful it was. NEVER AGAIN

Everyone is different but this is my genuine experience - Female living in Scottish Borders. Just back from having my cystoscopy at our local hospital. Was dreading it but nurses and doctor put me at ease. They were also very good at retaining your modesty as best as they could. Doctor numbed the area and, yes you can feel the scope going in, but to be honest, for me it wasn't painful at all and the procedure was over in less than 5 minutes. You do feel the need to pee afterwards because you've been scoped and they've put a solution into you and it is a bit "burny" the first few times you go to the toilet. Just remember to keep drinking plenty of water for a few days. I think it is slightly different for men and women. Women only have approx 4cm in through the urethra before the bladder whereas a man has approx 15-20cm and different angled pathway on the journey to the bladder. As I say, everyone is different and I'm sure doctors carrying out the procedure are different too but my experience was fine.
I had a cystoscopy about a month ago. It wasn't that bad. They didn't even use numbing gel. Fortunately, my doctor used a flexible scope. The worst part was insertion which was very briefly uncomfortable. After that, it was no big deal. The vaginal exam my doctor did afterward was more painfu.l The medical assistant said I did well for my first one. I wasn't nervous, so I'm sure that was helpful. I know some women will not have the same type of experience I had, even with a flexible scope. I want people, especially women to know they may have a positive experience. I don't have chronic pelvic pain, but pelvic exams are always somewhat painful for me as I have a tight pelvic floor. And even with that, my cystoscopy experience was positive. I can only speak from my own experience, but often people are more likely to share negative experiences than positive ones. So, hopefully this helps encourage people who may have seen overwhelmingly negative comments.
I just returned from a cystoscopy procedure performed under local anesthesia. The procedure was completely painless, performed quickly and efficiently for diagnostic purposes. The whole thing took a maximum of 10 minutes. Despite natural fears, they turned out to be completely unjustified. I felt comfortable during the procedure. Maybe it's a matter of the doctor's skill. So look for one from a recommendation.
I had a cystoscope about 3 weeks ago and I’ll be honest with you, I was so nervous that my Urologist gave me a nerve pill before the procedure. The pain was probably about a 4-5 out of 10. It wasn’t as bad as I had built it up in my mind to be, but it still hurt. Hope I never have to have another one
I have to have one sometime soon, as I am getting a kidney transplant sometime in the near future, and have some other issues with my prostate. I am absolutely freaking out - even the thought of putting something in my penis creeps me out. I think it is an unnecessary procedure in my case, and am still fighting not to have it done.
@thethecommander When he said "nerve pill", I figured he meant an opioid, not anxiety meds, lol. Anxiety meds will reduce stress, but it won't reduce pain.
I ate a Xanax bar and a ten mg oxycodone 90 mins before last procedure. I was completely baked and it was like trying to put out a forest fire with a squirt gun. Still 10/10.
I had this done 7 days ago (female). I was given no instructions ahead of time (or after), and I expected to go to work right after since I was told it would be a "simple 10 minute office procedure." Maybe I had a unprofessional doctor, but no numbing gel was used to my knowledge. The Dr could not fit the scope in my urethra due to post menopausal atrophy, so he decided to use expanding tubes twice. I've never cried from pain in my life even giving birth twice, but I cried through this procedure. I could not pee at all when I got home and had to take AZO max strength. The pain was worse than any UTI and lasted 3 days. The Dr also gave me a pill prior to the procedure and did not tell me what it was. I assumed it was pain meds. I researched the pill when I got home and discovered it was antibiotics. What doctor gives only 2 pills of antibiotics?? All that does is create superbugs. Be your own advocate. I usually research everything to death, but let my guard down because the Dr said it was a simple procedure. Continue to ask questions and find out what if the procedure does not go as expected.
You said it. You're right about their poor use of the antibiotics creating super bugs. And if they don't want to answer your questions before doing the procedure, then run away. They don't have your best interests in mind.
I ended up finding out that kidney stones was mimicking my uti feelings. I've passed about 8 since that exam. Using estradiol vaginal pill (or cream) helps soften vaginal and urethral tissues. So that might help the cystoscopy procedure (if you are premenopausal or menopausal). @@samiethecat2620
ive had multiple surgeries: acl repair, a dental implant, a tunneling surgery to relieve pressure from mrsa as well as many others. ive been through extreme rhabdo from overuse of my muscles (cpk of 75000), ive had serious hemipalegic migraines, i also had major burns accross a large portion of my stomach from a powder explosion. this test was more painful then all of them combined. when they went past the prostate i literally screamed. i know this isnt everyones experience but all of the resources ive seen are saying it doesnt hurt and honestly they are totally incorrect
I am so glad that there are a few people who experienced what I did today. It was insane and I groaned very loudly - I didn't wanna freak out the waiting room. My back involuntarily arched like I was John Rambo being electrocuted on that metal bed in Rambo III.
I watched this video and comments last night and got very worried. I just had a Flexible Cystoscopy exam where they used a numbing gel and I barely felt anything. It also lasted for all of 2 or 3 minutes. Everyone’s experience will be different, however, I am surprised that some people experienced such severe pain.
I had one done a couple years ago, and honestly, I felt brief discomfort and pressure, but it wasn't as bad as I thought it was going to be. It did hurt a little bit when I had to pee afterwards...but they gave me an antibiotic just as a precaution to prevent any UTI or infection, and that was it!
**Deidre Haye,** I had mine last year. The urologist had my legs in the GYN-type device as if I was about to give birth...LOL. He had a female assistant and the rushing of the water stimulated my prostate (I should have been forewarned). It caused an involuntary erection. There I was, embarrassed to no end when I asked, "Why is this happening, Doctor?" His reply was that it's completely normal. - STILL: I was embarrassed!
This procedure was hands down the most painful procedure I’ve ever experienced and I have had other procedures that are supposed to be painful such as teeth pulled with out numbing. The fact that there is no local anesthesia or numbing is crazy. While I believe this is a necessary procedure and has probably saved many lives when dealing with urology issues, Buckle up Buttercup it’s going to hurt!
Had a flexible cystoscopy a few hours ago, here in the West of Ireland. Gel put in, followed seconds later by the cystoscope. Was not too hurtful at all. On a discomfort scale out of ten, I would give it a two, that’s all. Even though my foreskin is tight, it slipped reasonably easily, the Doctor spent about 7~8 mins having a look around, and then removed it. The three nurses in attendance were very encouraging saying I was doing well, and to breath deeply, while smiling in a friendly manner, was more then helpful. Main point, at least in my case. It was nowhere near as bad as the comments on RUclips make out. The sense of relief, and enormous weight off my shoulders, after many many months of dread, anticipation, and worry, is something else. Enlarged prostrate 60cc was the result. Had I not read my medical notes, recently, that landed me in the emergency department in the first place, with kidney back pressure, fluid retention, AKI acute kidney and injury and uroseppis mainly caused by inability to urinate, and a stretched bladder, I would not have had this cystoscopy today, but I am very glad I did. I would encourage anyone advised to have this procedure, To go ahead and get it done. Bit like going to the dentist, no one really wants to go, but you will be glad that you did, and stop reading and watching overly sensational videos on RUclips, and particularly reading the negative, over the top comments like I was doing, that may cause you refuse this procedure like I did, before it maybe too late for you. I am just glad, and lucky that my problems ain’t stones, cancer or other issues, that can be hard enough to identify without such a procedure , but can be treated successfully if caught early enough. Her indication of minimal pain from the procedure, is ridiculous and not my experience. Horribly painful as it passes through the prostate, which she has no concept of. Personally I would look for a male urologist who has some empathy and experience with the procedure themselves.

@@ellitestar because they wear scrubs doesn’t mean they are nurses.The problems is male patients are left exposed in situations that female patients aren’t.If individuals are comfortable with being exposed to females is not the issue it is lack of respect for males in medical situations
I had three until now from 3 different doctors. The procedure was always 1st inject a anaesthetic gel into the uritrea. That burns a little bit. Then feel a medium pain when passing the close muscle. After removing the stethoscope I was bleeding one day during urination. This happens only at my last cystoscopy. But always I had horrible pain during urination a couple of days. After the 1st and 2nd I got Antibiotikum as a prophylactic.
I had mine this morning. It was painless with some minor discomfort. It wasn't that bad. You people are over exagerrating in these comments. I was scared like I don't know what after reading these comments. My doctor Dr. Mac was so kind and put me at ease.Not a bad experience at all.😊
To those nervous: I just got mine done this morning. On a scale of 1-10, if I could give it a 0, I would. I felt a bit of pressure, but it definitely didn’t hurt and I was fine right after. It didn’t hurt much to urinate after either. The hardest part was swallowing the big antibiotic capsule beforehand. 😅
The procedure was nothing. The nurse put some lidocaine lubricant into my urethra and about five minutes later the doctor arrived. Mine was a flexible tube and I only felt pressure. It was mildly uncomfortable when they filled the bladder with fluid. I don’t think it took longer than 60 seconds. When the scope was removed a lot of liquid came out and onto the pad I was sitting on and then they had me dress and pee before I left. It wasn’t fun but if I needed another one tomorrow I wouldn’t care.
A cystoscopy is VERY painful. Urologists ALWAYS play down the pain. I suggest getting stoned on Xanax before the procedure. Have someone drive you to and from the doctor's office. Some urologists are better than others, and it may be somewhat less painful, but it still hurts like hell. Think about it. The urethra was not designed to accommodate a medical instrument being shoved deep inside it. I really have to question why urologists are not more truthful about the pain.
Had this procedure done, painful, after urinating was very painful and some blood like you said, this lasted for about 2 days.They did it to determine the stricture> I wish there were doctors like you in my clinic and hospital.I had to deal with a stricture as well as an enlarged prostate which I believe was misdiagnosed prostatitis I am only 46 and every time I get the prostate checked it seems to be a normal size.

@@BallroomSceneNerd a history of UTIs slow stream , painful urination, discomfort after ejaculating.,even led to a bout of UR had to be catheterized, they put me on tamsulosin which I still take. Cause unknown they suspect it could be from the persistent UTIs. Had a scraping done with the tube recently but still have symptoms, next step maybe a uretheoplasty
I have had several cystoscopies following a diagnosis of stage 0 bladder cancer. It isn’t too bad…the only moment it hurts is when they penetrate the prostate…but that only lasts a few seconds.
I’ve had many cystoscopies and my urologist gave me some great advice that I’ve used every time since - wiggle your toes! It may seem silly but totally works.

I'm a 75 year old male and first had a cystoscopy in 2010 when I had bladder cancer. I have probably been scoped 40 or 50 times. I have had a handful of completely painless exams, but no excruciating or even very uncomfortable ones, either. Mostly, there is a jab or two somewhere along the way that is unpleasant, but in the grand scheme of things, a nothingburger. Several years ago my urologist switched to a scope where both he and I view what the scope sees on a screen in real time. From the point of view of my comfort, that is a very big development. Being able to watch the procedure is a distraction that calms or damps the occasional pinch.
I am an 39 y old male and had one last year. I was really scared(more when i saw the size of the instrument) but when it started i felt no pain what so ever. The only thing i felt was a strange (but good) sensation when it pushed past the prostate. The doctor was nice and explained allong the way. 2 weeks of anxiety all for nothing xD and the whole experiance was more interesting than anything else.
Had a flexible cystoscopy done a few weeks ago as a 20-something male presenting with enlarged prostate and bladder. Although the doctor said it would hurt more since i'm young, i'd say the pain was a max 5/10 (i'm bad with pain) after lidocaine injection and during insertion, and more discomfort than pain while it was in me. Surprisingly, I did not have any burning sensation while peeing, past some mild rawness on the first one immediately after surgery. I was back to my normal activities within an hour. Unfortunately, i'm going to have to go for a rigid cystoscope biopsy due to abnormal sightings. Overall, make sure you're getting local anaesthesia injected into your urethra + lubricated scope and you should be fine. I'm shocked that some commenters here have gotten it without any.
A coworker had to have these done regularly for bladder issues. She said the pain was higher than 10 and needed help walking when leaving she would be in so much pain.
Without a doubt, the single most painful 30 seconds of my life. Felt like my penis was being ripped apart from the inside. Have had trouble urinating (difficulty starting and weak, dribbling stream) since I was in my twenties (now 64). Was told it was not a prostate issue but extremely tight muscles and sphincter that would not relax. This was apparently the cause of my extreme pain. As a side note, a daily, low dose of Cialis has helped my urinating significantly.
That's strange when I've had viagra I pee much better in the night.. I had a flexible cystoscopy and it was much thicker than they said I would be and was uncomfortable not painful but embarrassing because I was naked with 5 people in the room. But the worst thing for me was the water infection after
Dempsey 95 ..... That's terrible that you had FIVE people in the room, thus making you feel understandably, embarrassed. This is procedure is normally a 2-person job .... the MD, plus one assistant, and worst case, MAYBE one extra person, so at most there would be perhaps THREE people needed but that's all. But FIVE people is way out of bounds for a flexible cystoscopy procedure. I have seen it done by the doctor alone, all by himself, with NO assistant, for the actual procedure. A patient should always be told who is in the room for such an invasive and embarrassing procedure, and why exactly they are there. The patient gets to make the rules .... because THEY are the one who is naked and fully exposed, feeling vulnerable, scared of the procedure, feeling pain and anxiety. To have EXTRA people in the room, who only add to the patient's overall level of discomfort, is BS and should never happen without the patient's expressed consent. That's crap.
Since you say you’ve been told the painfulness is due to tight muscles, you might be well served looking to Pelvic Floor Therapy. Maybe you suffered some kind of trauma and your subconscious mind would never unclench. It’s worth a try.
Putting the anaesthetic in the urethra beforehand (3/10) was more uncomfortable than the cystoscopy itself (2/10). Loved watching the visuals on the screen. Worst part was urination for 2-3 days after the procedure (4/10); burning sensation.
I wanted to give my anecdotal flexible cystoscopy experience from a male perspective in hopes that it might help someone suffering from anxiety about the procedure like I was. The most painful part of the whole thing for me was the injection of the lidocaine gel into my urethra. This actually did feel almost like a slightly time extended needle prick to me but there is no needle involved. On a 1/10 pain scale I would however just give it a 2 or 2.5 at best. I may have winced a little bit from it but that really is about it. Once the actual procedure began, I would describe it as just uncomfortable, not painful. I did not really even notice that the camera/scope was inserted until it reached the sphincter that it has to pass through near your prostate. I would liken the sensation of that happening to be the equivalent or less than the equivalent of a doctor’s finger palpating the prostate during a digital rectal exam even if you have an inflamed prostate like I do. Once inside the bladder and the doc was evaluating it for any abnormalities, I really only had a feeling of “fullness” which was just uncomfortable and not painful at all. I did not even feel the cystoscope being removed after he said all was clear. Everyone’s anatomy is different though and it’s clear by the comments that some people have a hard time with this procedure. By no means either do I think I have any kind of enhanced pain tolerance to be able to withstand this procedure better than anyone else. If you however need to have the rigid cystoscopy, that is a different animal it seems and should be done under anesthesia which I believe is standard practice. If your doctor recommends a flexible cystoscopy procedure be done in order to rule out any kind of disease including cancer, please do it. I am glad I did it now after stressing for weeks about it. The peace of mind now that I know I do not have bladder cancer was worth it for the price of a little bit of uncomfortableness of the course of what was less than 5 minutes and I would do it again with no stress involved.
Pain. Dr. Malik gets a score of 95% out of one-hundred for her use of the word pain versus "discomfort" or "uncomfortable." Here are two descriptions of when something is "uncomfortable." At night sometimes a pillow gets too warm and to stop it from being uncomfortable, it is flipped to the cool side. If a dry cleaner puts heavy starch in a collar instead of light it is uncomfortable. Anything beyond that is painful. Dr. Malik always followed up her use of discomfort by saying something like, "a little painful." My own experience is that this procedure hurts. It is painful. But, as with many men, after they were done, I was very stoic. Being stoic and quiet afterward might have led the doctors to assume it didn't hurt. Only one doctor (out of three) confirmed my lack of pain. He said, "that wasn't so bad, was it?" I think I muttered something that could have been mistaken for agreeing with him. It didn't matter much because he was out of the room before I had much of a chance to form a thought and explain my experience. All three of mine were done with a flexible scope. I understand that the pain is less with the flexible scope. I've had Foley catheters inserted and removed. I've also had one straight cath following surgery when I had to pee but could not. I've only felt minor pain for a few seconds when a Foley is inserted. Removing a Foley cath is so fast that the tiny little bit of pain lasted less than a second. The straight cath hurt a bit more than the Foley. Comparing a cystoscopy to a catheter, I would say the cystoscopy hurts at least 20 times more.
Aren’t I the lucky one. My pain level goes from zero to 100. As the scope goes past my prostate (straight scope, curved path), I typically want to rise up off the table and sink my fingernails into the ceiling (a la “ The Exorcist “). I have had dozens of these procedures (bladder cancer diagnosis, treatment and follow up) over the course of about 5 years. And the painful urination (passing razor blades) afterwards only lasts for about 2-3 days. Doctors seem to like to call this “some discomfort”, which translates to “I want to die”. Other than that, it’s a snap.

I had a rigid Cystoscophy. and had anaesthetic. I don`t do any pain nowadays since I had hellish experience with a Gyni camera x-ray. never again without an anaesthetic.
I had a ridged scope put in once. I think the urologist trying to teach me a lesson in order to get me to change behavior. It worked! Second worst thing ever done to me. When he had it in, he filled my bladder with saline. You go from no need to pee to the worst urge you’ve ever felt! Then he was moving it around to inspect my bladder. Felt like he was looking for targets.
Mine was horrible. First, no meds offered and I said I was a nervous wreck and no numbing. The doctor couldn’t find where to put it after 7 attempts then said maybe I had scar tissue. Out comes the large metal instrument to push, pull, scrap…unbelievable pain. The camera itself was no big deal after that. It was the metal tool that almost killed me!
I think the doctor should record the procedure. Once the scope was pushed passed the sphincter, I could look at the screen. I was just so focused on bracing myself for the pain and uncomfortable force. I would have liked to see the short procedure.
This was the most painful, horrific, traumatizing experience Ive ever had to endure. (45 year old male). The pain was beyond words. I have scarring from a previous infection and my urethra always feels sore and irritated already and this drove me over the edge. I have a very high pain threshold and this was an absolute 10/10 pain scale. It felt as if he was dragging a razor blade through my urethra. When he pushed through my urethral sphincter, i screamed so loud and begged him to stop the procedure. These 2 minutes were the worst 2 minutes of my life. And the most painful part was trying to urinate 15 min later. Extreme shockwaves of pain. I might as well have been peeing fire. Bloody urine. Extreme level 10 burning pain. Cannot be described in words. Im absolutely traumatized by this experience. I couldnt even drive away from the hospital for an hour, just sitting there sobbing to myself. Its been 48 hours since the procedure and peeing still is a nightmare. Still bloody urine. Still level 10 pain whenever urinating. Im not even myself. Just in a daze. Emotions all over the place. Cant concentrate on anything. Random crying spells. This shit fucked me up dude.
My experience exactly. The pain that I experienced left me in tears and mental trauma for a year. I like Dr. Malik but her description of the pain during the procedure is bullish*t.
But I also think it depends on the doctor, because I've seen some videos on it and they act like they don't care where they are putting it, plus there have people who have gotten permanent ED from it, again I think it's how the doctor cares about where they're putting it and not shoving it through fast.
I had a Cystoscopy done this morning and it took just about 10-15 minutes total. There was hardly any pain at all and I would rate that pain or discomfort at 2/10. My prostate was huge and was described by the doctor's assistant as 'Texas style'! After the procedure I urinated and just a little bit of blood came out. I kept drinking water during the day without any evidence of blood in the urine. All in all I followed my doctor's recommendations about relaxing and it worked like a charm.
I underwent a cystoscopy last week at the VA . It was performed with a flexible cystoscope, not a rigid scope.. The nurse inserted some lube with lidocaine . After a few minutes the doctor inserted the scope slowly into the urethra. I watched the video as the scope went through the urethra, prostate and into the bladder. The doctor then filled the bladder with clear saline solution and moved the camera around to see the inside of the bladder. It appeared that I had a tumor, possibly cancerous. A biopsy surgery is scheduled in two weeks to determine if cancer . The overall was procedure was uncomfortable but not extremely painful. The only painful part was when the scope passed into the bladder. The rest of time was uncomfortable especially when the bladder was filled with water. Made you want to pee withe scope inside you. Don't be afraid. Relax. You will get through it OK. It lasts about 10 minutes. Be thankful there is this technology which can save your life.
I watched your video today before my cystoscopy wondering what the hell to expect. I shouldn't have read the comments because they scared me more than anything. The procedure wasn't that bad. The worst part was when the doc pushed the scope through the prostate. The pain only lasted for a second.

My urologist told me his teacher required everyone in the class to have a cystoscopy done twofold to learn how to do it and to know what it feels like.

I've had 2 cystoscopies while awake and they were both horrible. Definitely a 10 out of 10! I especially like how doctors describe the cable as being so thin now... it sure felt like a very large diameter to me!

Urethrae come in different sizes due to genetics. My half brother had bed wetting issue as preteen and had to undergo a procedure to make his larger to solve that issue. Like most things with the body there is variation in sizes. And yes it was not comfortable for me, but it was bearable. I can see that some would be very painful and others no pain at all. Pretty sure the same would apply to catheters.
😡I had 1 done years ago, 4 blood in the urine & kidney stones. My scope was not a "small diameter"..it was the thickness of a magic marker!Sedated...but when I woke up IT WAS HORRIBLY PAINFUL! Most likely because they dislodged the stone & it passed. I was passing blood for 2 weeks. It was 1 of the most Painful Experiences I've Ever Had...& I've had ALOT of Pain in my Life!😭👎
I’ve had a lot of painful procedures in my life time and I can say on a scale of 1 to 10 this is a a 100. The last time I told the doctor do what you have to do to numb things up after several shot numbing locations he started, his raised eyebrow and my involuntary pain response was wow you really can feel that.
I have a cystoscope every 3-6 months for bladder cancer. This has been going on for 5 years now. Only one has been uncomfortable because they had to really fill the bladder. None have been painful and only a little pain on the first urination after the procedure. Make sure they use numbing cream prior to the procedure. I had one Urologist that didn’t and he is no longer my Urologist, just saying. I’ve been awake for all my procedures and returned to normal daily activity as soon as the procedure is done. I’m in my 50s and fortunately have not had any issues, though I don’t look forward to my 6 month appointments I don’t dread them either.
After reading comments here I was terrified about my procedure yesterday almost to the point of cancelling. However, I went for it and to my relief it was nowhere near as painful as expected. I must have had a very good surgeon and nursing staff. The worst part was when I felt a cold stinging sensation on application of the gel but hardly felt the insertion of the scope only slight discomfort on it passing the prostate and it was all over in a few minutes. I relaxed my pelvic area, closed my eyes and wiggled my toes during the process and this helped a lot. The worst part was the stinging feeling when urinating afterwards for the full day which subsides. I hope my experience helps alleviates any fears for anyone undergoing this procedure and I would describe it as more uncomfortable than painful.

No, it was more uncomfortable with the gel and some pain on scope passing the prostate. Maybe I had a good surgeon and it was over in minutes. Thank you Dr. Rena, I am not sure why my Urologist's office staff seem to have difficulty answering even the most simple questions patient's ask. Your explanation was helpful, and covered the basics most people are likely interested in knowing; not least the amount of pain involved. After having an in office uterine biopsy, I have a justifiable mistrust of medical staff who fail to be transparent. So, I really appreciate down to earth, straight forward approach.
I have had many cystoscopies, i am a male, when there is plenty of numbing jelly used it is uncomfotable but not painful. However for a couple of hours after procedure i have the feeling I need to pee. Even though bladder is empty. For me the procedure has become routine. Almost 5 yrs cancer free.

I had a cystoscopy done yesterday. Like many of you I was dreading it and wishing they would put me under. But it was mostly a big nothing burger. Very minor discomfort and it stung when I urinated the first few times but 18 hours later no pain at all. I got to look inside at the mass in my bladder and I'm scheduled for surgery in 2 weeks but the cystoscopy was no big deal. Like Dr. Malik says you can do this and you will be fine!

I went for my follow up 2 weeks later and figured now what. Chemo? Radiation? I was told that because it was caught early none of that was necessary. Just follow up every few months and see how it goes.
My cystoscopy may or may not have saved my life but it definitely improved my chances.

@@MyPetGoat2U I cannot talk to my Urologist he has the appointment desk to see his PA. Nurses answer all of my questions. Urology Specialists of the Carolinas is the busiest in the US. Appointments are set out 3 months or longer. I hate it but they are the one that diagnosed me with Prostate Cancer so have to stick with them.
I had a cystoscopy at age 29. It was by far the worst pain I have ever experienced in my life. The painful part is when they push it through to the bladder. In that moment my entire body from head to toe was immediately engulfed in what I can only describe as hellfire. It lasted for 3-5 minutes and I felt this extreme pain for the entire procedure until they pulled it out. Any person or doctor that says otherwise is bullshitting you. The camera is not thin. I (regrettably) looked over at the camera before they put it in and I swear to god that thing was the size of a spark plug. Never again.
A proceedure at gyni was unbearable,so I insisted on anaesthesia for cystoscopy. I coudn`t face any more pain. I also have severe osteo arthritis causing pain everyday. The procedure with Cystoscopy was a dream compared to the Gyni hellfire. Don`t remember a thing and woke up ,no pain and felt on top of the world.

I don’t know what you experienced and you don’t know what I experienced so saying that I am a bullshitter is bullshit. Pain for me was a 5 tops and was very brief. If you have symptoms that need to be checked do it. I have a very close friend walking around minus a bladder and prostate and has a bag glued to his abdomen. A cystoscopy is a picnic compared to what he has endured. Ask for sedative or whatever it takes just do it. Stop scaring people away from a simple procedure that might save their life.

Interesting, I've had two of these in the last 10 years and I agree with the experience being described. It was more discomfort than pain. Perhaps it depends on the type.. Rigid or flexible? Mine were the later.
in my case what bothers me is the prepping when they put that thing that injects the numbing gel.after that is easier at least for me. not everybody is the same.
Hell yes, that numbing gel contains an irritating chemical, and it has an acidic PH. It only hurts for about five seconds when they squirt the gel into the urethra, but it is quite uncomfortable. And there is conflicting Data from various studies, as to how effective the numbing Gel is. Some people would say it hurts bad enough putting in the gel, that they would rather not have the numbing gel. But of course whether you have the numbing gel or not, you would still need a lubricating gel, which does not contain the irritating chemical but the numbing Gel does. Anyway, do your homework as a patient to find out what this procedure is all about for you. Ask questions, be informed, as to your options, for pain relief. Especially if you have a big prostate.

Great video, and much appreciated. I've got a cystoscopy scheduled for a few days from now and as you can imagine I'm a bit nervous. This has eased my anxiety about it. I'll follow up after the procedure for an assessment of the discomfort. ***UPDATE*** Had my cystoscopy today. I've experienced some high levels of pain in the past, so I knew it would be okay, but I was certainly nervous going into it. For me it was a non-event. There was no pain at all and just a bit of mild discomfort. When the numbing gel was inserted, I'm not sure how it was done, but it felt as though it was forced into the urethra through a syringe. That felt a bit weird, but not painful. Dr came in 10 minutes later. I chose not to look at the camera beforehand as I did not want to unnecessarily increase my anxiety. Of course I felt the camera being inserted but there was not any pain involved at all. Just an unusual sensation that was mildly uncomfortable. I think it was more mentally uncomfortable than anything physical. Before I knew it he was in the bladder and looking around. Very slight pressure, still no pain. As for the removal of the camera, I did not feel it at all. The actual procedure was over very quickly. The Dr. explained there might be a bit of blood in my urine afterwards, which made me apprehensive to urinate. So far that has not been the case, though there is just a slight discomfort like a mild burning sensation.

I had a cystoscopy prior to a Rezum. I don’t know which was more painful. They were both 10 on the pain scale. I was in agony as the Dr. Literally rammed the scope into my bladder. Dr. Malik, you say no patient has asked you to stop the procedure. Sometimes a patient is in such agony, they only want it to be over. You are willing to endure the searing pain (hopefully) knowing it will be over soon. Don’t use that phrase, “not asking you to stop” as reasoning there isn’t pain.
It was incredibly painful…….. and that was with 5 mg diazepam on board.
My dr actually said that most pain will come from the final push through. You either have to relax or he will just apply pressure until the muscle can't fight anymore, which is normally 30 seconds on everyone. No, my pain was mostly upon entry. Lasted until it was out. 10 out of 10 through the whole process.
I have had a bladder cancer (non-aggressive, thank God) removed, followed by a cystoscopy to remove a stent between my bladder and right kidney, which was intense, but as you say, pretty quick. I've since had six weeks of mitomycin treatments, a cystoscopy that determined that I'm at present cancer free, and just had my first of three follow up mitomycin treatments, and then another cystoscopy in June. What a ride!! Thanks for this, Dr. Malik...I'm blessed that I have an amazing urologist and staff, and I appreciate all that you do!!

I am 71, male had the cystoscopy to remove a stent due to a kidney stone. Urologist said it would be quick and a bit uncomfortable but local anesthesia would be fine. I trusted him. Big mistake. I did my research and many people comment about minimal pain and I consider myself pain tolerant but no masochist.
Nurse inserted the tube with initial numbing which was slightly uncomfortable. They waited a good five minutes. The cystoscope tube was lubricated as well. Urologist started the initial insertion; for me it was similar to having the needle inserted for an IV -- a pinch then over with. I did have some "ooos and ahs" as the scope moved along but was fine. Then we hit the sphincter muscles before the prostate and all hell broke loose. It was clearly a 10/10 pain level, I was at the end of the my rope. Assistant did tell me to breathe which did help. And eventually the pain stopped. I think this was about four seconds. They then grabbed the stent and pulled it out which did not hurt and all was fine. My body did not deal well with the stent and just getting the stent out of me was relief. The whole procedure was short about two minutes (I think). Do I want to repeat it again - "hell no" but since the pain and whole procedure is short I think it is better than general anesthesia. I do agree with an earlier comment that urologist like to cause pain. With a stent inside me and four weeks of constant pain and urologist would only say take over the counter and I would say it is not working and then never going over with me the pain I would experience during the stent removal was horrible especially when he knew I was the kind of person that liked to know what was going on which allows me to mentally prepare for something. I was glad to hear that it is a small minority that have the overwhelming pain and now don't have to ___ my urologist for the pain he has put me through and when he kept telling me that all his other patients were not experiencing the pain that I did. This helped. Thanks.
They all lie. I put hundreds of catheters in men years ago. If they have a swollen prostate, it hurts like a son-of-a-bitch! It is not Rare! They don’t want to give you adequate pain medication because of the overreaction to the so called opiate epidemic. It is BullShit! Find a urologist who cares.
Payne is a very idiosyncratic phenomenon, but I think you’re Payne was greater than what’s covered in the video because you were having a procedure. It wasn’t just a simple CYSTOSCOPY.

I've had this procedure done, and it was way more mental concerning than physically painful. I rate it a 2 out of 10. The only discomfort was when passing through the prostate. But it was all worth it to find out everything was ok. Thanks so much for all your work and effort. I find so many of your topics SO relevant. Please keep it up, I appreciate it.
Had two cystoscopies several years ago. While it’s not fun, the anticipation was worse than the actual procedure. The only part that I would classify as uncomfortable was when the scope was passed beyond the prostate into the bladder. If there’s nothing wrong, the procedure doesn’t take more than a couple of minutes. Definitely not worth it to have general anesthesia for this. Just relax and it will be over before you know it.
I would rate this procedure similar to yourself. Urologist used numbing gel and a flexible cystoscope. Bit uncomfortable in the beginning, but the procedure itself lasted less than two minutes. Typical burning sensation when peeing post procedure but that didn't seem to last very long either.
As for the burning sensation when peeing after the procedure, patients can and probably should, purchase an over the counter pill
Called AZO, also known as, Pyridium, which is supposed to offer a soothing effect to the urethra. Be advised that the medicine turns your urine kind of bright orange. Anyway, this medicine should at least help with the burning sensation. Secondly, if the male patient has a very large prostate, this procedure can be extremely painful. Passing that scope through the part of the
urethra that runs through the prostate, Just before the bladder, can hurt like there's no tomorrow due to the narrowing of the urethra caused by the large prostate pressing on it.
So, if you know before going into this procedure that you have a large prostate, be forewarned! This procedure is not going to be any walk in the park. And while that pain may last for less than 10 seconds, you will remember it.
Just saying. The so called numbing gel likely is not going to reach that part of the urethra. And speaking of the numbing gel, it is quite uncomfortable for about five seconds after they squirt it in there. And by all means, make them wait at least five minutes after putting the numbing gel in so it has a chance to at least try and take affect. The medical team may well want to start the procedure right away after putting the numbing gel in but that is not the proper way to do it. I have read several studies on this topic and it is very important to wait at least five minutes. Don't let them rush you, the patient is in charge in so far as they are the one who will be experiencing the pain if the numbing gel isn't given a chance to at least help. Lastly, it's pretty easy to find a dentist who will give you some level of sedation for a simple cleaning of your teeth, but yet various urologists, based on what I have read, I think it is OK to inflict this level of pain on a patient without any kind of sedation except for the topical numbing gel. It's crazy.

The pain I would rate a 4 but very brief, I have felt more pain banging a knuckle when a wrench slipped. Burned a bit for the next 2 urinations. Maybe some Docs have better techniques than others or because of my age 61, it just wasn't that bad. BTW I have absolutely no reason to BULLSHIT anyone.
Discomfort is a serious understatement as I had to have a double cystoscopy when I was a kid. In short, it felt like a flexible needle forcing its way inside and the pain was an 11/10.
I then heard my family doctor had become furious upon hearing the procedure and insisted it wasn't necessary. Two days of painful peeing. But this was back in the 90s, I hope things have changed to make the procedure less painful.

I’m a female and was told by the doctor and many others that it doesn’t hurt and just uncomfortable, it did hurt!!! A LOT!! It was more bearable once it was fully in, worst pain was it going on but it still hurt pretty bad! I think that people who says that it doesn’t hurt are woman who already had a baby, cause everyone who said it didn’t hurt to me were moms. Just had this done today. Was very quick and painless with some pressure. Not bad just had to breathe like I was lifting weights. The doctor guided me through the whole thing. Was nowhere near what I feared it would be.
I had the same experience last week. Really quick and painless. After seeing this video and reading the comments it makes me wonder did my doctor do the procedure right or should I get a second opinion because he didn’t fill my bladder up the way this doctors is describing.

@@vernonparrish7066 it was Chesapeake Urology in Maryland. I have an uncle in Connecticut who recently had it done and he also said it didn’t hurt but he had no interest in looking at the monitor when the doctor was trying to show him what his issue was.

Very thorough and informative review Dr. Malik! As a bladder cancer survivor now for 6 plus years, I've been through this twice per year during the first five years and now just once per year! I've found that numbing jelly can help, but it's a quick procedure and the worst symptoms are the very next time you have to empty the bladder. They always issue five antibiotics and that helps insure you don't get any other infections.

I've had a cystoscopy done when I was 22 and although I've undergone various endoscopies, including colonoscopy and two gastroscopies while I was completely awake and without any sedation, I have to say that this one was by far the most painful. Turned out I had a significant urethral narrowing for which I had to go through a urethrectomy while awake with topical anesthesia which I also felt and all I gotta say is that I normally had to go back for an evaluation and I have been putting that off for years now. I think my narrowing has come back now but I am not willing at all to go back for another cystoscopy. My doctor said it hurts much more with male patients but any doctor who tells you that it won't hurt, they are plain bullshiting you.😬💯

Please, do not let ANYONE tell you that a cystoscopy is a "little uncomfortable." The pain was so intense that I almost fainted; I had to lie on my back on the cold floor with my feet elevated afterwards to prevent myself from passing out. It felt as though some little gremlin was inside of me with a dagger, stabbing me from the inside. The reason that no urologist will tell you this in advance is because if they did, you either (a) would not get the procedure done, with possible risks to your health, or (b) you would ask to be put under general anesthesia. Option B would make a cystoscopy an utterly impossible and incredibly expensive task and it would cease to be a diagnostic tool. I will be going under general anesthesia for my Urolift.

@@Shield954 They do most of these routinely in the clinical exam rooms; when "sedation" is used, the procedure is done in the operating room (since an anesthesiologist is required) such as for TURBT, Lithotripsy, kidney stents, etc.
Thanks for the video! I don't remember a great deal of pain. The main thing that I remember is that my bladder felt full even after I'd emptied it after the procedure.
I had rigid cystoscopy with only local numbing gel last week. It was the most painfull and embarrassing thing that ever happened to me. PAIN 11/10!!!!
I'm a 22 year old, healthy male. So i really can't even imagine what pain would it be if i would actually have something wrong with my urethra or blader. Considering that I had it done at the university hospital, I was also so lucky to have 3 medical students (all female) present for this procedure. So basically it was about 10-15 minutes of being fully naked with spread legs screaming in unimaginable excruciating pain in a room full of people (again females) around my age :)

When I was facing the need to have one, I was given the option to be put under general anesthesia. I asked my urologist what he would choose. He said general anesthesia for sure.
They never asked me, when I had it done. When I was in my 20’s.
I had one done prior to a Rezum procedure. I have a Hank Hill narrow urethra and it was definitely a 8-9 on the pain scale. And I developed an infection afterwards. Needless to say that when I went for the Rezume, I was unable to stand the pain and had the Doctor stop. I'm no Sally when it comes to pain usually but this was more than I could bare.
My Heart bypass surgery wasn't as bad.

Very mixed experiences on this video, which I knew it would be!
Anything to do with sticking something *INTO* the urethra is a masive hell fucking no! Why any sane person would choose this torture instead of just having it done under general anasthetic is beyond me. And I’m someone with a generally high pain threshold saying this.
Nope! Im not stupid. I aint falling for doctors lying saying it aint that bad. We BOTH know it’s unbearable!

I'd definitely give it at least a 5 on the pain scale. It didn't help that I had a Dr that told me to "suck it up" very hatefully either. I wish I could find a urologist like you Dr. Rena!

I have to go next week to see a urologist and I'm scared s***less. Been deployed, shot at, through aerobatic flight training, etc and never been more freaked out than this lol.

I had the flexible kind a couple years ago. I found it incredibly interesting to watch on the screen. I think I was too engrossed in watching it to pay much attention to the pain. I would rate it as much better than a dental visit.

I’ve had several cystoscopies and whilst uncomfortable and psychologically worrying it’s not that bad. I would certainly take a Xanax or similar beforehand if offered. Pain wise I would rank it around 4-5, definitely not a 2-3. The burning on void is afterwards which lasts around 24 hours is also unpleasant.
I concur. I seriously doubt that most people consider this pain level to be 2-3 on a scale of 1 to 10. The number of people who weigh in on this message board, Stating otherwise, is evidence that it is no walk in the park for a lot of people who undergo this procedure while fully awake. More people should probably be given Xanax or something like that at least a day or two leading up to the procedure because it can be extremely psychologically worrying to be scheduled for this scary procedure. It sounds barbaric, and for some patients, it is. Not trying to worry anybody, but do your homework and be informed, as to your options regarding sedation, pain relief, etc.

I would say my pain level was a 10 out of 10. Ridiculous to indicate an expected pain level of 2-3. This was the most painful and miserable medical procedure of my life.
I will be having one soon, and the thought of ANYTHING going into my penis totally creeps me out. It will also be humiliating.
I have had many and the pain is barely a 1 and only for a second or so.

Read a lot of the comments and realized I was extremely lucky. No pain at all. Little burning when the nurse injected the Lidocaine before the procedure but other than that no discomfort. More anxiety than anything else. Fortunately for me the doc said I had the prostate of a 25 year old. I’m 73. So no difficulty inserting the cystoscope past the prostate. Diagnosed with a neurobiology bladder condition. Been doing intermittent self cath now for 6 years.
You didn't consider a suprapubic catheter instead of doing catheterization by yourself?
I found it a painless and quick procedure . I watched it on screen which was fascinating. 65 year old lady here (UK).
Mine was a flexible cystoscopy.
I’ve had several of these. My pain level was much higher, especially when they added the fluid….Doc says…”you might feel some PRESSURE” yikes! Difference between slight pressure and 1000 psi. Only saving grace (this was to surveille for cancer) was it didn’t last long…seconds really. I have kidney stones, so I anticipate more of these in the future.
I had a cystoscopy and I felt it go through my prostate and when it went into my bladder. Uncomfortable but no pain. Then I had a vasectomy right afterwards. No problems with either procedure. This is a good video of the procedure.
Same as a few of my friends, sure it also depends what person you have doing it as well!
If you have to get a cystoscopy, I highly recommend doing it under sedation. Pain is usually after with urinating. Take Tylenol for pain
Just had this procedure yesterday. Incredible jolt of level 6-8 pain for about 3-5 seconds when they fill up your urethra with lidocaine gel to (ironically) prevent pain as they insert the scope. Several seconds after inserting the lidocaine they inserted the scope itself, which was pretty painless; only about a level 1, obviously because of the lidocaine. Another jolt of level 6-8 pain (very similar in quality to earlier) for 2-3 seconds when they go through the prostate. Once the scope is in the bladder pain level returns to about 1. Mostly it's the idea of the whole thing that is so disturbing - one wrong move and yikes! Despite there being next to no pain as they looked around, I was still sweating bullets and feeling a little nauseous. When it was finally time to remove the scope there was some pain, maybe level 3-4, but only for a split second because they remove the scope a lot faster than they insert it. If you must have this procedure, my advice is to NOT look at the scope before and certainly not during the procedure. You don't want to see the girth of this thing, trust me. But don't blow off getting the procedure either, as it's very informative and could save your life.
This was exactly my experience. I would also add the pain when urinating after you get home is pretty significant as well. I could urinate, but had to psyche myself up, and bite down on something.
After reading some of the comments, I was a little freaked out about have this done. I had mine today and was so happy that it was pretty much painless.
I had 8/10 level of pain. The numbing gel which they used liberally seemed to not help at all. The size of the tube used actually tore the end of the urethra at the pens tip.
The numbing jel didn't work for me either. The procedure stopped in the middle as i could not tolerate the pain. They say that they will carry out this procedure with anesthesia for me.
Having this procedure done tomorrow and I have been stressing for several days now. This makes me feel a little more are ease.
This was the worst procedure I have ever had. I had this done at a major hospital and not only was it painful but the bed side manner was inexcusable ! Believe me, I understand they do this all day everyday but the experience was new to me. Laying naked on a table for 30 minutes waiting for the doctor to come in. Several nurses and Associates coming and going the whole time and you're on exhibition. This table has stirrups so your legs are up in the air bent backwards and you are not I repeat not coverd up while waiting. You mentioned a small percentage experience level 5 or higher pain, I find that hard to believe!
I have had many types of tests in the past but this one is inhuman.. Next time knock me out
It is outrageous that they have no regard for your sense of privacy and human dignity. Yes, I know they do this all day routinely, but comon now! Show a little humanity! I never had this procedure, but I know very well how certain offices operate. I personally experienced humiliation at a dermatologist when the female doctor had me first fully strip --- then she left the room and I do believe she instructed a couple of colleagues (female non-doctors such as secretaries) to "happen" to enter the room for a "free good look". In no way do I believe that should be accepted as normal protocol in a doctor's office. Would these same office workers like to be fully naked for us males to see fully on display for a "free good look"???

What health care professionals need to understand. The comment ive seen it all before does not give me encouragement. It makes me feel like an inconvenience to worry about what they seem as a trivial thing. Why on earth does there have to be so many people in the room at the same time as I'm lying naked. Then to be told that I'm blocking the pathway for students learning for not letting them in the room. The doc said they won't be performing just watching. I'm trembling at the thought of getting undressed at this point. All hospital departments are the same no thought to as to what the patient is mentally going through or why we feel like this to being with.
Things will never change the big money is in womens health so offices are set up to make them comfortable lots of female nurses.Even urologist who largely deal with males usually have female nurses who handle your penis during procedures mostly like it is a gear stick.Also leave males naked and exposed for long periods of time whilst women staff members walk in and out.All the time they say don’t worry they see that all the time.Try using that statement at woman’s pap or breast exams

Had this procedure and it was no big deal. I’m female. What I am seeing in the comments is this procedure is much more painful for guys. Maybe there needs to be a different pre-procedure protocol for males. My brother had this done along with Rezum. He was traumatized by both. It’s something the urology community needs to think about. I think for men it's just a very strange sensation inside the penis they never felt before I had experience with catheters before I got the scope so it was lot like inserting one and removing it.
And more embarrassing males are not as used to being naked from waist down as women are we do not have to display our genitalia on regular basis as women do.Also when they do get us in stirrups with penis hanging out there always seems to be young females hanging around taking notes or doing some clerical task
I wouldn't do it at all. And if I absolutely had to it is being done with regional anesthesia (paralyzed from the waist down ). That is not something I am gonna wanna feel at all. Also male only don't care what they have seen I probably have seen worse and been through worse.

I was nervous about it so asked for it under anesthesia. They called and scheduled it for 1 mo from now. As a retired RN i know time is of essence so I called and said I would do it w/o anesthesia. It is 5 days. Still nervous. Your words and explanation are comforting. Thank you. 🙏🙏
How was it?
10 out of 10 pain! I was told not to urinate before. I was not given a numbing anything. There was lube on the tube which was inserted immediately after I laid back. I know pain and handle it better than most. This was a pain I've never felt or imagined. Construction, mechanic, smashed finger tips, broken bicep smacked my shoulder, kicked in the testicles, welding cross bracing on joist and being zapped in the testicles while straddling a wet joist. Stepping on Lego's wrong!! Nothing compares to the pain I felt immediately upon entry. That burning, that uncomfortable pressure feeling.. yup, felt that too. Almost like a warm friction burn as it slid in. No, I'm not talking about that pain. I'm talking about the feeling of my penis being ripped in half from the inside out, at the tip. The very tip! The pee hole!! A feeling like two of my wifes acrylic fingernails were pinching the very tip of it as hard as they could squeeze. From the time he entered until it was finished. All while he was speaking over me to tell me it was my fault and I needed to relax. All in about the longest, most painful, maybe 90 seconds, of my whole life. While in there he blasted apart some scar tissue he found in the urethra, in hopes that it will heal better this time and no longer restrict my urine flow. The blast was a whole different pain. Still nothing compared to point of entry. I'm sure everyone in the whole office heard my screaming and the pain I went through. The words I was screaming that I don't use in front of my mother or my kids. It was an embarrassment to walk out of there. Today is 3 days and I think the blood has stopped. I quit the azo yesterday and it only hurts towards the end of urination. If I had known, I would have went with restricted urine flow for the rest on my life. It wasn't that bad!! Lots more details, but that's the most of it. If you feel there's no way to avoid having it done, take the meds! It's not worth finding out if you are in the 5 percent.
It's been 2 months, 3 antibiotics for infection that wouldn't go away, multiple doctors appointments, hurt to urinate for almost 6 weeks after. Still hurts today to sit on a hard surface too fast. Weird feeling in my taint shoots another weird feeling all the way through to the end on my penis like squeezing a hot dog balloon and it shooting out the end of your fist. If you think you have a small penis hole, you prolly do. Trust me. If I had known, I may have tried some stretching or something. Idk. It's a nightmare that I wish I never would've went to the Dr for. A feeling I will never forget and a scary feeling if my heart was gonna allow me to make it through the procedure without stopping. Never again!!

Yes, worst pain ever!! Rough Doctor in Columbus, Ohio, now moved to Florida. Probably got ran out of town. They could have use a child’s size scope on this little guy but today I would never do it again even with a smaller scope. Other guys have given the same pain descriptive experience. I can not explain how painful it was. NEVER AGAIN

Everyone is different but this is my genuine experience - Female living in Scottish Borders. Just back from having my cystoscopy at our local hospital. Was dreading it but nurses and doctor put me at ease. They were also very good at retaining your modesty as best as they could. Doctor numbed the area and, yes you can feel the scope going in, but to be honest, for me it wasn't painful at all and the procedure was over in less than 5 minutes. You do feel the need to pee afterwards because you've been scoped and they've put a solution into you and it is a bit "burny" the first few times you go to the toilet. Just remember to keep drinking plenty of water for a few days. I think it is slightly different for men and women. Women only have approx 4cm in through the urethra before the bladder whereas a man has approx 15-20cm and different angled pathway on the journey to the bladder. As I say, everyone is different and I'm sure doctors carrying out the procedure are different too but my experience was fine.
Can general anesthesia be applied for that?

I had a cystoscopy about a month ago. It wasn't that bad. They didn't even use numbing gel. Fortunately, my doctor used a flexible scope. The worst part was insertion which was very briefly uncomfortable. After that, it was no big deal. The vaginal exam my doctor did afterward was more painfu.l The medical assistant said I did well for my first one. I wasn't nervous, so I'm sure that was helpful. I know some women will not have the same type of experience I had, even with a flexible scope. I want people, especially women to know they may have a positive experience. I don't have chronic pelvic pain, but pelvic exams are always somewhat painful for me as I have a tight pelvic floor. And even with that, my cystoscopy experience was positive. I can only speak from my own experience, but often people are more likely to share negative experiences than positive ones. So, hopefully this helps encourage people who may have seen overwhelmingly negative comments.

I just returned from a cystoscopy procedure performed under local anesthesia. The procedure was completely painless, performed quickly and efficiently for diagnostic purposes. The whole thing took a maximum of 10 minutes. Despite natural fears, they turned out to be completely unjustified. I felt comfortable during the procedure. Maybe it's a matter of the doctor's skill. So look for one from a recommendation.
I had a cystoscope about 3 weeks ago and I’ll be honest with you, I was so nervous that my Urologist gave me a nerve pill before the procedure. The pain was probably about a 4-5 out of 10. It wasn’t as bad as I had built it up in my mind to be, but it still hurt. Hope I never have to have another one
What nerve pill? And how long before pill took effect?
I have to have one sometime soon, as I am getting a kidney transplant sometime in the near future, and have some other issues with my prostate. I am absolutely freaking out - even the thought of putting something in my penis creeps me out. I think it is an unnecessary procedure in my case, and am still fighting not to have it done.
@thethecommander When he said "nerve pill", I figured he meant an opioid, not anxiety meds, lol. Anxiety meds will reduce stress, but it won't reduce pain.

I ate a Xanax bar and a ten mg oxycodone 90 mins before last procedure. I was completely baked and it was like trying to put out a forest fire with a squirt gun. Still 10/10.
I had this done 7 days ago (female). I was given no instructions ahead of time (or after), and I expected to go to work right after since I was told it would be a "simple 10 minute office procedure." Maybe I had a unprofessional doctor, but no numbing gel was used to my knowledge. The Dr could not fit the scope in my urethra due to post menopausal atrophy, so he decided to use expanding tubes twice. I've never cried from pain in my life even giving birth twice, but I cried through this procedure. I could not pee at all when I got home and had to take AZO max strength. The pain was worse than any UTI and lasted 3 days. The Dr also gave me a pill prior to the procedure and did not tell me what it was. I assumed it was pain meds. I researched the pill when I got home and discovered it was antibiotics. What doctor gives only 2 pills of antibiotics?? All that does is create superbugs. Be your own advocate. I usually research everything to death, but let my guard down because the Dr said it was a simple procedure. Continue to ask questions and find out what if the procedure does not go as expected.
You said it. You're right about their poor use of the antibiotics creating super bugs. And if they don't want to answer your questions before doing the procedure, then run away. They don't have your best interests in mind.
I ended up finding out that kidney stones was mimicking my uti feelings. I've passed about 8 since that exam. Using estradiol vaginal pill (or cream) helps soften vaginal and urethral tissues. So that might help the cystoscopy procedure (if you are premenopausal or menopausal). @@samiethecat2620
ive had multiple surgeries: acl repair, a dental implant, a tunneling surgery to relieve pressure from mrsa as well as many others. ive been through extreme rhabdo from overuse of my muscles (cpk of 75000), ive had serious hemipalegic migraines, i also had major burns accross a large portion of my stomach from a powder explosion. this test was more painful then all of them combined. when they went past the prostate i literally screamed. i know this isnt everyones experience but all of the resources ive seen are saying it doesnt hurt and honestly they are totally incorrect

I am so glad that there are a few people who experienced what I did today. It was insane and I groaned very loudly - I didn't wanna freak out the waiting room. My back involuntarily arched like I was John Rambo being electrocuted on that metal bed in Rambo III.
I watched this video and comments last night and got very worried. I just had a Flexible
Cystoscopy exam where they used a numbing gel and I barely felt anything. It also lasted for all of 2 or 3 minutes. Everyone’s experience will be different, however, I am surprised that some people experienced such severe pain.
I had one done a couple years ago, and honestly, I felt brief discomfort and pressure, but it wasn't as bad as I thought it was going to be. It did hurt a little bit when I had to pee afterwards...but they gave me an antibiotic just as a precaution to prevent any UTI or infection, and that was it!
**Deidre Haye,** I had mine last year. The urologist had my legs in the GYN-type device as if I was about to give birth...LOL. He had a female assistant and the rushing of the water stimulated my prostate (I should have been forewarned). It caused an involuntary erection. There I was, embarrassed to no end when I asked, "Why is this happening, Doctor?" His reply was that it's completely normal. - STILL: I was embarrassed!

This procedure was hands down the most painful procedure I’ve ever experienced and I have had other procedures that are supposed to be painful such as teeth pulled with out numbing. The fact that there is no local anesthesia or numbing is crazy. While I believe this is a necessary procedure and has probably saved many lives when dealing with urology issues, Buckle up Buttercup it’s going to hurt!
Had a flexible cystoscopy a few hours ago, here in the West of Ireland. Gel put in, followed seconds later by the cystoscope. Was not too hurtful at all. On a discomfort scale out of ten, I would give it a two, that’s all.
Even though my foreskin is tight, it slipped reasonably easily, the Doctor spent about 7~8 mins having a look around, and then removed it. The three nurses in attendance were very encouraging saying I was doing well, and to breath deeply, while smiling in a friendly manner, was more then helpful.
Main point, at least in my case. It was nowhere near as bad as the comments on RUclips make out. The sense of relief, and enormous weight off my shoulders, after many many months of dread, anticipation, and worry, is something else.
Enlarged prostrate 60cc was the result. Had I not read my medical notes, recently, that landed me in the emergency department in the first place, with kidney back pressure, fluid retention, AKI acute kidney and injury and uroseppis mainly caused by inability to urinate, and a stretched bladder, I would not have had this cystoscopy today, but I am very glad I did.
I would encourage anyone advised to have this procedure, To go ahead and get it done. Bit like going to the dentist, no one really wants to go, but you will be glad that you did, and stop reading and watching overly sensational videos on RUclips, and particularly reading the negative, over the top comments like I was doing, that may cause you refuse this procedure like I did, before it maybe too late for you.
I am just glad, and lucky that my problems ain’t stones, cancer or other issues, that can be hard enough to identify without such a procedure , but can be treated successfully if caught early enough. Good Luck .
Thank you, Dr. Malik. I'm having a cystoscopy in August. Your video has helped calm my anxiety a bit. The doctor said I have Microhematuria, so I have had a CT scan done, and next thing on the list is the cystoscopy. I appreciate the time you take doing your videos.
Her indication of minimal pain from the procedure, is ridiculous and not my experience. Horribly painful as it passes through the prostate, which she has no concept of. Personally I would look for a male urologist who has some empathy and experience with the procedure themselves.
Ask her if she employs male nurses to reduce the embarrassment to her mostly male patients

@@19530ashland im a male and I'm much more comfortable showing my junk to female nurses than to males I prefer to be in a room filled with female nurses looking at me undressed than to have male nurses looking at me undressed.
@@ellitestar because they wear scrubs doesn’t mean they are nurses.The problems is male patients are left exposed in situations that female patients aren’t.If individuals are comfortable with being exposed to females is not the issue it is lack of respect for males in medical situations

I had three until now from 3 different doctors. The procedure was always 1st inject a anaesthetic gel into the uritrea. That burns a little bit. Then feel a medium pain when passing the close muscle. After removing the stethoscope I was bleeding one day during urination. This happens only at my last cystoscopy. But always I had horrible pain during urination a couple of days. After the 1st and 2nd I got Antibiotikum as a prophylactic.
I had mine this morning. It was painless with some minor discomfort. It wasn't that bad. You people are over exagerrating in these comments. I was scared like I don't know what after reading these comments. My doctor Dr. Mac was so kind and put me at ease.Not a bad experience at all.😊

To those nervous: I just got mine done this morning. On a scale of 1-10, if I could give it a 0, I would. I felt a bit of pressure, but it definitely didn’t hurt and I was fine right after. It didn’t hurt much to urinate after either. The hardest part was swallowing the big antibiotic capsule beforehand. 😅

The procedure was nothing. The nurse put some lidocaine lubricant into my urethra and about five minutes later the doctor arrived. Mine was a flexible tube and I only felt pressure. It was mildly uncomfortable when they filled the bladder with fluid. I don’t think it took longer than 60 seconds. When the scope was removed a lot of liquid came out and onto the pad I was sitting on and then they had me dress and pee before I left. It wasn’t fun but if I needed another one tomorrow I wouldn’t care.

Can I request the use of a rigid scope instead of a flexible? I really would much rather be asleep, is it even possible to request one over the other?
A cystoscopy is VERY painful. Urologists ALWAYS play down the pain. I suggest getting stoned on Xanax before the procedure. Have someone drive you to and from the doctor's office. Some urologists are better than others, and it may be somewhat less painful, but it still hurts like hell. Think about it. The urethra was not designed to accommodate a medical instrument being shoved deep inside it. I really have to question why urologists are not more truthful about the pain.
Had this procedure done, painful, after urinating was very painful and some blood like you said, this lasted for about 2 days.They did it to determine the stricture> I wish there were doctors like you in my clinic and hospital.I had to deal with a stricture as well as an enlarged prostate which I believe was misdiagnosed prostatitis I am only 46 and every time I get the prostate checked it seems to be a normal size.

@@BallroomSceneNerd a history of UTIs slow stream , painful urination, discomfort after ejaculating.,even led to a bout of UR had to be catheterized, they put me on tamsulosin which I still take. Cause unknown they suspect it could be from the persistent UTIs. Had a scraping done with the tube recently but still have symptoms, next step maybe a uretheoplasty

I have had several cystoscopies following a diagnosis of stage 0 bladder cancer. It isn’t too bad…the only moment it hurts is when they penetrate the prostate…but that only lasts a few seconds.
I’ve had many cystoscopies and my urologist gave me some great advice that I’ve used every time since - wiggle your toes! It may seem silly but totally works.

My older Dad had a bunch done, even though he has no major urinary problems!

I'm a 75 year old male and first had a cystoscopy in 2010 when I had bladder cancer. I have probably been scoped 40 or 50 times. I have had a handful of completely painless exams, but no excruciating or even very uncomfortable ones, either. Mostly, there is a jab or two somewhere along the way that is unpleasant, but in the grand scheme of things, a nothingburger. Several years ago my urologist switched to a scope where both he and I view what the scope sees on a screen in real time. From the point of view of my comfort, that is a very big development. Being able to watch the procedure is a distraction that calms or damps the occasional pinch.
Do you have a Neobladder, and were your prostate and seminal vesicles also removed?
I am an 39 y old male and had one last year. I was really scared(more when i saw the size of the instrument) but when it started i felt no pain what so ever. The only thing i felt was a strange (but good) sensation when it pushed past the prostate. The doctor was nice and explained allong the way. 2 weeks of anxiety all for nothing xD and the whole experiance was more interesting than anything else.
Had a flexible cystoscopy done a few weeks ago as a 20-something male presenting with enlarged prostate and bladder. Although the doctor said it would hurt more since i'm young, i'd say the pain was a max 5/10 (i'm bad with pain) after lidocaine injection and during insertion, and more discomfort than pain while it was in me. Surprisingly, I did not have any burning sensation while peeing, past some mild rawness on the first one immediately after surgery. I was back to my normal activities within an hour. Unfortunately, i'm going to have to go for a rigid cystoscope biopsy due to abnormal sightings.
Overall, make sure you're getting local anaesthesia injected into your urethra + lubricated scope and you should be fine. I'm shocked that some commenters here have gotten it without any.
I had two cystoscopies and both were 12 out of 10 on the pain scale. I did have BPH at the time but it was horrific.

I have BPH too and this procedure was horrific. Writhing in pain. Plus a few F words. Fortunately it is pretty fast.
A coworker had to have these done regularly for bladder issues. She said the pain was higher than 10 and needed help walking when leaving she would be in so much pain.
Without a doubt, the single most painful 30 seconds of my life. Felt like my penis was being ripped apart from the inside. Have had trouble urinating (difficulty starting and weak, dribbling stream) since I was in my twenties (now 64). Was told it was not a prostate issue but extremely tight muscles and sphincter that would not relax. This was apparently the cause of my extreme pain. As a side note, a daily, low dose of Cialis has helped my urinating significantly.
That's strange when I've had viagra I pee much better in the night.. I had a flexible cystoscopy and it was much thicker than they said I would be and was uncomfortable not painful but embarrassing because I was naked with 5 people in the room. Dempsey 95 ..... That's terrible that you had FIVE people in the room, thus making you feel understandably, embarrassed. This is procedure is normally a 2-person job .... the MD, plus one assistant, and worst case, MAYBE one extra person, so at most there would be perhaps THREE people needed but that's all. But FIVE people is way out of bounds for a flexible cystoscopy procedure. I have seen it done by the doctor alone, all by himself, with NO assistant, for the actual procedure. A patient should always be told who is in the room for such an invasive and embarrassing procedure, and why exactly they are there. The patient gets to make the rules .... because THEY are the one who is naked and fully exposed, feeling vulnerable, scared of the procedure, feeling pain and anxiety. To have EXTRA people in the room, who only add to the patient's overall level of discomfort, is BS and should never happen without the patient's expressed consent. That's crap.

Since you say you’ve been told the painfulness is due to tight muscles, you might be well served looking to Pelvic Floor Therapy. Maybe you suffered some kind of trauma and your subconscious mind would never unclench. It’s worth a try.

Putting the anaesthetic in the urethra beforehand (3/10) was more uncomfortable than the cystoscopy itself (2/10). Loved watching the visuals on the screen. Worst part was urination for 2-3 days after the procedure (4/10); burning sensation.
I wanted to give my anecdotal flexible cystoscopy experience from a male perspective in hopes that it might help someone suffering from anxiety about the procedure like I was.
The most painful part of the whole thing for me was the injection of the lidocaine gel into my urethra. This actually did feel almost like a slightly time extended needle prick to me but there is no needle involved. On a 1/10 pain scale I would however just give it a 2 or 2.5 at best. I may have winced a little bit from it but that really is about it.
Once the actual procedure began, I would describe it as just uncomfortable, not painful. I did not really even notice that the camera/scope was inserted until it reached the sphincter that it has to pass through near your prostate. I would liken the sensation of that happening to be the equivalent or less than the equivalent of a doctor’s finger palpating the prostate during a digital rectal exam even if you have an inflamed prostate like I do.
Once inside the bladder and the doc was evaluating it for any abnormalities, I really only had a feeling of “fullness” which was just uncomfortable and not painful at all.
I did not even feel the cystoscope being removed after he said all was clear.
Everyone’s anatomy is different though and it’s clear by the comments that some people have a hard time with this procedure. By no means either do I think I have any kind of enhanced pain tolerance to be able to withstand this procedure better than anyone else. If you however need to have the rigid cystoscopy, that is a different animal it seems and should be done under anesthesia which I believe is standard practice.
If your doctor recommends a flexible cystoscopy procedure be done in order to rule out any kind of disease including cancer, please do it. I am glad I did it now after stressing for weeks about it. The peace of mind now that I know I do not have bladder cancer was worth it for the price of a little bit of uncomfortableness of the course of what was less than 5 minutes and I would do it again with no stress involved.
I have had this as part of a kidney stone removed. I was under general anaesthesia; fortunately!!
Pain. Dr. Malik gets a score of 95% out of one-hundred for her use of the word pain versus "discomfort" or "uncomfortable."
Here are two descriptions of when something is "uncomfortable." At night sometimes a pillow gets too warm and to stop it from being uncomfortable, it is flipped to the cool side. If a dry cleaner puts heavy starch in a collar instead of light it is uncomfortable. Anything beyond that is painful.
Dr. Malik always followed up her use of discomfort by saying something like, "a little painful."
My own experience is that this procedure hurts. It is painful. But, as with many men, after they were done, I was very stoic. Being stoic and quiet afterward might have led the doctors to assume it didn't hurt. Only one doctor (out of three) confirmed my lack of pain. He said, "that wasn't so bad, was it?" I think I muttered something that could have been mistaken for agreeing with him. It didn't matter much because he was out of the room before I had much of a chance to form a thought and explain my experience.
All three of mine were done with a flexible scope. I understand that the pain is less with the flexible scope.
I've had Foley catheters inserted and removed. I've also had one straight cath following surgery when I had to pee but could not. I've only felt minor pain for a few seconds when a Foley is inserted. Removing a Foley cath is so fast that the tiny little bit of pain lasted less than a second. The straight cath hurt a bit more than the Foley.
Comparing a cystoscopy to a catheter, I would say the cystoscopy hurts at least 20 times more.
Aren’t I the lucky one. My pain level goes from zero to 100. As the scope goes past my prostate (straight scope, curved path), I typically want to rise up off the table and sink my fingernails into the ceiling (a la “ The Exorcist “). I have had dozens of these procedures (bladder cancer diagnosis, treatment and follow up) over the course of about 5 years. And the painful urination (passing razor blades) afterwards only lasts for about 2-3 days. Doctors seem to like to call this “some discomfort”, which translates to “I want to die”. Other than that, it’s a snap.

Question: (1.) Is there a particular type or kind of cystoscope that you recommend, either because of its efficacy, ease of use or patient comfort level? (2.) In instances where a HG bladder cancer has been detected, has not yet penetrated the muscle wall, and has responded very favorably to a full round of BCG instillations, how should patients address the possibility of recurrence, e.g., what is the best practices treatment for periodic cystoscopy exams? Also, are such patients ever, statistically, considered to be a very low risk or even ‘out of the woods’ altogether?

I had a rigid Cystoscophy. and had anaesthetic. I don`t do any pain nowadays since I had hellish experience with a Gyni camera x-ray. never again without an anaesthetic.
I had a ridged scope put in once. I think the urologist trying to teach me a lesson in order to get me to change behavior. It worked! Second worst thing ever done to me. When he had it in, he filled my bladder with saline. You go from no need to pee to the worst urge you’ve ever felt! Then he was moving it around to inspect my bladder. Felt like he was looking for targets.
Mine was horrible. First, no meds offered and I said I was a nervous wreck and no numbing. The doctor couldn’t find where to put it after 7 attempts then said maybe I had scar tissue. Out comes the large metal instrument to push, pull, scrap…unbelievable pain. The camera itself was no big deal after that. It was the metal tool that almost killed me!

I think the doctor should record the procedure. Once the scope was pushed passed the sphincter, I could look at the screen. I was just so focused on bracing myself for the pain and uncomfortable force. I would have liked to see the short procedure.
This was the most painful, horrific, traumatizing experience Ive ever had to endure. (45 year old male). The pain was beyond words. I have scarring from a previous infection and my urethra always feels sore and irritated already and this drove me over the edge. I have a very high pain threshold and this was an absolute 10/10 pain scale. It felt as if he was dragging a razor blade through my urethra. When he pushed through my urethral sphincter, i screamed so loud and begged him to stop the procedure.
These 2 minutes were the worst 2 minutes of my life. And the most painful part was trying to urinate 15 min later. Extreme shockwaves of pain. I might as well have been peeing fire. Bloody urine. Extreme level 10 burning pain. Cannot be described in words. Im absolutely traumatized by this experience. I couldnt even drive away from the hospital for an hour, just sitting there sobbing to myself. Its been 48 hours since the procedure and peeing still is a nightmare. Still bloody urine. Still level 10 pain whenever urinating. Im not even myself. Just in a daze. Emotions all over the place. Cant concentrate on anything. Random crying spells. This shit fucked me up dude.
My experience exactly. The pain that I experienced left me in tears and mental trauma for a year. I like Dr. Malik but her description of the pain during the procedure is bullish*t.
Have you taken phenazopyridine urinary pain relief tablets to lessen the pain? They are available OTC at drugstores. AZO

But I also think it depends on the doctor, because I've seen some videos on it and they act like they don't care where they are putting it, plus there have people who have gotten permanent ED from it, again I think it's how the doctor cares about where they're putting it and not shoving it through fast.

2-3 out 10? that doesn't seem right, coming from somebody who did a cystoscopy. maybe a 5-6 at very least?
I had a Cystoscopy done this morning and it took just about 10-15 minutes total. There was hardly any pain at all and I would rate that pain or discomfort at 2/10. My prostate was huge and was described by the doctor's assistant as 'Texas style'! After the procedure I urinated and just a little bit of blood came out. I kept drinking water during the day without any evidence of blood in the urine. All in all I followed my doctor's recommendations about relaxing and it worked like a charm.
I would never have one without anesthesia!!!
I underwent a cystoscopy last week at the VA . It was performed with a flexible cystoscope, not a rigid scope.. The nurse inserted some lube with lidocaine . After a few minutes the doctor inserted the scope slowly into the urethra. I watched the video as the scope went through the urethra, prostate and into the bladder. The doctor then filled the bladder with clear saline solution and moved the camera around to see the inside of the bladder. It appeared that I had a tumor, possibly cancerous. A biopsy surgery is scheduled in two weeks to determine if cancer . The overall was procedure was uncomfortable but not extremely painful. The only painful part was when the scope passed into the bladder. The rest of time was uncomfortable especially when the bladder was filled with water. Made you want to pee withe scope inside you. Don't be afraid. Relax. You will get through it OK. It lasts about 10 minutes.
Be thankful there is this technology which can save your life.
